Rotator cuff tears are common. When indicated, surgical repair is a highly successful procedure. There are circumstances when there is not e … t: inserting a biodegradable balloon within the subacromial space to reduce the articulation of the humeral head on the acromion and aid in recentering the humeral head to restore balance to the remaining rotator cuff and improve deltoid function.

Four-fragment fractures have to be accorded great importance because they are so severe. The morphology allows differentiation of two types, … fractures were scored of 87.2 while the impacted fractures were scored at 88.8. No necrosis of the humeral head was found. These good results show that after four-fragment fractures primary reconstruction of the humeral head should be preferred over primary prosthetic replacement.

The biomechanics of posterior dislocation of the shoulder was evaluated using nine cadaver shoulders. Each was arthroscoped, roentgenograms were obtained, and then each was dislocated in a testing device. Force displacement data was obtained during testing, and posttesting roentgenograms, arthroscopic evaluation, and subsequent open dissections were performed to evaluate the pathology created. Although instability was created in all cases with displacement of the humerus to the diameter of the humeral head, force displacement of the humerus to the diameter of the humeral head, force displaceme…

The purpose of this study was to evaluate the axillary node displacement away from chest wall and their anatomical location in relation to the humeral head, according to the position of the arms, when the axilla is the site of enlarged nodes. In 13 patients with enlarged axillary nodes, the anatomical span of the nodes according to two arms positions, akimbo (A) and up over the head (U), was prospectively evaluated using computed tomography (CT) and magnetic resonance imaging (MRI). The nodes were classified into two groups, i.e. the lower and upper groups. The mean distances of the lower node…

Humeral head atraumatic avascular necrosis is a rare diagnosis. It concerns young patients with a high functional demand. The treatments are mostly surgical. The radiographic classification of Cruess assesses the severity of the humeral head avascular necrosis and guides surgical indications. This chapter reports a review of the literature based on meta-analyses and clinical series. Surgical treatments may be conservative or prosthetic. Conservative treatments include core decompression, bone grafting and arthroscopic debridement. Prosthetic replacements are performed by resurfacing, hemiarthr…

Three-dimensional (3D) printing technology is a reality in medicine. In Orthopedics and Traumatology, 3D printing guides a precise and tailored surgical treatment. Understanding and disseminating its applicability, use, and outcomes can foster academicism and improve patient care. This is a report of a rare case of a female young adult patient with osteonecrosis of the humeral head due to avascular necrosis developed in early childhood. The treatment was tailored and optimized with 3D printing, which helped determine the steps for partial humeral arthroplasty.

A rare case of a fracture of the anatomical neck of the humerus associated with a posterior dislocation of the shoulder in a 43-year-old man is presented. It was managed by immediate surgery using locking plate osteosynthesis. The shoulder was immobilised for 5 weeks in a Gilchrist brace. The outcome was very good with a full range of motion and a stable shoulder at 1 year after injury, with no signs of avascular necrosis of the humeral head. The aetiology of this injury and possibilities of its treatment are discussed. Key words: shoulder dislocation, humeral fractures.

The classification of the proximal humeral fractures according to the four-segment concept of Neer has proven to be very useful. It not only considers the fracture itself but also lesions of the soft tissue. Adapting Neer's concept we propose a modified classification of the proximal humeral fractures which we believe is clearer and more practicable. This classification is illustrated by means of drawings and typical roentgenograms. The description of the different fracture-types includes the mean characteristics such as fragment-displacement, status of the rotator cuff and blood circulation t

An erosive arthropathy of the hands and wrists has been recognized in patients with primary and secondary hyperparathyroidism [3, 5]. Recently, intra-articular erosions of the humeral head were described in six patients who had been on chronic long-term hemodialysis with secondary hyperparathyroidism [1]. We would like to present the finding of shoulder erosions in four patients with primary hyperparathyroidism and one patient with renal osteodystrophy and suggest that the humeral erosion can occur in both an intra-articular and peri-articular location.

Cup arthroplasty is a conservative bone-sparing option for resurfacing of the humeral head. Earlier reports have shown its effectiveness in appropriately selected patients with osteoarthritis, osteonecrosis, and severe rheumatoid arthritis. Patients with cuff tear arthropathy may also benefit from a modified cup arthroplasty technique. The purpose of this article is to describe the surgical technique involved in humeral cup arthroplasty in cuff tear arthropathy patients. We will review the indications, contraindications, complications, and postoperative rehabilitation.

Understanding the pathophysiology and treatment of rotator cuff disorders is the key to understanding all other aspects of shoulder rehabilitation. Impingement rehabilitation focuses on strengthening the humeral head depressors, while ignoring the deltoid and supraspinatus muscles. Later treatment includes specific retraining of scapular balancing muscles. The final phase of treatment includes strengthening the prime humeral movers in positions that avoid further stress to the injured rotator cuff tendons and, last of all, specifically strengthening the supraspinatus muscle.

BACKGROUND: Accurate humeral stem positioning is critical for achieving favorable outcomes after hemiarthroplasty for complex proximal humeral fractures. This study aimed to compare the clinical and radiological outcomes of hemiarthroplasty performed using the pectoralis major reference technique (PMRT) with those performed using a conventional technique (CT) METHODS: Thirty-nine patients who underwent hemiarthroplasty for complex proximal humeral fractures were included in this study. The conventional technique was used in the first 24 cases (CT group) and PMRT was applied in the subsequent 1

BACKGROUND: Large osseous defects of the posterosuperior aspect of the humeral head can engage the glenoid rim and cause recurrent instability after arthroscopic Bankart repair for glenohumeral dislocation. Filling of the humeral head defect with the posterior aspect of the capsule and the infraspinatus tendon (i.e., Hill-Sachs remplissage) has recently been proposed as an additional arthroscopic procedure. Our hypothesis is that the capsulotenodesis heals in the humeral bone defect without a severe adverse effect on shoulder mobility, allowing return to preinjury sports activity.
METHODS: Of…

BACKGROUND: A subacromial balloon spacer is an option to treat irreparable rotator cuff tears. We hypothesized that the balloon would restor … act shoulder in either the balanced or the unbalanced condition. The irreparable tear displaced the humeral head superiorly in the unbalanced condition, decreasing the acromion-humeral interval. The balloon moved the head inferiorly by a mean (and standard error of the mean) of 6.2 ± 1.3 mm (p < 0.001) at 0° of abduction, 4.4 ± 1.3 mm (p < 0.001) at 30°, and 3 …

BACKGROUND: Percutaneous insertion of third-generation straight humeral nails is a recent alternative to the conventional open method. Rather than splitting, retracting and subsequently repairing the supraspinatus fibers to visualize the humeral head entry site, the percutaneous approach utilizes a cannulated awl to enter the intramedullary canal through the supraspinatus fibers without visualizing internal shoulder structures. Despite recent evidence demonstrating satisfactory outcomes in the percutaneous method, the potential for iatrogenic injury to the rotator cuff and other shoulder struc…

OBJECTIVE: To evaluate the value of radiographs during the diagnostic work-up of rotator cuff tears, using arthroscopy as reference standard.
MATERIALS AND METHODS: This retrospective study included 236 shoulders of 236 patients. All radiographs were evaluated for inferior cortical acromial sclerosis, lateral acromial spur, superior migration of the humeral head, greater tubercle cysts, and subacromial space calcifications. Predictive value of these radiographic signs in predicting rotator cuff tears was determined with arthroscopy as reference standard.
RESULTS: According to arthroscopy, 13…

The interaction of various factors in the vicious cycle (VC) of subacromial impingement syndrome (SIS) is complex and there are conservative treatment preferences for speedy rehabilitation. The mechanism of SIS is not fully understood and the inappropriate treatment priorities cause delay in rehabilitation. SIS is related to the reduction in the subacromial space (SS). Posterior capsular tightness (PCT) and rotator cuff (RC) dysfunction are the two basic mechanisms in this regard. PCT may cause anterosuperior translation of humeral head (HH) and anterior acromion tipping through scapular dyski…
