A critical analysis of the postmortem diagnostic criteria for asphyxia leads to a difficulty that has significant implications for the forensic pathologist. The difficulty is that there are no universally recognized pathognomic signs of asphyxia, but pathologists frequently make this diagnosis based on observations that individually have indeterminate significance but combined together, in the appropriate context, have diagnostic value. This leads to the question: if asphyxia has no recognizable signs at autopsy how are we able to diagnose this entity in the latter circumstances? This problem

We describe a patient with a history of psychiatric disorder who was brought to our hospital after attempted suicide by hanging. Severe subcutaneous facial, palpebral and cervical emphysema was present, with dysphonia, dysphagia and slight respiratory difficulty. Fiberoptic bronchoscopy revealed upper airway obstruction due to edema in an intact airway. Successive CAT scans gave evidence of hyoid fracture and laryngocele, in addition to the corresponding emphysema of the subcutaneous area and pneumomediastinum. Given the persistence of dysphagia, we ordered esophageal tests, which showed funct

Elongated Styloid Process Syndrome requires its consideration in differential diagnosis of degenerative disease of the cervical spine. Radiating pain on certain movements of the cervical spine is a symptom of an elongated styloid process as well. First basic anatomy and pathology, symptoms and differential diagnosis of the elongated styloid syndrome are discussed. A case history is presented together with X-rays of the cervical spine and images of three dimensional CT-reconstruction to show the full length of an elongated styloid process.

Differential diagnosis of atypical facial pain can be a perplexing problem. Similarities in symptomatology and nonspecific subjective findings make diagnosis difficult. The astute diagnostician, regardless of his speciality, must be acutely aware of key findings that will differentiate the myriad of chronic pain syndromes in his patients. Accurate correlation of clinical and radiographic findings in patients with "Eagle's syndrome" may assist the clinician in distinguishing this entity from other pain syndromes, such as glossopharyngeal or vagoglossopharyneal neuralgia.

A thyroglossal duct cyst (TGDC) is the result of incomplete degeneration of the thyroglossal duct during gestation. It is the most common ty … cystic hygroma. Surgical resection showed a large cyst of 71 x 40 x 52 mm with no attachment to the hyoid bone and no invasion in blood vessels or other surrounding tissue. Histopathological investigation led to a diagnosis of TGDC.

Carotid endarterectomy (CEA) is an established surgical procedure for carotid stenosis. We present the case of a 74-year-old man who underwent CEA for symptomatic internal carotid artery (ICA) stenosis. During the operation, we found that the hyoid bone (HB) and the superior horn of the thyroid cartilage covered the carotid sheath, preventing adequate visualization. Since this was anticipated based on preoperative examinations, the superior horn of thyroid cartilage was removed with the help of an otorhinolaryngologist. The HB was preserved because it could be retracted, and thereafter we perf…

Knowledge of retropharyngeal carotid arteries in patients is important in order to avoid potential catastrophic injuries. Previous studies demonstrated that carotid arteries can change in position at or near the level of the hyoid bone on serial scans. We report the presence of wandering carotid arteries at the level of the cricoid cartilage and superiorly in the neck of a 74-year-old female patient over multiple months. The potential for carotid arteries to randomly change positions in the suprahyoid and infrahyoid neck should be known by clinicians to avoid misdiagnosing occult neck masses a…

Obstructive sleep apnoea syndrome is a type of sleep-disordered breathing that affects 1 to 5% of all children. Pharyngeal and palatine tonsil hypertrophy is the main predisposing factor. Various abnormalities are predisposing factors for obstructive sleep apnoea, such as decreased mandibular and maxillary lengths, skeletal retrusion, increased lower facial height and, consequently, increased total anterior facial height, a larger cranio-cervical angle, small posterior airway space and an inferiorly positioned hyoid bone. The diagnosis is based on the clinical history, a physical examination a…

The partial horizontal supracricoid laryngectomy with cricohyoidoepiglottopexy consists of resection of the whole thyroid cartilage and paraglottic space. The cricoid cartilage, the hyoid bone, most of the epiglottis, and at least one arytenoid cartilage are conserved. Thirty-six patients with squamous cell carcinoma of the glottis who underwent this procedure from 1974 through 1986 are presented. All 36 recovered physiologic deglutition and phonation. None required a permanent tracheotomy. The 3-year actuarial survival rate was 86.5%. The local recurrence rate was 5.5%. The indications for th…

Ectopic thyroid is a rare clinical disease, with the majority of the patient presented with hypo-functional thyroid tissue and absence of orthotopic thyroid gland. During embryogenesis, the thyroid gland descends from the foramen cecum to its normal position located below the larynx and hyoid bone, anterior to the 2nd, 3rd, and 4th tracheal cartilaginous rings. Any defect that occurs during this period will result in ectopic thyroid. Hence, ectopic thyroid is usually found along the course of the thyroglossal duct, laterally in submandibular glands and even in distant places such as mediastinu…

This paper reported a case of extramedullary plasmacytoma of the larynx. The patient presented with persistent hoarseness and foreign body sensation in pharynx for 5 months. Left anterior laryngeal mass, left epiglottis fold distention was found during laryngoscopy. Cervical image examination showed that the left side wall of the glottis was thickened and showed a lump-like change, part of which protrusion into the laryngeal cavity, the laryngeal chamber became narrow, and the growth of thyroid cartilage was broken outwards and reached the level of the hyoid bone. After completing the examinat…

Compressive syndromes of the cervical arteries caused by musculoskeletal structures include bow hunter's syndrome, beauty parlour stroke syndrome, carotid compression by the hyoid bone, carotid compression by the digastric muscle and Eagle syndrome. They are a rare but increasingly recognised group of syndromes, so a high level of suspicion is needed so the diagnosis is not missed. The diagnosis is typically based on a combination of clinical history and advanced imaging investigations. Compression of the arteries may be static (only provoked by compression) or dynamic (exaggerated by movement…

We evaluated the role of adenotonsillar hypertrophy and facial morphology in children with obstructive sleep apnea (OSA) and compared these data with an age-matched control group. We performed cephalometric analysis to evaluate facial morphology using lateral facilal roentgenograms. Adenotonsillar and maxillary hypertrophy was remarkable in OSA children. Maxillary protrusion was significantly smaller in the OSA group than in the control group in older children (5-9 years old). Mandibular protrusion was significantly smaller in the OSA group even at younger ages (1-2 years old). The hyoid bone …

The laryngocele is a dilatation of the laryngeal ventricle and represents a rare condition. In most cases there is a typical history. Patien … rnal laryngocele may proceed into an external one, which presents as a swelling at the level of the hyoid bone. We now present 13 cases of laryngoceles with special regard to the CT and intraoperative findings. In the differential diagnosis other tumors of the larynx are considered, since the mucus-filled laryngocele is difficult to distinguish.

By examining 80 consecutive cases of death due to hanging, fractures of the thyroid cartilage or the hyoid bone or both, were noted in 45%. Fracture of the cricoid cartilage did not occur. No fracture occurred below the age of 25, and the frequency was slightly increasing with increasing age. There was a preponderance of fractures in males. The highest incidence of fractures was noted in typical hangings, while incomplete hanging to a very great extent was combined with congestion to the face, especially if the location of the ligature was atypical. Typical hanging in general did not produce c…

Radiodiagnostically relevant normal values and variations for measurements of the cervical region, the arithmetical average and the standard deviation were determined from adequate computer tomograms on 60 healthy women and men, aged 20 to 83 years. The sagittal diameter of the prevertebral soft tissue and the lumina of the upper respiratory tract were evaluated at exactly defined levels between the hyoid bone and the incisura jugularis stuni. The thickness of the aryepiglottic folds, the maximal sagittal and transverse diameters of the thyroid gland and the calibre of the great cervical vesse…

OBJECTIVE: The purpose of this work was to evaluate orofacial airway dimensions and the position of the hyoid bone related to changes induced by facemask therapy (combined with rapid palatal expansion) or by chincup therapy as compared to an untreated Class III control group.
METHODS: In all, 67 patients with skeletal Class III malocclusion were divided into a facemask group (n = 25; 15 girls and 10 boys), a chincup group (n = 15; 16 girls and 9 boys), and a control group (n=17; 9 girls and 8 boys). Hard- and soft-tissue parameters were analyzed on lateral cephalograms, which were available f…

BACKGROUND: A clicking noise in the larynx can probably be provoked in many adults. However, these clicks are not usually associated with pa … on of the structures associated with the clicking noise by thyroplastic surgery or reduction of the hyoid bone greater horn. It is therefore important to inform patients with mild CLS that these clicking sounds are harmless and to advise CLS patients suffering from pain of the appropriate treatment options.
