Biomechanical Comparison of a Novel Tibial Fixation Technique Versus Interference Screw Fixation for ACL Reconstruction Using Soft Tissue Grafts.

BACKGROUND: Controversy remains regarding the optimal technique for tibial fixation of soft tissue grafts in anterior cruciate ligament (ACL) reconstruction. PURPOSE/HYPOTHESIS: To compare the biomechanical outcomes of a novel transtibial tubercle fixation technique with those of a commonly utilized interference screw fixation at the tibial site. Our hypothesis was that transtibial tubercle fixation achieves higher ultimate failure loads than interference screw fixation. STUDY DESIGN: Controlled laboratory study. METHODS: We used 24 matched porcine tibias and digital extensor tendons, from

Functional outcomes, union rate, and complications of the Anser Clavicle Pin at 1 year: a novel intramedullary device in managing midshaft clavicle fractures.

BACKGROUND: Surgical management of displaced midshaft clavicle fractures in adults leads to better union rates, improved early functional outcomes, and increased patient satisfaction compared with nonoperative treatment. However, both intramedullary fixation and plate osteosynthesis are subject to a specific array of disadvantages and complications. The Anser Clavicle Pin is a novel intramedullary device designed to address these disadvantages and complications. The aim of this study was to evaluate the union rate, functional outcomes, and complications of the Anser Clavicle Pin at 1-year foll

Surgical treatment of clavicle fractures in the adolescent athlete.

CONTEXT: Current literature has clearly shown that the indications for surgical treatment of clavicle fractures in adults are expanding. Although clavicle fractures in children and adolescents have traditionally been treated nonoperatively, surgical treatment of displaced clavicle fractures may be indicated for adolescent athletes. EVIDENCE ACQUISITION: A review of relevant articles published between 1970 and 2013 was completed using MEDLINE and the terms clavicle fracture and adolescent athlete. STUDY DESIGN: Clinical review. LEVEL OF EVIDENCE: Level 3. RESULTS: Excellent outcomes and rap

Massive osteolysis due to galvanic corrosion after total knee arthroplasty: a rare cause for early revision?

A 66-year-old male underwent a total knee arthroplasty for osteoarthritis after previous anterior cruciate ligament (ACL) reconstruction. Seven years postoperatively, a symptomatic large lytic lesion was present surrounding the tibial stem. A titanium interference screw, which was used prior to fixate the Anterior Cruciate Ligament (ACL) graft, was in direct contact with the tibial component. Galvanic corrosion may have attributed to the development of the lytic lesion. It is advised to remove any metal hardware in the vicinity of joint prosthesis in order to prevent a possible galvanic corros

Complications of open anterior stabilization of the shoulder.

Complications of surgery for glenohumeral instability are relatively uncommon. When they occur, salvaging failures and obtaining a stable joint can be awesome challenges. Accurate recognition of the cause of the instability and application of the appropriate surgical technique are critical. Deficiencies of the glenoid concavity, the anterior capsule, or the subscapularis may be present and require correction. Overtightening a shoulder and eliminating its normal laxity should be avoided. Loose or malpositioned hardware about the glenohumeral joint must be recognized as soon as possible and remo

Outcomes of suture tension band technique for fixation of olecranon fractures: a retrospective case series.

BACKGROUND: Tension band wiring is one of the earliest and most common techniques for the fixation of olecranon fractures with good functional outcomes. However, it is fraught with hardware-related complications, with reported reoperation rates of 33.3%-79.2%. Despite the aim of reducing hardware-related complications, low-profile precontoured plates continue to have hardware removal rates of 15%-56%. Suture tension band fixation is a novel technique for the management of select olecranon fractures with equivalent or superior biomechanical performance to tension band wiring and avoids hardware…

Elastic Locking Intramedullary Nails Fixation in Trimalleolar Fractures.

OBJECTIVE: Since the 1960s, although open reduction and internal fixation for ankle fractures has been widely used, it is associated with complications such as wound dehiscence, infection, prominent hardware, and failure. Closed reduction and internal fixation, on the other hand, offers greater biomechanical strength, requires minimal incisions, and features low-profile hardware. Our study compares the efficacy of elastic locking intramedullary nails (ELIN) fixation featuring minimally invasive microenvironmental protection microstress shielding versus rigid internal fixation (RIF) for trimall

Does Pin Removal at Three Versus Five Weeks Influence Clinical Outcomes in Gartland Type III Supracondylar Humerus Fractures? A Comparative Study.

OBJECTIVE: The optimal timing of pin removal following closed reduction and percutaneous pinning (CRPP) for pediatric Gartland type 3 supracondylar humerus fractures remains controversial. Although pins are commonly removed between the third and fifth postoperative weeks, high-level evidence comparing clinical and radiological outcomes between these time points is limited. This study aimed to evaluate whether pin removal at the third vs. the fifth postoperative week influences radiographic healing or functional outcomes. METHODS: This retrospective observational study included 95 pediatric pa

Successful minimal-invasive management of misplaced sacroiliac screw perforating the left external iliac vein: an interdisciplinary intervention by angiology and traumatology-a case report.

BACKGROUND: Percutaneous sacroiliac screw fixation is an effective option for unstable pelvic ring fractures. It is minimally invasive, causes little soft tissue damage, and reduces complication risk. Nevertheless, complications such as screw loosening or nerve root damage are reported. Although vascular injuries are also described, detailed management strategies are rarely found in the literature. CASE PRESENTATION: A 58-year-old white German female suffered polytrauma, including a type c posterior pelvic ring fracture, after a fall from great height. The fracture was treated with screw oste
