Impact of public smoking bans on children's exposure to tobacco smoke at home: a systematic review and meta-analysis.

BACKROUND: Meta-analysis of the impact of public smoking bans on children's exposure to secondhand smoke (SHS) exposure at home. METHODS: The electronic databases of PubMed, Web of Science, PsycINFo, ASSIA, CINAHL were searched. German public health journals not captured by these databases and grey literature were considered in addition. Studies were included when they reported children's SHS exposure at home in relation to smoke-free legislation by measuring exposure before and after the introduction of a public smoking ban. Studies had to provide results on exposure prevalences of children

The Risk of Hospital Admission Due to Traumatic Brain Injury Is Increased in Older Persons With Severe Functional Limitations.

INTRODUCTION: Hospital admissions due to traumatic brain injuries (TBIs) in older persons are increasing. Falls are the leading mechanism of injury in this age group. TBIs are associated with unfavorable outcomes such as mortality and institutionalization. OBJECTIVES: To estimate rates of TBIs in older persons with severe functional limitations, expressed as "care need," living in the community, and in older persons with care need living in nursing homes compared with older persons without care need. PARTICIPANTS: More than 1.2 million persons aged 65 years and older living in Bavaria, Germa

Pediatric allergy and immunology at Siriraj Hospital.

Like other parts of the world, prevalence of childhood allergic diseases in Thailand, particularly of asthma and allergic rhinitis, has risen sharply over the past decade. Epidemiologic studies in this country indicated that allergic sensitization (mainly to house dust mites, cockroaches and cat dander) is the major important risk factor for the development of asthma. House dust mites are the most important source of allergens causing sensitization among allergic Thai children. A nationwide survey indicated that house dust mites are ubiquitous in Thai homes. Despite the authors' earlier findin

Cockroach allergy and exposure to cockroach allergen in Polish children with asthma.

BACKGROUND: Asthma morbidity increases every year, especially among children, and exposure to high levels of indoor allergens is a very important factor. We evaluated the prevalence and exposure to cockroach (CR) allergen in asthmatic children in Poland, and also tested the hypothesis that asthma with allergy to CR is more severe than with allergy to other antigens. METHODS: One hundred and sixty children with asthma were examined, had skin prick tests (SPT) with common and CR allergens, underwent spirometry, and provocation tests to histamine. Children with positive SPT to CR had measured sp

Home compared with clinic-based screening for sexually transmitted infections: a randomized controlled trial.

OBJECTIVE: To determine whether home-based screening for sexually transmitted infections results in a higher sexually transmitted infection screening rate compared with clinic-based screening in participants using long-acting reversible contraception. METHODS: We performed a randomized clinical trial of women using long-acting reversible contraception methods in the Contraceptive CHOICE Project (n=558). Participants were randomly assigned to home-based testing (swabs mailed to the participant's home) or clinic-based testing. Self-collected vaginal swabs were tested for Chlamydia trachomatis a

Home versus hospital phototherapy for term infants with hyperbilirubinemia: a comparative study.

Twenty-five term infants with hyperbilirubinemia were selected for phototherapy administered at home if they fulfilled clinical and laboratory criteria designed to exclude any pathologic cause of hyperbilirubinemia. Ongoing nursing surveillance of home phototherapy was provided to assure adequate nursing care. No phototherapy-related complications were noted in the study infants. Effectiveness of phototherapy at home was compared with results in a similar group of 33 infants who underwent standard hospital phototherapy. The average decrease in serum bilirubin concentration was similar for the

The effectiveness of home-based phototherapy for vitiligo: A systematic review of randomised controlled trials.

BACKGROUND: Vitiligo may be treated with hospital-based phototherapy, but this requires long-term frequent appointments. Self-treatment using home-based phototherapy is a convenient alternative, which may improve adherence and results, but evidence is limited, and so it is not routinely recommended. This systematic review aims to assess the effectiveness and safety of home-based phototherapy for vitiligo. METHODOLOGY: Searches were conducted on Medline, Scopus and The Cochrane Library for randomised controlled trials comparing home-based phototherapy with institution-based phototherapy or pla

Using visual inquiry to reveal differences in nursing students' perception of case management.

An exploratory, qualitative study of 24 senior generic nursing students (GNSs) and 23 practicing registered nurse baccalaureate students (PRNSs) was conducted to evaluate students' perceptions about case management for community-based children with disabilities, and to examine the differences in the two student groups' perceptions utilizing the visual inquiry method. For a class project, the students used words and photographs to show how they solved one problem for a family caring for a child with a disability in the community. All students agreed on the overall theme, home environment; howev

Ultrasound assisted homing of human umbilical cord mesenchymal stem cells promotes recovery from acute respiratory distress syndrome.

BACKGROUND: Human umbilical cord mesenchymal stem cells (hUC-MSCs) show potential for treating acute respiratory distress syndrome (ARDS), however, their homing to the lungs and survival time are insufficient. In this study, we evaluated whether pulsed focus ultrasound (pFUS) could promote the homing and prolonged retention of hUC-MSCs in the lungs of ARDS mice and explored the mechanisms involved. METHODS: Mice were divided into four groups: the NC group, the LPS group, the MSCs group, and the pFUS + MSCs group. Except for the NC group, the other three groups were constructed as ARDS models

Effectiveness of aquatic therapy on balance and functionality in children with Duchenne and Becker muscular dystrophy : a prospective controlled pilot study.

OBJECTIVE: To investigate the effects of combining aquatic therapy with a home exercise program on balance, functionality, and quality of life in children with DMD/BMD, compared with a home exercise program alone. METHODS: Sixteen patients aged 5-18 years with DMD/BMD were included. Those at the top of the waiting list were assigned to the intervention group (n = 8), and those further down to the control group (n = 8). The intervention group received 30-minute Halliwick-based aquatic therapy three times per week for five weeks in a 30°C pool, in addition to a home exercise program. The contro

Effectiveness and safety of a home-based cardiac rehabilitation programme of mixed surveillance in patients with ischemic heart disease at moderate cardiovascular risk: A randomised, controlled clinical trial.

BACKGROUND: Previous studies have documented the feasibility of home-based cardiac rehabilitation programmes in low-risk patients with ischemic heart disease, but a similar solution needs to be found for patients at moderate cardiovascular risk. The objective of this study was to analyse the effectiveness and safety of a home-based cardiac rehabilitation programme of mixed surveillance in patients with ischemic cardiopathology at moderate cardiovascular risk. METHODS: A randomised, controlled clinical trial was designed wherein 28 patients with stable coronary artery disease at moderate cardi

Home management of malaria with artemether-lumefantrine compared with standard care in urban Ugandan children: a randomised controlled trial.

BACKGROUND: Home management of malaria-the presumptive treatment of febrile children with antimalarial drugs-is advocated to ensure prompt effective treatment of the disease. We assessed the effect of home delivery of artemether-lumefantrine on the incidence of antimalarial treatment and on clinical outcomes in children from an urban setting with fairly low malaria transmission. METHODS: In Kampala, Uganda, 437 children aged between 1 and 6 years from 325 households were randomly assigned by a computer-generated sequence to receive home delivery of prepackaged artemether-lumefantrine for pres

Achieved home systolic blood pressure and cardiovascular risk in older hypertensive patients.

Intensive blood pressure treatment reduces cardiovascular events, but the optimal achieved home systolic blood pressure (SBP) level for older patients with hypertension remains uncertain. This secondary analysis was based on data from the Strategy of Blood Pressure Intervention in Older Hypertensive Patients trial. Morning and evening home blood pressure measurements obtained between 3 and 12 months after enrollment were used to calculate the achieved home SBP. Participants were then categorized into four achieved home SBP groups: <125 mmHg, 125-130 mmHg, 130-135 mmHg, and ≥135 mmHg. The prima

The impact of different patterns of home- and community-based services on nursing home admission: National data from Taiwan.

BACKGROUND: This study had two aims: (a) to identify the different patterns of use of home- and community-based services (HCBS) among older adults in Taiwan, and (b) to examine the effects of the different use patterns on HCBS recipients' use of institutional long-term care services. METHODS: The study analyzed cohort data from Taiwan's first National 10-Year Long-Term Care Plan database and from National Health Insurance Claim Data. We extracted baseline information on older adults who were first evaluated for and prescribed HCBS from 2010 through 2013 (N = 71,260). We used latent class anal

Safety profile of idursulfase administered at home in patients with mucopolysaccharidosis II enrolled in the Hunter Outcome Survey.

The Hunter Outcome Survey (HOS) collected global, real-world data on the natural history of mucopolysaccharidosis II and its treatment with intravenous idursulfase. For eligible patients, home therapy offers a convenient alternative to in-clinic therapy. Using data in HOS as of January 2023, we provide an updated assessment of the safety/tolerability profile of home therapy with idursulfase. The analysis population comprised 333 patients who had received at least one home infusion and 708 patients who had never received home therapy. Median (10th percentile [P10], 90th percentile [P90]) age at

A prospective study of incident comprehensive medical home care use among the elderly.

This prospective study directly examines, in a defined community population, the extent to which a wide array of characteristics predict utilization of an important long-term care (LTC) service--medical home care--over a two-year interval among the cohort of 3,706 people aged 65 or older. The overall age-sex adjusted rate of two-year incident home care use was 3.2 per cent. For both men and women, the rates among the aged 85 or older group were approximately 12 times the rates of those aged 65 to 74. The multivariate predictors of incident home care, adjusted for age and sex, were five: receiv

Telephone-based joint protection education in lateral epicondylitis: A randomized controlled trial.

BACKGROUND: Lateral epicondylitis (LE) can occur for many different reasons such as compelling repetitive movements in daily readings, incorrect posture use and work-related factors. Although several treatments are available for LE, the optimal evidence-based treatment remains uncertain. Joint protection techniques have been developed as a self-management intervention to reduce pain and disability and improve functionality by applying ergonomic approaches. OBJECTIVES: This study aimed to investigate the effects of telephone-based follow up on top of a home-based joint protection education pro
