Herpes simplex virus (HSV) typically causes mucocutaneous disease, encephalitis, and acute men ingitis. There have been no previous reports of chronic meningitis due to this virus. A case of chronic meningitis due to herpes simplex virus type 2 (HSV-2) in a previously healthy 35-year-old woman whose predominant symptoms were headache and meningism without fever is described. Analysis of cerebrospinal fluid (CSF) revealed a lymphocytic pleocytosis, elevated protein, and hypoglycorrhachia. The diagnosis of herpes simplex meningitis was supported by the detection of HSV-2 DNA in CSF by polymerase…

Herpetic tracheobronchitis and pneumonia occur basically in immunodepressed patients, but have rarely been reported in patients with the acq … ing systemic corticotherapy and mechanical ventilation. It illustrates the risk of dissemination of herpes simplex virus (HSV) from a herpetic stomatitis to the lower respiratory tract, even after the endotracheal cannula has been removed.

In the experimental animals, the author succeeded in establishing the pathogenicity of herpes simplex virus (HSV) in the facial nerve. KOS strain of HSV type 1 was inoculated to the tongue or the auricle of the balb/c mouse previously treated by intraperitoneal injection of cyclophosphamide. Between 4 and 12 days after the inoculation, the facial nerves were resected and submitted to histopathological and immunohistochemical investigations. Inflammatory changes were mainly observed in the geniculate ganglion and its proximal portion of the facial nerve irrespective of the site of the inoculati…

Herpes simplex virus (HSV) infection of the hand occurs predominantly in three different population groups. Young adults with a recurrent HSV II infection of the hand account for the majority of cases. A prodromal phase of up to 72 hours and a recurrence of seven to 10 days' duration occasionally associated with lymphangitis, lymphadenopathy, and lymphedema are characteristic. HSV I infection of the hand classically occurs in children with herpetic stomatitis and in health care workers infected during patient care delivery. In health care workers, the infection may last 21 to 28 days and be as…

We describe a homosexual man with simultaneous infection of the conjunctiva by Herpes simplex virus and Chlamydia trachomatis. This dual infection was associated with a genital and disseminated Herpes simplex virus infection as well as asymptomatic chlamydial infection of the rectum and "nonspecific" urethritis. The findings in this case show the importance of laboratory investigation in cases of conjunctivitis associated with genital infection.

The authors performed a decision analysis to determine whether a patient suspected to have herpes simplex encephalitis (HSE) should undergo a brain biopsy or be treated empirically with medical therapy. In most cases, empiric treatment with acyclovir would be slightly favored; brain biopsy was not essential in management. However, brain biopsy was found useful for patients who had low CSF glucose at the time of initial lumbar puncture; such patients may have a very high risk to have other treatable conditions such as tuberculosis, brain abscess, toxoplasmosis, or cryptococcosis. The results of…

Considerable variations in the disease make an accurate portrayal of genital herpes simplex virus (HSV) infections quite difficult. Primary genital HSV infections classically produce ulcerated lesions lasting a mean of 16-21 days in men and 10-16 days in women. HSV shedding has been documented for a mean of 9.1-11.6 days in men and 8.0-14.7 days in women. Probably many adults have a less severe initial genital infection, however, and it may be so mild as to be misdiagnosed or undetected. Studies of recurrent genital HSV infections are often misleading because they overestimate the frequency an…

The computed tomographic (CT) findings in six cases of neonatal herpes simplex encephalitis (HSE) are reviewed and compared with previous reports. The diagnoses were made on the basis of isolation of the virus from a brain biopsy specimen in one case, from cerebrospinal fluid in two cases, from tracheal aspirate in one case, and on clinical grounds in two cases. Five infants survived; all had significant neurologic deficits. CT showed bilateral cerebral involvement with relative sparing of the lower neuraxis in all cases. Bilateral patchy low-density zones involving the periventricular white m…

Excretion of herpes simplex virus (HSV) in the oral cavity was studied in eight human subjects with a history of herpes labialis. Serial intraoral specimens were obtained by gargling broth and examined for virus by centrifugal inoculation of primary human amnion cells. Forty-seven of 637 specimens (7.4%) contained HSV. The majority of isolates (62%) were found in clusters, and the rate of excretion was significantly increased during the common cold (21%) and after oral trauma (17%) (P = 0.001 and 0.04, respectively). Oral HSV excretion often occurred in parallel with episodes of herpes labiali…

In this study the HLA antigen frequency was determined in a group of patients with erythema multiforme and in a separate group of patients with recurrent herpes simplex to establish whether either disease had any antigens whose frequency deviated from that of a normal population. The frequency of HLA-B15 was significantly increased in both patient groups (P less than 0.5). The increase in the erythema multiforme group does not appear to be due to the fact that some of the erythema multiforme patients have associated herpes simplex and this increase remained significant after further correction…

Two fatal cases of sporadic herpes simplex virus (HSV) encephalitis were seen and diagnosed at the Sir Charles Gairdner Hospital, Perth, between 1979 and 1981. Necropsy studies were directed to the possible portal of entry of HSV into the central nervous system. Viruses were present in the left olfactory nerve in one patient. No viruses were found in the Gasserian ganglia of either patient. The finding of HSV within an olfactory nerve provides further evidence of an olfactory portal of entry to the central nervous system in sporadic human HSV encephalitis.

A term newborn suffered disseminated herpes simplex virus (HSV) type II infection five days after cesarean section delivery for fetal distress. The mother had no history or evidence of herpetic lesions; the father had a history of genital herpetic lesions. The infant's terminal course was dominated by disseminated intravascular coagulation (DIC) with hepatic and renal failure. Microscopic examination revealed a necrotizing vasculitis of small and medium-sized lung and peripancreatic arteries. Nuclear inclusions characteristic of HSV were found in these arteries, as well as in the adrenal paren…

The treatment of herpetic corneal ulcerations with trifluorthymidine (F3T) has been evaluated in a clinical trial. F3T was found to be an effective antiviral agent for herpetic keratitis and a valuable alternative to idoxuridine (IDU)-therapy. Clinically IDU-resistent herpetic ulcers responded favourably to treatment with F3T.

Six cases of bullous perphigoid in association with rheumatoid arthritis have previously been reported in the literature. In the case reported in this paper, herpes simplex virus was found in the vesicular fluid. It is the first time this has been reported to this author's knowledge; this finding may contribute to the knowledge of the ultimate etiology of pemphigoid.
