Mechanical circulatory support devices in heart failure: a narrative review of current evidence, clinical applications, complications, and future directions.

Mechanical circulatory support devices (MCSDs) have evolved from rescue therapy for refractory cardiogenic shock (CS) to an essential component of advanced heart failure management, spanning temporary support, bridge strategies, durable left ventricular assist device (LVAD), and total artificial heart (TAH). This structured narrative review summarizes the historical evolution, current indications, device classification, major short-term and long-term platforms, adverse events, and future directions of MCSDs. Literature was selected in a structured narrative manner with emphasis on contemporary

Stroke in Patients with Left Ventricular Assist Devices.

BACKGROUND: Left ventricular assist devices (LVADs) are artificial pumps used in end-stage heart failure to support the circulatory system. These cardiac assist devices work in parallel to the heart, diverting blood from the left ventricle through an outflow graft and into the ascending aorta. LVADs have allowed patients with end-stage heart failure to live longer and with improved quality of life compared to best medical therapy alone. However, they are associated with significant risks related to both thrombosis and bleeding in this medically complex patient population. As LVADs continue to

The total artificial heart: indications and preliminary results.

The development of the total artificial heart (TAH) has reached a level where it is now available for clinical applications. The TAH has demonstrated distinct advantages over other forms of mechanical circulatory assistance. As of December 1, 50 TAHs have been implanted: 5 as permanent devices, and 45 as a temporary mechanical bridge to cardiac transplantation. The use of the TAH has increased in the last several months, leading to a growing interest in defining the indications and contraindications to its use. End-stage cardiomyopathy (either idiopathic, ischemic, viral, or postpartum) has be

[Postoperative management of patients undergoing left ventricular assist device implantation].

Left ventricular assist devices (LVAD) have become an important option for the treatment of patients with advanced heart failure. Unfortunately, patients undergoing LVAD-implantation still experience considerable perioperative morbidity and mortality. Right ventricular failure, bleeding, infections and device-associated problems represent the major postoperative complications. The present overview summarizes current strategies for preoperative risk estimation and for the prevention and treatment of frequently occurring perioperative complications.

Impact of Advanced Therapies for Improving Survival to Heart Transplant in Patients with Hypertrophic Cardiomyopathy.

Heart transplant has become an increasingly important option for patients with end-stage nonobstructive hypertrophic cardiomyopathy (HC). However, clinical details related specifically to the overall HC transplant experience remain sparse. We assessed outcomes of HC heart transplants, from 2002 to 2016, at Tufts Medical Center. Fifty-two nonobstructive severely symptomatic patients underwent evaluation at 47 ± 13 years; 11 (21%) declined or failed to qualify, most commonly because of co-morbidities (n = 7). Of the remaining 41 patients ultimately listed, 6 (15%) died of heart failure awaiting

A Comprehensive Review of Mechanical Circulatory Support Devices.

Treatment strategies to combat cardiogenic shock (CS) have remained stagnant over the past decade. Mortality rates among patients who suffer CS after acute myocardial infarction (AMI) remain high at 50%. Mechanical circulatory support (MCS) devices have evolved as novel treatment strategies to restore systemic perfusion to allow cardiac recovery in the short term, or as durable support devices in refractory heart failure in the long term. Haemodynamic parameters derived from right heart catheterization assist in the selection of an appropriate MCS device and escalation of mechanical support wh

A Multi-objective Physiological Control for Continuous Flow Left Ventricular Assist Devices: Comparison of Estimator versus Sensor-based Feedback.

Left Ventricular Assist Devices have been successfully used for the treatment of Congestive Heart failure in patients who are not eligible for heart transplantation. This paper describes the implementation and comparison of the performance of a pressure sensor-based feedback controller. The strategies were tested on a mock loop of the systemic circulation. The results show that the use of pressure sensors generated a more accurate response of the controller compared to the use of estimators.Clinical Relevance- The study describes the integration of a LVAD physiological controller to a dynamic

Roles of cyclooxygenase-2 and phosphorylated Akt (Thr308) in cardiac hypertrophy regression mediated by left-ventricular unloading.

OBJECTIVES: Cyclooxygenase-2 is associated with cardiac hypertrophy during chronic heart failure and is regulated through the PI3K/Akt pathway. Cyclooxygenase-2-induced cell growth through Akt phosphorylation was demonstrated in vitro. In chronic heart failure, left ventricular assist devices lead to hypertrophy regression and molecular changes. Therefore, the expression of cyclooxygenase-2, phosphorylated Akt (p-Akt), and p-Erk 1/2, as well as cardiac hypertrophy before and after left ventricular assist device insertion, was investigated. METHODS: In myocardial tissue before and after left v

Contemporary Phase I Cardiac Rehabilitation: A Case Report of Pre- and Post-Heart Transplant.

Patients in cardiac intensive care units (ICUs) often require mechanical circulatory support (MCS) devices. Phase I cardiac rehabilitation for patients requiring a single MCS device appears safe and feasible. However, the simultaneous use of multiple MCS devices is relatively new and rehabilitation practices have had less evaluation in these circumstances. This case report describes phase I cardiac rehabilitation delivery in a 73-year-old man with cardiogenic shock supported with simultaneous femoral-femoral, veno-arterial extracorporeal membrane oxygenation and percutaneous ventricular assist

Durable circulatory support with a paracorporeal device as an option for pediatric and adult heart failure patients.

OBJECTIVES: Not all patients in need of durable mechanical circulatory support are suitable for a continuous-flow left ventricular assist device. We describe patient populations who were treated with the paracorporeal EXCOR, including children with small body sizes, adolescents with complex congenital heart diseases, and adults with biventricular failure. METHODS: Information on clinical data, echocardiography, invasive hemodynamic measurements, and surgical procedures were collected retrospectively. Differences between various groups were compared. RESULTS: Between 2008 and 2018, a total of

Support with intra-aortic balloon pump vs. Impella2.5® and blood flow to the heart, brain and kidneys - an experimental porcine model of ischaemic heart failure.

BACKGROUND: Cardiogenic shock as a complication to an acute myocardial infarction has an unacceptably high death rate that has not changed for the last 15years. Mortality is partly related to organ hypoperfusion and mechanical assist devices are used for the most severe cases but we do not know which assist device is the best option. Therefore, we have investigated how an IABP and an Impella®-pump influenced blood flow to the brain, heart and kidneys, in a closed-chest porcine model of severe left ventricular failure. METHODS: 13 pigs were anesthetised and left ventricular failure was induced

Bridge to transplantation with the MicroMed DeBakey ventricular assist device axial pump: a single centre report.

OBJECTIVE: Left ventricular assist devices (VADs) are an accepted therapy to bridge patients with end-stage heart failure to heart transplantation. The DeBakey VAD, a continuous axial flow pump weighing 93 g, has been introduced into clinical practice as a bridge to transplant. METHODS: Starting from April 2000,17 patients (12 males, five females, mean age 44.3 +/- 12.8 years; 11 dilated idiopathic cardiomyopathy, five ischaemic cardiomyopathy, one pulsatile device failure) with end-stage heart failure were implanted with a DeBakey VAD as a bridge to transplantation at our institution. A review regarding the article 'Effectiveness of mechanical circulatory support devices in reversing pulmonary hypertension among heart transplant candidates: A systematic review.'.

Pulmonary hypertension (PH) with high pulmonary vascular resistance (PVR) is a very often diagnosed contraindication for orthotopic heart transplantation (OHT). It is a direct consequence of left ventricle failure characterized by high diastolic pressure obstructing the collection of blood from the pulmonary vessels. The occurrence of this situation grows with the increasing time of waiting for OHT, and with the progression of heart failure. Mechanical circulatory support (MCS) devices, particularly left ventricular assist devices (LVADs), have emerged as pivotal interventions for patients wit

Anesthetic management of children with in situ Berlin Heart EXCOR.

Modern mechanical devices can support children with severely impaired cardiac function until a donor heart is found for transplantation or native function recovers. Pediatric heart transplantation offers a good chance of survival with a high quality of life to individuals with limited life expectancy and/or severe limitation to daily activities, but many die on the transplant list or are not listed because of a shortage of donor organs. In recent cohorts, there is better outcome when ventricular assist devices (VADs) rather than extracorporeal membrane oxygenation are used as a 'bridge' to tra

Cardiac extracorporeal life support: state of the art in 2007.

Mechanical circulatory support is an invaluable tool in the care of children with severe refractory cardiac and or pulmonary failure. Two forms of mechanical circulatory support are currently available to neonates, infants, and smaller children, namely extracorporeal membrane oxygenation and use of a ventricular assist device, with each technique having unique advantages and disadvantages. The intra-aortic balloon pump is a third form of mechanical support that has been successfully used in larger children, adolescents, and adults, but has limited applicability in smaller children. [CardioWest, a complete artificial heart, the French experience].

The development of implantable electric assist systems for the left ventricle cannot be used in all patients. Approximately one-third of them who could benefit from circulatory support while waiting for transplantation also require a supporting system for the right ventricle. In addition, the small number of available donor organs means that only those patients with the best prognosis can be transplanted. From January 1993 to December 1995, 36 patients (33 men, 3 women) mean age 41 years (range 19-60) were treated with a total artificial heart system, CardioWest at the La Pitié Hospital in Par

Short-term mechanical circulatory support as a bridge to durable left ventricular assist device implantation in refractory cardiogenic shock: a systematic review and meta-analysis.

Short-term mechanical circulatory support (MCS) is increasingly used as a bridge to decision in patients with refractory cardiogenic shock. Subsequently, these patients might be bridged to durable MCS either as a bridge to candidacy/transplantation, or as destination therapy. The aim of this study was to review support duration and clinical outcome of short-term MCS in cardiogenic shock, and to analyse application of this technology as a bridge to long-term cardiac support (left ventricular assist device, LVAD) from 2006 till June 2016. Using Cochrane Register of Trials, Embase and Medline, a
