[Application of low temperature thermoplastic plate combined with special abdominal band in fixing abdominal pedicled flap for repairing 17 patients with deep electric burn wounds in hands].

If the abdominal pedicled flaps are not well fixed after repair of deep electric burn wounds in hands, many problems such as poor blood supply may occur. In order to solve the above problems, we designed and manufactured the individualized low temperature thermoplastic plate combined with special abdominal band to fix abdominal pedicled flaps for repairing of 17 patients (12 males and 5 females, aged 2-35 years) with deep electric burn wounds in hands from February 2016 to August 2018, and achieved the desired results. The shoulder joint, elbow joint, and wrist joint were fixed by low temperat

Does donor-recipient body mass index ratio influence heart transplantation outcomes??

BACKGROUND: Obesity is a common risk factor for heart failure, and heart transplantation (HTx) is the treatment of choice for end-stage heart failure. Due to the limited availability of the donor's heart, efforts are made to increase the donor pool on one hand and to find predictors that can impact HTx outcomes on the other hand. These predictors can help improve donor organ allocation and HTx outcomes. This study aims to investigate the impact of the donor-recipient BMI ratio on follow-up mortality and other outcomes after HTx. METHODS: From 2012 to 2021, 821 patients underwent HTx in our ce

Real-Time Remotely Supervised Home-Based Rehabilitation Exercise Program for Kidney Transplant Recipients: A Pilot Study to Explore the Optimal Approach.

BACKGROUND AND PURPOSE: Patients undergoing Kidney Transplant (KT) have a reduced mobility postoperative period and infection hazard because of immunosuppression. Remotely supervised exercise programs reduce muscle impairment, unnecessary patient displacement, and carbon footprint. Herein, we evaluate the outcomes of a remotely supervised program in KT recipients. METHODS: Twenty-six recent KT recipients (12 in intervention group [IG] and 14 in control group [CG]) were recruited. IG conducted remotely supervised home-based rehabilitation exercise training for 6 months. Participants performed

Proximal ulna corticocancellous autograft for the repair of metacarpal defect following ballistic hand injury.

Metacarpal defects following ballistic hand injuries are a devastating complication that can compromise hand function. Autologous bone grafting is the gold standard treatment of these osseous defects, most commonly utilizing the anterior iliac crest as the donor site. Alternatively, use of corticocancellous bone graft from the proximal ulna has been described, although with mixed clinical outcomes. Additionally, the upper limit of defect that can be repaired with this technique remains unknown. Herein we described the case a patient who sustained a partial extensor tendon injury and significan

Surgical correction of the hand in epidermolysis bullosa dystrophica.

Epidermolysis bullosa dystrophica (polydysplastic type) is a rare congenital skin anomaly which, in the hands, because they are exposed to repeated trauma, results in a severe "mitten"-like deformity. Functional benefit was obtained in three patients by separation of the digits and application of split-thickness grafts. Wolfe grafts or "split-off" (epidermis) grafts. Arthrodesis of the interphalangeal joints and filleting of the little finger to provide a flap which could be turned in to the palm, resulted in an improvement in hand function.

Neurovascular free foot flaps in reconstruction of the mutilated hand.

The neurovascular free flap from the first web region of the foot affords unrivalled potential for refurbishing sensibility to the denervated hand. Its greatest application is when no conventional island flap tissue remains for transfer. Its two-point discrimination is only moderate, but when used for thumb reconstruction, its sensibility is adequate because no functional substitute exists. Functionally and cosmetically it is superior to other methods of thumb reconstruction because of its pulp, size, and nail availability. There are no finger identification problems and secondary donor defect

Common injuries of the athlete's hand.

Hand injuries are among the most frequent accidents seen in sports medicine. All too commonly they are considered trivial since the athlete may continue to participate actively and neglect his injury. The consequent delay in diagnosis and proper treatment may result in long-standing or even permanent disability. This paper describes the more commonly encountered hand injuries, their diagnosis and their optimal treatment. Included are soft-tissue injuries, ligamentous injuries, fractures and tendon avulsions. The basic principles applicable to skeletal and soft-tissue trauma of the hand, which

Brachial artery aneurysm as a late complication of arteriovenous fistula.

INTRODUCTION: Brachial artery aneurysm (BAA) is a rare late complication of arteriovenous fistula (AVF). It brings the risk of peripheral embolism and hand ischemia and is defined by brachial artery diameter above 10 mm or by regional dilatation by >50%. BAA is described in the literature in closed radiocephalic arteriovenous fistulas after kidney transplantation. The aim of the study was to analyze the prevalence of BAA and of their more dangerous forms. METHOD: A observational one center study performed on patients after kidney transplantation with AVF or arteriovenous graft (AVG). We invit

Tubular dysfunction following kidney transplantation.

After transplantation the kidney is subjected to rejection and other deleterious factors including ischemic damage, acute tubular necrosis, rejection and the use of cyclosporine A (CsA) or FK506. As a result, kidney damage may be generalized with azotemia as its hallmark. These tubular syndromes may cause profound changes in the acid base balance and in the level of certain blood electrolytes and minerals. As a general rule, the renal tubular acidosis (RTA) that appears early following transplantation disappears spontaneously and is predominantly a sequela to acute renal failure. On the other

[Inhibition of acute rejection of heart graft in rats without immunosuppressants after intrathymic myocardial cell inoculation in the neonatal period].

There have been no successful cases reported of organ transplantation in application of neonatal immunological tolerance. Isolated myocardial cells from Lewis (LEW/Crj) rat hearts were inoculated into the thymus of incompatible King (WKAH/Hkm) rats. Ten weeks later the heart from LEW donor was transplanted heterotopically to the WKAH rat that had inoculated intrathymic myocardial cells without immunosuppression (n = 4, Group 1). Group 2 was made up of 4 isografts of heart transplantation (nontreated isograft). Group 3 was made up 4 allografts of heart transplantation (nontreated allograft). Th

[Reconstruction of the I carpo-metacarpal joint by transplantation of the II metatarsophalangeal joint on the neuro-vascular pedicle].

In the article are presented the results of treatment of a patient with the consequences of severe damage of hand. As the basic method of treatment there was used free microneurovascular autotransplantation of tissue complexes. The main attention was paid to the problem of reconstruction of the I carpometacarpal joint by transplantation of the II metatarsophalangeal articulation on the neurovascular pedicle. There was achieved a favourable anatomic and functional result. There are indicated the advantages of the given method in case of irreversible destruction of the I carpometacarpal joint.

[Reconstruction of gripping function (author's transl)].

Description of two cases of badly injuried hands, where the grasp-function has been restored. In one case, the traumatically amputated thumb was reconstructed in the conventional way, transposing the index finger on its neurovascular bundle. The other case demonstrates the use of modern microsurgical methods in substituting a freely transplanted toe with microvascular anastomoses for four lost fingers. Resulting function was satisfactory in both cases.

Current status of vascularized composite tissue allotransplantation.

Vascularized composite tissue allotransplantation (VCA) offers treatment options of complex functional deficiencies that cannot be repaired with conventional reconstructive methods. VCAs consist of blocks of functional units comprising different tissue types such as skin, bone, muscle, nerves, blood vessels, tendons, ligaments and others, and are thus substantially different from the composition of organ transplants. The field of VCA has made fascinating progresses in the recent past. Among other VCAs, numerous successful hand, face and limb transplants have been performed in the world. At the

Immunosuppression in Liver Transplant Recipients in the Setting of Sepsis.

Management of immunosuppression (IS) in liver transplant recipients in the setting of sepsis is an open stage for debate. The age-long practice of reduction or complete cessation of IS during sepsis has been followed by most centres across the world, although, their exact strategies are highly heterogeneous. On the other hand, the emergence of striking new evidence suggesting that there is, in fact, decreased mortality with the continuation of IS in sepsis, has raised doubts about our previously conceived intuitive notion that IS portends increased risk in sepsis. The theory postulated is that

[Keratopathy as a sign of multifocal congenital sensory polyneuropathy. A case report].

PATIENT: A 63-year-old female with bilateral recurrent corneal ulcerations for 10 years, suffered from vascularisation of the cornea and absence of corneal sensitivity. Other symptoms were multifocal hypoaesthesia with hypalgesia of hands and legs, generalised lack of deep tendon reflexes, absence of somato-sensory evoked potentials (SSEP) and of sensory nerve action potentials (SNAP) in these regions. A sural biopsy demonstrated extreme lack of myelinated fibres. Acquired causes for polyneuropathy were excluded. THERAPY: Subsequent to local ocular treatment we carried out a perforating corne

[Transplantation for primary hyperoxaluria. Role of oxalate crystal deposits in the occurrence of kidney failure].

We analyzed the records of 3 patients transplanted for end-stage renal failure due to primary hyperoxaluria and evaluated on repeat biopsies the role played by oxalate deposits in the constitution of renal failure after isolated kidney graft, or combined liver and kidney transplantation. Early failure of the renal graft is frequent and often interpreted as the consequence of recurrence because of the presence of oxalate deposits on the graft biopsy. In fact, the decrease in oxalate deposits observed in our 2 cases of combined liver and kidney transplantation despite the progressive renal failu

Laparoscopic live donor nephrectomy in a patient with duplex inferior vena cava.

BACKGROUND: Modern imaging, such as CT and MRI, improves the preoperative assessment for variants of renal vasculature. We present a kidney donor with a duplex inferior vena cava. In conjunction with CT and hand-assisted laparoscopic surgery, live donor nephrectomy was performed successfully. METHODS: A 35-year-old woman wished to donate a kidney to her son. Preoperative CT showed normal functional kidneys without uretal duplication. A duplex inferior vena cava was noted below the level of the left renal vein. A hand-assisted transperitoneal laparoscopic left nephrectomy was performed. Blood
