Hydrocortisone-associated death and hospital length of stay in patients with sepsis: A retrospective cohort of large-scale clinical care data.

PURPOSE: Sepsis is one of the leading causes of morbidity and mortality worldwide with approximately 50 million annual cases. There is ongoing debate on the clinical benefit of hydrocortisone in the prevention of death in septic patients. Here we evaluated the association between hydrocortisone treatment and mortality in patients diagnosed with sepsis in a large-scale clinical dataset. METHODS: Data from patients between 2008 and 2019 were extracted from the retrospective Medical Information Mart for Intensive Care IV (MIMIC-IV) database. Patients who received hydrocortisone after diagnosis w…

Long-term prednisone versus hydrocortisone treatment in children with classic Congenital Adrenal Hyperplasia (CAH) and a brief review of the literature.

BACKGROUND: Debate still exist about the safety of long-term use of prednisone (PD) versus hydrocortisone (HC) for treating children with congenital adrenal hyperplasia -21OH D (CAH). Despite recent developments in congenital adrenal hyperplasia -21OH D (CAH), several issues related to patient growth and final height remain unsolved. Debate still exist about the safety of long-term use of PD versus HC for treating children with CAH. The mechanism by which glucocorticoid therapy interferes with growth is complex and multifactorial. Relatively slight supra-physiologic levels may be enough to blu…

Effect of alternate-day hydrocortisone therapy on the immunologically immature rat. II: Changes in T- and B-cell areas in spleen.

The histologic effect of 400 mg/M2/day of hydrocortisone, given alternatively from day 7 to day 19 after birth, was studied on the spleen of the immunologically immature rat. Two days after the cessation of treatment, the periarteriolar lymphatic sheaths were found to be largely depleted of small lymphocytes. Immunoperoxidase studies confirmed a depletion of T lymphocytes. The effects of hydrocortisone on the thymus seem to be more important than its direct lymphocytolytic effect in producing this splenic lesion. In contrast, no apparent change in the number of medium-sized B lymphocytes in th…

A randomized clinical trial of two topical preparations (framycitin/gramicidin and oxytetracycline/hydrocortisone with polymyxin B) in the treatment of external otitis.

In a randomized trial, 55 patients with acute external otitis were treated with either topical framycitin/gramicidin (Sofradex) or oxytetracycline/hydrocortisone (Terracortril) with polymyxin B (TPB) ear-drops for 1 week. Staphylococcus aureus and Pseudomonas pyocyanea were the bacteria most frequently found in the ear canal; 78% of the patients were cured. However, no significant differences in therapy were found when either of the preparations was used. S. aureus seemed to be most resistant to treatment, while P. pyocyanea was less of a therapeutic problem. Additionally, previous episodes of…

The effects of adrenalectomy and hydrocortisone replacement on the thyriod of the adult male rat I. Morphometrical data and histochemistry of some oxidative enzymes.

Morphometrical and histochemical studies were performed to determine the effects of adrenalectomy and hydrocortisone replacement on the rat thyroid. It follows from the performed studies that morphometrical and histochemical changes in the rat thyroid induced by adrenalectomy or hydrocortisone replacement depend upon the duration of the experiment. From the 4th day of the experiment adrenalectomy results in an increase in the volume fraction of the epithelium of the thyroid vesicles with a concomitant decrease of the colloid fraction. As compared to sham operated animals on the 10th day of the…

Cardiomyogenic differentiation of human sternal bone marrow mesenchymal stem cells using a combination of basic fibroblast growth factor and hydrocortisone.

The alarming rate of increase in myocardial infarction and marginal success in efforts to regenerate the damaged myocardium through conventional treatments creates an exceptional avenue for cell-based therapy. Adult bone marrow mesenchymal stem cells (MSCs) can be differentiated into cardiomyocytes, by treatment with 5-azacytidine, thus, have been anticipated as a therapeutic tool for myocardial infarction treatment. In this study, we investigated the ability of basic fibroblastic growth factor (bFGF) and hydrocortisone as a combined treatment to stimulate the differentiation of MSCs into card…

Comparative antiphlogistic activity of meseclazone, 5-chlorosalicylic acid, acetylsalicylic acid, phenylbutazone, indomethacin and hydrocortisone in various rat paw edema models.

The antiphlogistic activity of meseclazone and its major metabolite, 5-chlorosalicylic acid, have been directly compared to acetylsalicylic acid, phenylbutazone, indomethacin and hydrocortisone in rat paw edema induced by eleven different phlogistic agents. Based on the profile of activity and milligram/kilogram potency displayed by meseclazone, it most closely resembles acetylsalicylic acid and phenylbutazone in its mode of action.

Switching From Immediate-Release to Fractionated Dual-Release Hydrocortisone May Improve Metabolic Control and QoL in Selected Primary Adrenal Insufficiency Patients.

OBJECTIVE: The use of once-daily dual-release HC (DR-HC) in primary adrenal insufficiency (PAI) is often associated with benefits in metabolic parameters when compared to immediate-release HC (IR-HC). In this study, we evaluated the effects on clinical, biochemical and metabolic parameters of switching from IR-HC to lower-dose DR-HC given both in once and fractionated daily doses. METHODS: Twenty autoimmune-PAI subjects were included. Patients on 30 mg/day divided in three doses IR-HC regimen (group A) were switched to DR-HC 25 mg/day given in two daily doses (20 mg in the morning and 5 mg at
