OBJECTIVE: To study the methods and the results of operative treatment of displaced proximal humeral fractures.
METHODS: We reviewed thirty-one patients who had been diagnosed as having displaced proximal humeral fractures and had been operated on from July 1989 to December. 1998 in our hospital. The mean follow-up time was 40.5 months (8 - 124 months). Their age ranged from 15 to 62 years (average, 36.8 years); 18 patients were male and 13 female. Neer fracture classification system and rating system were used. In all patients, delto-pectoral approach was adopted. Twelve fractures were fixed…

We report our experience with a modified implant and a new technique for locked intramedullary nailing of the humerus in 41 patients. Locking was by cross-screws placed from lateral to medial in the proximal humerus, and anteroposteriorly in the distal humerus. Early in the series, 11 nails were inserted at the shoulder, but we found that rehabilitation was faster after retrograde nailing through the olecranon fossa, which was used for the other 30. We used a closed technique for 29 of the nailings. Of the 41 patients treated, 21 had acute fractures, five had nonunion, and 15 had pathological

OBJECTIVE: To describe the anterior approach for open reduction of supracondylar fractures in children and to assess the morbidity and functional outcome.
DESIGN: Retrospective analysis.
SETTING: University Hospital.
SUBJECTS: Eleven consecutive children (mean age 6 years, range 4-12) with supracondylar fractures who were treated by open reduction and fixation with a Kirschner wire through an anterior approach from 1984-1990 and followed up after a mean of 9 weeks (range 5-16 weeks).
RESULTS: There was no morbidity. Full functional recovery was achieved within four months of the accident.

Hackethal's technique of stacked intramedullary (IM) nailing was used to treat 43 consecutive acute fractures of the humeral diaphysis. Of these, 6 were lost to follow-up and 4 additional patients were excluded for pathologic fractures. All of the remaining 33 traumatic fractures were followed until union. The average age was 34 years. Mean operative time was 90 min. Usually four or five 2- or 3-mm IM Kirschner wires were introduced in a retrograde manner, through a posterior unicortical window under radioscopic control. Closed reductions were performed in 28 patients, and open reductions were

We report about two patients with posterior fracture-dislocation of the shoulder following convulsive seizures, one case unilateral, one case bilateral. The radiological findings are typical and similar to the cases described in literature: Impression or head splitting fracture of the ventro-medial part of the humeral head associated with posterior dislocation. The mechanism of injury, methods of treatment and late results are discussed.

INTRODUCTION: Treatment of proximal humeral fractures with plate osteosynthesis or intramedullary nail fixation in humeral shaft fractures with a proximal locking bolt carries the risk of iatrogenic injury of the axillary nerve. The purpose of this anatomical study is to define a more reliable safe zone to prevent iatrogenic axillary nerve injury using the humeral head instead of the acromion as a (radiographic) reference point during operative treatment.
MATERIALS AND METHODS: Anatomical dissection and labeling of the axillary nerve and branches was performed on 10 specially embalmed human s…

BACKGROUND: Recent studies have identified specific subsets of diaphyseal humeral fractures for which functional bracing is less effective. The present study tested the hypothesis that a gap between fracture fragments may be a risk factor (after accounting for other potential risk factors) for fracture instability six weeks after functional bracing of humeral shaft fractures.
METHODS: We retrospectively identified seventy-nine adult patients (forty-six men, thirty-three women; forty-two fractures on the right side, thirty-seven fractures on the left), each with an acute, closed, AO type-A2 (o…

BACKGROUND: Humeral shaft fractures account for approximately 3% of orthopedic injuries each year. While the standard treatment is nonoperative management, surgery is performed in cases where fixation is required to restore alignment and optimize healing. The success of these procedures depends on a number of factors, including fracture characteristics, intraoperative fixation, and patient characteristics. This study was undertaken to better understand the effect of nicotine dependence on surgical outcomes following humeral shaft fracture surgical management. The study hypothesis was that comp

Management of humeral head fractures presents numerous challenges, particularly in cases of separation fractures, which are relatively rare and often caused by high-energy trauma. Due to the complexity of these fractures and the absence of a standardized treatment algorithm, surgical intervention can be technically intricate and fraught with postoperative complications. In young patients, the decision to pursue open reduction and internal fixation must be meticulously made to achieve optimal outcomes while minimizing long-term sequelae. We present a rare case of a man, age 25 years, sustaining

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) has proven to be an effective surgical procedure for irreparable rotator cuff tears, comminuted fractures of the proximal humerus, and shoulder arthroplasty revision surgeries. We know from the literature that the functional results are good in the short term, but not whether these results remain stable in the long term or, on the contrary, show deterioration in functionality.
OBJECTIVE: This study aims to analyze the functionality, degree of satisfaction, complications, and prosthesis survival at different cut-off points in patients with

INTRODUCTION AND IMPORTANCE: Plastic deformities usually occur in skeletally immature bones not completely ossified due to longitudinal forces on their distal ends, especially the forearm and lower legs. Pediatric lateral humeral condylar fractures (LHCFs) constitute the second most common intra-articular fracture in the upper extremity. Nevertheless, plastic deformities accompanied by other traumatic injuries, including LHCFs, are atypical and rare. This study presents an exceedingly rare case of the concurrence of a plastic deformity and an ipsilateral fracture of the lateral condyle of the

Background and Objectives: The proximal humeral fracture (PHF) is one of the most common fractures in elderly patients. A PHF might influence the quality of life (QoL) on several different levels, especially in elderly patients, but it is unclear which treatment option results in a better QoL outcome. Therefore, we aimed to systematically review the current literature for studies that have analyzed the QoL and pain of elderly patients treated either surgically or non-operatively for PHF. Materials and Methods: A comprehensive search of the literature was performed in the PubMed database from J

Open supracondylar humeral fractures (SHFs) are rare, and there have been few papers specifically addressing their management. In this report, we describe the management and 27-month outcomes of an open SHF. A healthy eight-year-old boy presented with open SHF of the right elbow and underwent percutaneous lateral fixation using two Kirschner wires (K-wire) after irrigation and debridement (I&D) through an anterior approach. Excellent outcomes were obtained without any complications over a 27-month follow-up period. The management of open SHFs in children is yet to be standardized; nevertheless…
