Ventilation strategy for video-assisted thoracoscopic clipping of patent ductus arteriosus in children.

Video-assisted endoscopic techniques have reduced operative trauma in adult thoracic and general surgery but its application in children with congenital heart disease has been limited. We report the use of video-assisted thoracoscopic (VAT) technique of clipping patent ductus arteriosus (PDA) in children. Forty patients with PDA were divided into two groups: during VAT surgery patients in group A [mean age=3.6 +/- 2.4 (SD) years] were managed with right main stem bronchial intubation and those in the group B [mean age=3.7 +/- 2.7 (SD) years] received low tidal volume-high frequency ventilation

[Inhaled nitric oxide: a physiologic treatment of persistent pulmonary arterial hypertension in the newborn].

Fetal pulmonary circulation is characterized by high resistance and low pulmonary blood flow. Right-to-left shunting through the foramen ovale and/or patent ductus arteriosus is necessary to perfuse the placenta and insure fetal life. At birth, pulmonary arterial blood flow increases immediately by 8- to 10-fold, and allows pulmonary gas exchange and postnatal life. In some circumstances, this adaptation to extra-uterine life is inadequate, because of persistent high pulmonary resistance (PPHN). Due to the lack of a selective pulmonary vasodilator, the treatment of this syndrome remained purel

Rigid bronchoscopy and jet ventilation in an extremely low birthweight infant.

Until recently, rigid bronchoscopy was considered too risky for premature infants. We report a 658 g infant with life-threatening mediastinal shift due to right pulmonary atelectasis. Rigid bronchoscopy revealed tenacious mucous plugs in the right main bronchus. The collapsed lung was successfully reinflated after bronchial lavage with a rigid bronchoscope, with the help of intermittent high-frequency jet ventilation (HFJV). The infant's mediastinal shift was reversed and the vital signs became stable. In our experience, rigid bronchoscopy combined with HFJV is a valuable diagnostic and therap

[Persistent pulmonary hypertension of newborn. The PFC syndrome].

Persistent pulmonary hypertension of the newborn (PPHN), initially described by Gersony et al as persistent foetal circulation (PFC syndrome), results from a flawed transition from foetal to extrauterine pulmonary circulation. It is characterised by the maintenance of a high pulmonary vascular resistance and right-to-left shunting through the ductus arteriosus and foramen ovale. Infants with a wide variety of underlying clinical conditions develop PPHN. According to Rudolph three main anatomic types of PPHN can be identified: normal pulmonary vascular development increased pulmonary vascular s

[Ventilation with metallic tubes in CO2 laser surgery of the larynx].

One of the risks of laryngeal laser surgery is the ignition of the disposable plastic endotracheal tubes used for ventilation. In 1985, Hunton and Oswal (Anaesthesia, 40: 1210, 1985) developed a metallic tube which is now available in France. In this study, the tube has been tested on 30 patients undergoing this type of surgery. The tube was sterilized in glutaraldehyde before re-using it. There never was any problem with placing the tube and surgical conditions were satisfactory. There was neither incident due to the laser, nor any difficulties with the use of high-frequency jet-ventilation.

Congenital diaphragmatic hernia: impact of preoperative stabilization. A prospective pilot study in 13 patients.

In case of congenital diaphragmatic hernia (CDH), survival generally depends not on prenatal diagnosis, planned delivery, and immediate postnatal operation, but on the gravity of pulmonary hypoplasia and persistent hypertension (PPH). Many vasoactive drugs have become available for lowering PPH, but the mortality rate for CDH still amounts to 40% to 70%. Preoperative stabilization might prevent or at least reduce the risk of PPH. This method was evaluated in a pilot study lasting 15 months and involving 13 patients. All were admitted to the pediatric surgical intensive care unit within six hou

Respiratory Management of the Extremely Premature Infant.

Advancements in the fields of obstetrics and neonatal medicine, notably the use of antenatal corticosteroids and exogenous surfactant, have significantly improved the chances of survival for babies born prematurely. Extremely premature infants, born before 28 weeks of gestation, have underdeveloped lungs and pulmonary vasculature, necessitating a carefully tailored respiratory management strategy that incorporates both noninvasive and invasive methods throughout their NICU stay. The primary goal of respiratory management in this cohort is to provide the least amount of support necessary while

Anesthetic Techniques for Ablation in Atrial Fibrillation: A Comparative Review.

Atrial fibrillation, the most prevalent cardiac arrhythmia, has witnessed significant advancements in treatment modalities, transitioning from invasive procedures like the maze procedure to minimally invasive catheter ablation techniques. This review focuses on recent improvements in anesthetic approaches that enhance outcomes in catheter atrial fibrillation ablation. We highlight the efficacy of contact force sensing catheters with steerable introducer sheaths, which outperform traditional catheters by ensuring more effective contact time and lesion formation. Comparing general anesthesia wit

Inspiratory high frequency airway oscillation attenuates resistive loaded dyspnea and modulates respiratory function in young healthy individuals.

Direct chest-wall percussion can reduce breathlessness in Chronic Obstructive Pulmonary Disease and respiratory function may be improved, in health and disease, by respiratory muscle training (RMT). We tested whether high-frequency airway oscillation (HFAO), a novel form of airflow oscillation generation can modulate induced dyspnoea and respiratory strength and/or patterns following 5 weeks of HFAO training (n = 20) compared to a SHAM-RMT (conventional flow-resistive RMT) device (n = 15) in healthy volunteers (13 males; aged 20-36 yrs). HFAO causes oscillations with peak-to-peak amplitude of

A Hunsaker Mon-Jet tube trapped in the larynx.

OBJECTIVE: We report a unique complication arising from the use of a Hunsaker Mon-Jet ventilation tube during microlaryngeal surgery, and we briefly review the literature on the use of this tube for jet ventilation. METHOD: A case report on the safe and successful management of a potential airway compromise, and a brief literature review on using the Hunsaker Mon-Jet ventilation tube during microlaryngeal surgery. CASE REPORT: A 46-year-old woman was scheduled to undergo removal of a vocal cord polyp under general anaesthesia. However, the polyp became stuck in the basket of the Hunsaker tub

Randomized controlled trial of early compared with delayed use of inhaled nitric oxide in newborns with a moderate respiratory failure and pulmonary hypertension.

OBJECTIVE: To evaluate whether early treatment with inhaled nitric oxide (iNO) will prevent newborns with moderate respiratory failure from developing severe hypoxemic respiratory failure (oxygenation index (OI)>or=40). STUDY DESIGN: A total of 56 newborns with moderate respiratory failure (OI between 10 and 30) were randomized before 48 h after birth to early treatment with 20 p.p.m. of iNO (Early iNO group, n=28) or conventional mechanical ventilation with FiO(2) 1.0 (Control group, n=28). Infants received iNO and/or high-frequency oscillatory ventilation (HFOV) if they developed an OI>40.

Factors effecting adoption of new neonatal and pediatric respiratory technologies.

OBJECTIVE: There remains significant variation in the level and rate of adoption of new pediatric respiratory technologies, in spite of two … nal neonatal/pediatric respiratory conferences. We asked about use of four specific indications for high-frequency ventilation (HFV) and nasal continuous positive airway pressure (nCPAP). RESULTS: These four specific respiratory therapies were aggressively used by most, despite significant differences in the evidence supporting their utility: elective use of HFV (57.4%); H …

Emergency tracheal catheterization for jet ventilation: a role for the ENT surgeon?

Stridor causing respiratory failure is an ENT and anaesthetic emergency requiring prompt management to secure a clear airway. We describe a case of subacute partial upper airway obstruction due to a large laryngeal carcinoma in an 81-year-old male resulting in respiratory failure. The patient became apnoeic after gaseous induction of general anaesthesia, and after two failed intubation attempts an emergency transtracheal airway catheter was placed by the surgeon under direct vision below the cricothyroid membrane, as this had tumour involvement. The patient was subsequently manually jet-ventil

Endogenous pulmonary surfactant metabolism is not affected by mode of ventilation in premature infants with respiratory distress syndrome.

OBJECTIVE: To determine if high frequency oscillatory ventilation (HFOV) decreases surfactant production in premature infants with respiratory distress syndrome (RDS). STUDY DESIGN: We randomized 19 infants <28 weeks of gestation to either HFOV (n = 8) or conventional ventilation (CV, n = 11) at 24 hours of life. After a 24-hour continuous infusion of uniformly labeled carbon 13 glucose (U-(13)C(6)) glucose, we measured (13)C enrichment in surfactant phosphatidylcholine (PC) in tracheal aspirate samples using gas chromatography/mass spectrometry. We calculated the fractional synthetic rate (F

Determinants of oxygenation during high frequency oscillation.

Two studies are reported in which the aim was to assess whether oxygenation on transfer to or during high frequency oscillation (HFO) was influenced by the mean airway pressure (MAP) level. Sixteen infants, median gestational age 28 weeks, were recruited into the first study and 14 with a median gestational age of 29 weeks into the second. In the initial study, blood gases were measured immediately before and 30 min after transfer to HFO and comparison made of those infants in whom oxygenation did or did not improve. In the second study the infants were studied at two MAP levels, 2 and 5 cmH2O
