Psychogenic Pruritus and Its Management.

Chronic itching can be frustrating for patients and providers, and patients are not always willing to accept that their psychiatric health has an impact on their skin. Psychogenic pruritus is defined as itch not related to dermatologic or systemic causes. When a patient presents with pruritus, regardless of the presumed cause, the standard work-up should include a thorough history, dermatologic examination, and laboratory examinations or biopsies as needed. If no medical source is found, the provider must work in partnership with the patient to explore other causes and that may include acknowl

Simultaneous occurrence of pyogenic granuloma at multiple sites associated with bone loss: Report of a rare case.

Pyogenic granuloma (PG) is a reactive hyperplasia of connective tissue that occurs in response to low-grade local irritation, traumatic injury, foreign materials, or hormonal causes and rarely associated with bone loss. Although solitary PG is a common lesion in the orofacial region, presence of multiple such lesions at the same time with associated bone loss is rare. In addition, repeated recurrences of such lesions sometimes frustrate the clinician. This article presents a rare case of simultaneous occurrence of PG at multiple sites associated with bone loss in a young female and its managem

Prostatitis: epidemiology of inflammation.

Approximately 8 million prostatitis-related outpatient visits per year have been reported worldwide. Only a small percentage presents with bacterial prostatitis. Chronic pelvic pain syndrome/prostatodynia is a frustrating disease for patients and caregivers. This paper is an attempt to analyze causes and types of inflammation and to determine if inflammation may have an impact on treatment and outcome. Is inflammation even a relevant factor in the diagnosis of chronic pelvic pain syndrome? Do we need to begin to look for different factors involved in the development of this often crippling dis

Molecular mycology: a genetic toolbox for Histoplasma capsulatum.

Research in medical mycology has traditionally been a mix of exciting biology and frustrating genetics, although the excitement has been steadily increasing as genetic obstacles have been successfully overcome. Now, a variety of fungal pathogens can be studied using molecular techniques derived from classical bacterial and yeast genetics, but with selective and strategic adaptations. Histoplasma capsulatum is the best-studied of the primary pathogens known as 'dimorphic' fungi, and tailored molecular genetic strategies are beginning to reveal a repertoire of genes and gene products intimately

Chronic nonprogressive headaches in children and adolescents.

Chronic nonprogressive headaches (CNPHA) are common in children and increase in frequency in adolescents. Features are usually, but not always, distinct from those of migraine. CNPHA have also been called chronic daily headaches, tension-type headaches, muscle contraction headaches, and psychogenic headaches. These headaches represent a diagnostic and therapeutic challenge to family physicians, pediatricians, and pediatric neurologists. The evaluation is time-consuming and the treatment frustrating and often unsuccessful. They are a significant cause of school absences. This review addresses t

Chronic mandibular dislocation: the role of non-surgical and surgical treatment.

Although the management of acute dislocations of the temporomandibular joint (TMJ) has not changed significantly in recent years, chronic dislocations continue to be treated by a variety of methods. Long-standing cases are the most difficult and frustrating to manage. This paper reports on four cases demonstrating the signs and symptoms associated with some forms of chronic. TMJ dislocations, and the difficulties encountered in the management of some of these conditions. An algorithm based on a critical review of the literature is proposed for the management of both acute and chronic TMJ condi

Hospital management of a patient with factitial dermatitis.

A 64-year-old male with a 40-year history of dermatologic disorders is presented. The patient carries diagnoses of neurodermatitis and suspected myosis fungoides on which has been superimposed a factitial dermatitis requiring increasingly extended hospitalizations. Given a very disturbed home life and personality style, the patient showed no incentive for change, and his family and caregivers became increasingly frustrated and distraught. An operant behavioral approach proved helpful to staff in standardizing care and setting realistic expectations. This led to a reversal of a downward course

RESULTS: • People in hospital report widespread frustration, feel that hospital environments are not conducive to getting/staying well, and face multiple barriers to leaving hospital. Without someone to fight for them, people struggle to overcome the inertia built into our current systems and processes. • Fr …

The often relatively high fragility and large extent of cooperative motion are found in the GET to derive from the often relatively large packing fru …

Approach to the Patient with Severe, Refractory Irritable Bowel Syndrome.

Defining which patients with irritable bowel syndrome (IBS) are "refractory" is a highly subjective undertaking. Duration of symptoms, sever … other practitioners looking for the elusive cure. A key component of this process is an increasing frustration and cynicism regarding the healthcare system and physicians in particular, which does no good for anyone involved. It is clearly critical for the physician dealing with a patient with IBS and a history of poor response to treatment to understand the …

Multidimensional aspects of dyspnea in obese patients referred for cardiopulmonary exercise testing.

We investigated the contributions of obesity on multidimensional aspects of dyspnea on exertion (DOE) in patients referred for clinical cardiopulmonary exercise testing (CPET). Ratings of perceived breathlessness (RPB, Borg scale 0-10) were collected in obese (BMI ≥ 30; n = 47) and nonobese (BMI ≤ 25; n = 27) patients during two (one lower: ∼30 W; and one higher: ∼50 W) 4-6 min constant load cycling bouts. Multidimensional dyspnea profiles (MDP) were collected in the final 26 obese and 14 nonobese patients of the sample. RPB was greater (p = 0.05) in obese (3.3 ± 2.2 vs 2.4 ± 1.4) at lower wor

Exercise and body image distress in overweight and obese women with polycystic ovary syndrome: a pilot investigation.

OBJECTIVE: The objectives of the present pilot study were to: (1) examine the prevalence of body image distress in overweight and obese women with polycystic ovary syndrome (PCOS); (2) assess the effects of a low-cost intervention in the form of a self-directed brisk walking program on body image distress; and (3) assess the level of participation, the feasibility of a larger study and the sample size required. METHODS: This was an observational study whereby volunteers acted as their own control. Thirty-five women with PCOS (mean age 29.26 +/- 7.57 years) with body mass index (BMI) > 25 kg/m
