OBJECTIVE: Intractable Meniere's disease has traditionally been treated surgically, with either labyrinthectomy, vestibular nerve section, or endolymphatic sac procedures. Another option is the use of intratympanic gentamicin to effect a 'gentamicin labyrinthectomy.' A single injection of gentamicin in the awake guinea-pig induces a transient blockade of the suppression by contralateral white noise of tone-pip evoked compound action potential (Smith et al., 1994). We report here the reversible blockade, in exactly the same experimental conditions, of the suppression of click-evoked otoacoustic emissions. These observations confirm the transient action of gentamicin at the medial efferent/outer hair cell synapse, likely via the reversible blockade of calcium channels observed in vitro (Dulon et al., 1989).

To evaluate the efficacy of ceftazidime monotherapy in the empirical treatment of febrile neutropenic patients, a randomized trial was performed of ceftazidime (C) vs. ceftazidime and gentamicin (C + G) with the addition of teicoplanin (T) for those patients with a long intravenous line-associated infection. Success without modification at the end of treatment was seen in 32.7% (C), 28.3% (C + G), 52.4% (C + T) and 65.2% (C + G + T). Monotherapy with ceftazidime is as effective as when used in combination with an aminoglycoside in neutropenia.

A case of Listeria monocytogenes endocarditis occurring as a complication of a vascular access infection in a patient on chronic hemodialysis that was successfully treated with a combination of vancomycin and gentamicin is reported.
