Association between harm reduction strategies and healthcare utilization in patients on long-term prescribed opioid therapy presenting to acute healthcare settings: a protocol for a systematic review and meta-analysis.

INTRODUCTION: Opioids are routinely used to treat a variety of chronic conditions associated with pain. However, they are a class of medications with a significant potential for adverse health effects, with and without misuse. Opioid misuse, as defined as inappropriate use of appropriately prescribed opioids, is becoming more well-recognized publicly but does not have clear treatment options. Opioid misuse has been linked to variety of poor outcomes and its consequences have a significant impact on healthcare resource utilization. The evidence on harm reduction strategies to mitigate adverse e…

Population size estimation of people who inject drugs using capture-recapture analysis of multiple respondent-driven sampling rounds: Implications for HIV/HCV burden and harm reduction service planning in Thessaloniki, Greece.

BACKGROUND: Accurate population size estimation of people who inject drugs (PWID) is essential for evidence-based drug policy and service pl … nds through interaction terms. We then estimated HIV/HCV disease burden and assessed prevention and harm reduction service coverage against international standards (HIV testing, OAT, NSP). RESULTS: Based on data from 1093 unique participants across five rounds (53.9% currently injecting, 20.3% currently in OAT), capture-recapture analysis estimated 1512 PWID (9 …

"Now that I'm here, I actually have the stability and the time to really think about stuff like that": Perspectives on medications for opioid use disorder in low-threshold harm reduction housing.

Sweeping of encampments is one policy approach to the growing visibility of homelessness and substance use in U.S. cities but is associated with increased overdose deaths. In 2022, to mitigate the impacts of a sweep, the City of Boston created seven harm reduction housing (HRH) sites to accommodate displaced individuals. HRH sites offered on-site or off-site medications for opioid use disorder (MOUD). As part of a broader parent study, we recruited 28 residents from HRH sites previously enrolled in a survey for semi-structured interviews, exploring their experiences with housing, the current H…

People Control Their Addictions: No matter how much the "chronic" brain disease model of addiction indicates otherwise, we know that people can quit addictions - with special reference to harm reduction and mindfulness.

The world, led by the United States, is hell bent on establishing the absence of choice in addiction, as expressed by the defining statement that addiction is a "chronic relapsing brain disease" (my emphasis). The figure most associated with this model, the director of the American National Institute on Drug Abuse, Nora Volkow, claims that addiction vitiates free will through its effects on the brain. In reality, while by no means a simple task, people regularly quit their substance addictions, often by moderating their consumption, usually through mindfulness-mediated processes (Peele, 2007).

Harm reduction in the fourth wave of the opioid epidemic: A case series investigation of the effectiveness of definitive vs presumptive urine drug testing for opioid use disorder with co-occurring polysubstance use.

BACKGROUND: We report investigating effectiveness and medical necessity of urine drug testing in a real-world setting comparing near-real-time definitive chromatography/mass spectrometry to real-time presumptive immunoassay for guiding evidence-based harm reduction in a buprenorphine medication for opioid use disorder clinic during the initial swell of the opioid epidemic's fourth wave. METHODS: Our exploratory case series included all opioid use disorder patients, most of whom exhibited co-occurring polysubstance use, treated with buprenorphine for one year in a single outpatient clinic. "COVID just kind of opened a can of whoop-ass": The rapid growth of safer supply prescribing during the pandemic documented through an environmental scan of addiction and harm reduction services in Canada.

OBJECTIVES: In the context of the ongoing overdose crisis, a stark increase in toxic drug deaths from the unregulated street supply accompanied the onset of the COVID-19 pandemic. Injectable opioid agonist treatment (iOAT - hydromorphone or medical-grade heroin), tablet-based iOAT (TiOAT), and safer supply prescribing are emerging interventions used to address this crisis in Canada. Given rapid clinical guidance and policy change to enable their local adoption, our objectives were to describe the state of these interventions before the pandemic, and to document and explain changes in implement

How do restrictions on opioid prescribing, harm reduction, and treatment coverage policies relate to opioid overdose deaths in the United States in 2013-2020? An application of a new state opioid policy scale.

BACKGROUND: Identifying the most effective state laws and provisions to reduce opioid overdose deaths remains critical. METHODS: Using expert ratings of opioid laws, we developed annual state scores for three domains: opioid prescribing restrictions, harm reduction, and Medicaid treatment coverage. We modeled associations of state opioid policy domain scores with opioid-involved overdose death counts in 3133 counties, and among racial/ethnic subgroups in 1485 counties (2013-2020). We modeled a second set of domain scores based solely on experts' highest 20 ranked provisions to compare with th…

Problemas ligados ao álcool em centros de emergência (PLACE)-People experiencing homelessness with alcohol-related problems in Lisbon's emergency shelters during the COVID-19 pandemic: a description and analysis of a harm reduction intervention.

INTRODUCTION: Alcohol-related problems disproportionally affect people experiencing homelessness. As the first wave of the COVID-2019 pandemic spread in 2020, a number of emergency shelters were opened in Lisbon. Increased difficulties in obtaining alcohol could have led to an increased incidence of alcohol withdrawal. Therefore, a low-threshold harm reduction intervention was introduced to these emergency shelters. This consisted of a fixed medication treatment, made available immediately for those with specific conditions, without the need for a medical evaluation or abstinence from alcohol,…

Differences between men who have sex with men that use drugs and engage in chemsex and those who do not engage in chemsex in the French ANRS-PREVENIR cohort: the need to rethink harm reduction services.

BACKGROUND: An increasing number of studies are exploring the profiles of men who have sex with men (MSM) in the context of the sexualized use of drugs (chemsex). However, less attention has been paid to MSM who use drugs but do not engage in chemsex. We do not know to what extent the latter are different from the former, or whether they require similar harm reduction services. This study aimed to compare both populations in terms of their sexual risk practices, health, and pre-exposure prophylaxis (PrEP) use. METHODS: We used longitudinal data from the ongoing French cohort study ANRS-PREVEN…

"Every Time I Go in There, It Gives Me Time to Reflect": A Qualitative Study of Patient Perspectives on Substance Use, Medications for Opioid Use Disorder, and Harm Reduction Following Hospitalization for Serious Injection-Related Infection.

BACKGROUND: Serious injection-related infections (SIRIs) have high morbidity and mortality, in part from incomplete antibiotic treatment, ongoing substance use and reinfection. Understanding how hospitalizations for SIRIs affect patient perspectives on substance use, harm reduction, and medications for opioid use disorder (MOUD) in the era of hospital-based addiction services will inform efforts to improve care. METHODS: We conducted qualitative interviews at Boston Medical Center with individuals hospitalized with SIRIs between 2020 and 2024. To ensure diverse experiences, we recruited quali…

Effectiveness and cost-effectiveness of a peer-delivered, relational, harm reduction intervention to improve mental health, quality of life, and related outcomes, for people experiencing homelessness and substance use problems: protocol for the 'SHARPS' cluster randomised controlled trial.

BACKGROUND: Those experiencing homelessness and problem substance use find it challenging to access the healthcare and treatment they need. The Supporting Harm Reduction through Peer Support (SHARPS) feasibility study demonstrated that Peer Navigators can help these individuals to improve their service engagement, increase access to opioid substitution therapy, and lead to reductions in drug use and risky injection practices. Specifically, participants indicated that the lived experience of Peer Navigators was particularly helpful by enabling the development of trusting relationships. A cluste…

The Effect of a Mandatory Play Break on Subsequent Gambling Behavior among British Online Casino Players: A Large-Scale Real-World Study.

In recent years, various novel responsible gambling (RG) tools have been implemented to aid harm-minimization. One such RG tool has been the implementation of enforced mandatory play breaks. Despite many responsible gambling operators using mandatory play breaks, only three previous studies have examined their efficacy and the findings were mixed. Therefore, the present investigation was a large-scale real-world study which was designed to see whether a 60-minute mandatory play break influenced subsequent depositing and wagering. The authors were given access to 27 days of player data prior to

Moving on from responsible gambling: a new discourse is needed to prevent and minimise harm from gambling.

OBJECTIVES: To address the current status of responsible gambling (RG) as the dominant discourse for reducing gambling harm. STUDY DESIGN: The article is a narrative review of relevant literature analysed discursively. METHODS: We identified significant texts describing the discourse of RG and analysed these to extract major characteristics and themes of the discourse. These were then subjected to a critique, using the public health discourses as an alternative system for addressing gambling harm. RESULTS: The discourse of RG is inadequate for preventing or minimising gambling harm. A publi

Beyond 'single customer view': Player tracking's potential role in understanding and reducing gambling-related harm.

BACKGROUND: Usage of electronic gaming machines (EGMs) and on-line gambling is strongly associated with gambling-related harm. Player-tracking systems can monitor a gambler's activity across multiple sessions and/or operators, providing a clearer picture of the person's risk of harm with respect to these gambling formats and enabling harm reduction efforts. The Finnish and Norwegian state monopolies have player-tracking systems in place, while the United Kingdom is implementing an operator-led system called 'single customer view' for on-line gambling, and Australian states are proposing simila…
