Social determinants of mental health care systems: intensive community based Care in the Veterans Health Administration.

BACKGROUND: Since deinstitutionalization in the 1950s-1970s, public mental health care has changed its focus from asylums to general hospitals, outpatient clinics and specialized community-based programs addressing both clinical and social determinants of mental health. Analysis of the place of community-based programs within a comprehensive health system such as the Veterans Health Administration (VHA) may illuminate the role of social forces in shaping contemporary public mental health systems. METHODS: National VHA administrative data were used to compare veterans who exclusively received

Barriers to Palliative Care for Terminally Ill Children in a Developing Country Cancer Hospital: A Qualitative Study.

INTRODUCTION: Pediatric palliative care (PPC) aims to improve the quality of life for children with life-limiting illnesses and support their families by addressing physical, psychological, social, and spiritual needs. Globally, significant barriers to PPC delivery include limited integration into health systems, inadequate provider training, restricted access to pain medications, and communication challenges. These issues are particularly pronounced in low- and middle-income countries, where cultural, infrastructural, and resource constraints further complicate care. Despite growing recogniti

Evaluation of Sepsis Management in a Regional Community Hospital.

INTRODUCTION: Sepsis remains a leading cause of global mortality. Despite established guidelines by the Surviving Sepsis Campaign (SSC), adherence to time compliance with the SSC bundle is often inconsistent. In New Brunswick hospitals, this compliance has not been studied. This study evaluates compliance with the SSC bundle in a regional community hospital in New Brunswick (NB), Canada, and its impact on clinical outcomes. METHODS: A retrospective, single-center observational study was conducted to review sepsis and septic shock cases over 6 months. Patients were categorized into two groups:

Bridging Research and Care: Integrating Patient-Reported Outcome Measures from REDCap into Clinical Routine.

INTRODUCTION: Patient-reported outcome measures (PROMs) provide essential insights into patients' symptoms, functional status, and health-related quality of life, supporting value-based and patient-centered care. Although widely collected in research platforms such as REDCap, PROMs are rarely integrated into clinical routine, limiting their clinical impact. Legal frameworks in the European Union, including General Data Protection Regulation and the Medical Device Regulation (MDR) further constrain data use, especially when automated score calculation or decision support is involved. Fostering safer intensive care units: How incident reporting drives cultural change.

BACKGROUND: Patients admitted to intensive care units (ICUs) are more prone to patient safety incidents (PSIs), with substantial negative effects. Patient safety culture (PSC) is seen as a crucial pillar to address these safety problems. The aim of our study was to evaluate the impact of the implementation of a PSIs reporting system on PSC in ICUs. METHODS: We conducted a quasi-experimental study in 15 ICUs in two Tunisian university hospitals. One hospital was chosen as the intervention site (10 ICUs), where a PSI reporting system was implemented, while the other served as the control (five

Economics of decriminalizing mental illness: when doing the right thing costs less.

The United States' criminal justice system has seen exponential growth in costs related to the incarceration of persons with mental illness. Jails, prisons, and state hospitals' resources are insufficient to adequately treat the sheer number of individuals cycling through their system. Reversing the cycle of criminalization of mental illness is a complicated process, but mental health diversion programs across the nation are uniquely positioned to do just that. Not only are these programs providing humane treatment to individuals within the community and breaking the cycle of recidivism, the p

Forecasting Local Surges in COVID-19 Hospitalizations through Adaptive Decision Tree Classifiers.

IntroductionDuring the COVID-19 pandemic, many communities across the United States experienced surges in hospitalizations, which strained the local hospital capacity. Some risk metrics, such as the Center for Disease Control and Prevention's (CDC's) Community Levels, were developed to predict the impact of COVID-19 on the community-level health care system based on routine surveillance data. However, they had limited utility as they were not routinely updated based on accumulating data and were not directly linked to specific outcomes, such as surges in COVID-19 hospitalizations beyond local

Examination of the relative importance of hospital employment in non-metropolitan counties using location quotients.

INTRODUCTION: The US Health Care and Social Services sector (North American Industrial Classification System 'sector 62') has become an extremely important component of the nation's economy, employing approximately 18 million workers and generating almost $753 billion in annual payrolls. At the county level, the health care and social services sector is typically the largest or second largest employer. Hospital employment is often the largest component of the sector's total employment. Hospital employment is particularly important to non-metropolitan or rural communities. A high quality health

Turning Compliance Into Action: An Augmented Intelligence-Enabled Framework for Advocacy Organizations to Drive Population Health Improvement.

Community Health Needs Assessments (CHNAs), mandated by the Affordable Care Act for tax-exempt hospitals, represent an underutilized yet rich data source for disease-specific advocacy. This commentary proposes a novel framework in which disease advocacy organizations-such as Alzheimer's Los Angeles, the American Heart Association, and the National Alliance on Mental Illness-deploy artificial intelligence (AI) agents to systematically analyze CHNAs, identify gaps in condition-specific care, generate personalized outreach to hospital leadership, and publicly score health systems on their respons
