Diagnosis, prevention, and management of bleeding episodes in Philadelphia-negative myeloproliferative neoplasms: recommendations by the Hemostasis Working Party of the German Society of Hematology and Medical Oncology (DGHO) and the Society of Thrombosis and Hemostasis Research (GTH).

Philadelphia-negative myeloproliferative neoplasms (Ph-negative MPN) comprise a heterogeneous group of chronic hematologic malignancies. The quality of life, morbidity, and mortality of patients with MPN are primarily affected by disease-related symptoms, thromboembolic and hemorrhagic complications, and progression to myelofibrosis and acute leukemia. Major bleeding represents a common and important complication in MPN, and the incidence of such bleeding events will become even more relevant in the future due to the increasing disease prevalence and survival of MPN patients. This review discu

Dual-scope method for hemostasis during transbronchial lung cryobiopsy: A two-patient case report.

RATIONALE: Bleeding during transbronchial lung cryobiopsy can be clinically consequential. Conventional balloon occlusion and sequential 2-scope techniques may limit visualization or require bronchoscope exchange. We developed a dual-scope method in which an ultrathin bronchoscope for sampling and a therapeutic bronchoscope for balloon control are positioned simultaneously. PATIENT CONCERNS: An 84-year-old man with a 0.6-cm peripheral right upper lobe nodule after prior resection of squamous cell carcinoma and a 55-year-old woman with a 1.2-cm peripheral right lower lobe nodule during surveil

Nanomangostin-loaded chitosan aerogel: a multifunctional biomaterial for hemostasis, anti-bacteria and wound healing.

The rise in wound infections underscores the need for chitosan-based biomaterials, which, when loaded with bioactive agents, provide antibacterial, wound-healing, and effective long-term drug delivery capabilities. In this study, a chitosan-based dressing loaded withα-mangostin was successfully fabricated in the form of an aerogel. The new aerogel, incorporatingα-mangostin prepared as nanoparticles (nanomangostin), exhibited multifunctional activities including wound healing, hemostasis, and antibacterial effects. A crosslinked network structure was created using glutaraldehyde (GA) at a conce…

Quinone-mediated, tissue-adaptive double-network hydrogel for instant hemostasis and wet-tissue adhesion.

Rapid control of bleeding in complex wounds remains a major clinical challenge. Here, we develop a tissue-adaptive double-network hydrogel that enables instant adhesion to wet tissue and rapid hemostasis. We show that the material can be applied either as an injectable sealant for confined sites or as a conformal patch for large defects. The hydrogel is formed from interpenetrating polymer networks dynamically crosslinked to provide mechanical toughness, rapid self-healing, and shear-thinning injectability. We demonstrate that partial oxidation of catechol groups generates reactive quinone spe…

Visible-Light-Curable Catechol-Reinforced Gelatin Bioglue for Rapid Hemostasis and Engineered Tissue Assembly.

Hydrogel-based tissue adhesives hold promise for diverse biomedical applications, yet achieving strong, durable adhesion to wet, dynamic biological surfaces remains challenging. The catechol-containing amino acid 3,4-dihydroxy-l-phenylalanine (DOPA) is a crucial functional motif in mussel-inspired adhesives and has been extensively investigated for incorporation into various biomaterials. However, conventional approaches of DOPA incorporation and subsequent crosslinking have shown limited potential for clinical translation. We developed a bioinspired strategy involving pH-controlled enzymatic

Double crosslinked cryogels based on glycidyl methacrylate modified chitosan for hemostasis and wound healing.

Post-traumatic wound management is a critical issue that needs to be addressed. Chitosan (CS) with inherent biocompatibility and biodegradability is widely applied in wound healing, but the products of CS often suffer from poor water solubility and mechanical strength. Herein, we developed new double-crosslinked CS-based cryogels. Firstly, glycidyl methacrylate (GMA) was used to modify CS for the crosslinking of double bonds, followed by further crosslinking with 1,4-butanediol diglycidyl ether (BDDE). A series of CS-based cryogels were prepared by adjusting the concentration of CS from 2wt% t

Blunt Splenic Injury in Hemophilia A: Embolic Agent Selection When Hemostasis Cannot be Assumed.

Blunt splenic injury in a patient with hemophilia A presents a dilemma that has traditionally been settled by splenectomy. Removing the spleen leaves a patient already prone to bleeding without its immune function, and the factor VIII (FVIII) exposure that open surgery requires is itself a recognized trigger for inhibitor formation. In the general trauma population, nonoperative management is now the standard for hemodynamically stable patients: observation suffices in most, and splenic artery embolization is reserved for higher-risk injuries. Patients with inherited coagulopathy, however, sit

Enhancing hemostasis potency in hemophilia with a small interfering ribonucleic acid targeting protein S.

BACKGROUND: One hemophilia treatment concept focuses on rebalancing coagulation and anticoagulation to restore normal blood clotting. Target … argeted PS small interfering RNA (siRNA) therapy using N-acetylgalactosamine conjugation to restore hemostasis in hemophilia without increasing thromboembolic risks. METHODS: We assessed TG in plasma from patients with HA and HB. To target the liver specifically, we developed a PS-siRNA conjugated with N-acetylgalactosamine. This approach ensures that PS le …

A lateral suboccipital approach for hemostasis in removing transorbital, transcavernous penetrating chopsticks: illustrative case.

BACKGROUND: Transorbital penetrating objects traversing the cavernous sinus can cause life-threatening vascular injuries. Here, the authors report a unique case of bilateral transorbital penetrating plastic chopsticks traversing the cavernous sinus, in which a lateral suboccipital approach was used to manage bleeding from the posterior wall of the cavernous sinus. OBSERVATIONS: A 39-year-old man attempted suicide by stabbing plastic chopsticks through his orbits into the intracranial compartment. On admission, he had lost vision and exhibited complete ophthalmoplegia. A CT scan revealed chops

Red cell microparticles produced using high-pressure extrusion enhance both primary and secondary hemostasis.

BACKGROUND: Current therapies to treat excessive bleeding are associated with significant complications, which may outweigh their benefits. Red blood cell-derived microparticles (RMPs) are a promising hemostatic agent. Previous studies demonstrated that they reduce bleeding in animal models, correct coagulation defects in patient blood, and have an excellent safety profile. However, their exact mechanism of action is not known. We investigated the potential role of RMPs on primary and secondary hemostasis. METHODS: To evaluate the effects of RMPs, prepared using high-pressure extrusion, on pr…

Polyethylene Glycol (PEG)-Based Wet-Adhesive Absorbable Bone Wax for Osseous Hemostasis and Repair.

The non-absorbability and biological inertia of traditional bone wax often result in foreign body retention, inhibit bone healing, and increase the risk of infection. Herein, a novel absorbable bone wax was developed by mixing polyethylene glycol (PEG), tannic acid (TA), sodium carboxymethyl cellulose (CMC), and low-molecular-weight poly-L-lactic acid (PLLA) via one-pot method. Among them, PEG functions as the matrix, endowing it with plasticity and hydrophilicity, thereby facilitating its facile, close fit to the surface of bone defects. TA, with its profusion of phenolic hydroxyl groups, bes

ENDOSCOPIC INJECTION HEMOSTASIS AND LOCAL TREATMENT OF GASTRODUODENAL BLEEDING. LITERATURE REVIEW AND OWN DEVELOPMENTS.

Gastric ulcer and duodenal ulcer remain one of the most common and dangerous pathologies, accompanied by a high level of gastrointestinal bleeding. Endoscopic methods of hemostasis, including injection and combined methods, are the basis for the treatment of gastroduodenal bleeding, but there is a problem of high risk of relapse and insufficient effectiveness of individual methods. This article discusses modern approaches to endoscopic hemostasis in ulcer bleeding, including injection of vasoconstrictors and the use of the latest methods of local treatment. Particular attention is paid to the …

Calcium (Ca2+) hemostasis, mitochondria, autophagy, and mitophagy contribute to Alzheimer's disease as early moderators.

This review rigorously investigates the early cerebral changes associated with Alzheimer's disease, which manifest long before clinical symptoms arise. It presents evidence that the dysregulation of calcium (Ca2+) homeostasis, along with mitochondrial dysfunction and aberrant autophagic processes, may drive the disease's progression during its asymptomatic, preclinical stage. Understanding the intricate molecular interplay that unfolds during this critical period offers a window into identifying novel therapeutic targets, thereby advancing the treatment of neurodegenerative disorders.
