PTH mRNA transcription analysis in infantile tumors associated with hypercalcemia.

The ability of infantile hypercalcemic tumors (three rhabdoid renal tumors, one cellular mesoblastic nephroma, and one hepatoblastoma) to produce parathyroid hormone (PTH) was tested using RNA-DNA hybridization. Results were compared with those obtained in one lung epidermoid carcinoma and one parathyroid adenoma from adult patients. Elevated plasma immunoreactive PTH (iPTH) concentrations were observed in three of five children. The only tumor in which PTH-RNA hybridization could be detected was the parathyroid adenoma. The integrity of the RNA preparations was further confirmed by positive h

Nonmetastatic bladder cancer associated with hypercalcemia, thrombocytosis and leukemoid reaction.

We reviewed 4 patients with urothelial bladder cancer and hypercalcemia but without evidence of bony metastasis. Of the patients 2 presented with a leukemoid reaction (1 also had thrombocytosis). None of the patients had evidence of bone metastases or other causes of hypercalcemia, such as hyperparathyroidism, sarcoidosis or vitamin D intoxication. All 4 patients received aggressive therapy for the tumors. In each instance the serum calcium returned to normal following radiation therapy or tumor removal, lending support to the theory of humoral hypercalcemia of cancer. A diagnostic and therape…

Spontaneous hypercalcemia in patients undergoing dialysis. Etiologic and therapeutic considerations.

Ten dialysis-treated patients with hypercalcemia (11.5 +/- 0.3 mg/dl, mean +/- SE) due to renal osteodystrophy were compared with 30 control dialysis-treated patients who were not hypercalcemic (9.5 +/- 0.1 mg/dl). The hypercalcemic patients were more disabled than the control patients. Fifty percent of the hypercalcemic patients and 37 percent of the control patients had a mineralization defect (p greater than 0.6). In the control group, intact parathyroid hormone level was significantly higher in patients with osteitis fibrosa than in those with osteomalacia (247 +/- 39 pg/ml versus 60 +/- 2…

Reversible diminished calcitonin secretion in the rat during chronic hypercalcemia.

A transient increase in serum calcitonin (CT) concentration is induced by an acute iv calcium (Ca) load, whereas in the chronic hypercalcemic state, serum CT levels, as well as CT content of the thyroid, are equivocal. The secretion of CT was tested in three models of hypercalcemia in rats: tumor induced by the hypercalcemic Walker carcinosarcoma 256 (HWCS 256), chronic parenteral Ca administration, and chronic 1,25(OH)2D3 administration. In rats with HWCS 256, serum CT levels increased from basal values of 0.21 +/- 0.11 ng/ml to a maximum of 0.42 +/- 0.20 ng/ml on day 4 after tumor transplant…

Delayed hypercalcemia with acute renal failure associated with nontraumatic rhabdomyolysis.

Delayed hypercalcemia developed in two patients with acute renal failure and rhabdomyolysis. In patient 1, the hypercalcemia appeared 14 days after the beginning of the diuretic phase of the illness and was associated with severe diastolic hypertension and soft-tissue calcification, including the lungs and skeletal muscles. The blood levels of 25-hydroxyvitamin D (25OHD3) were elevated during the hypercalcemia and decreased to normal when the patient became normocalcemic. In patient 2, the hypercalcemia occurred 55 days after the start of the diuretic phase and at a time when renal function wa…

Hypercalcemia in poorly differentiated endometrial cancer: a report of two cases.

BACKGROUND: Hypercalcemia as a paraneoplastic syndrome is associated with poor prognosis. It is most commonly caused by parathyroid hormone-related protein (PTHrP) production, less commonly by bony metastases, and very rarely by ectopic parathyroid hormone (PTH) secretion. Hypercalcemia in endometrial cancer is relatively rare. CASE PRESENTATION: We present two cases of hypercalcemia in patients with poorly differentiated endometrial cancer, both of whom had elevated PTH levels and no evidence of parathyroid disease or bone metastases at baseline. In the first case, hypercalcemia rapidly prog…

From hypercalcemia to the diagnosis of pseudohypoparathyroidism type 1b: a case report.

Pseudohypoparathyroidism type 1b (PHP-1b) is a rare genetic disorder caused by mutations or epigenetic alterations of the maternal GNAS gene … a case of a girl with an unusual course of PHP: from non-autoimmune hypothyroidism coexisting with hypercalcemia at the age of 1 year to the diagnosis of PHP-1b at the age of 3.5 years, before the onset of symptomatic hypoparathyroidism. In the absence of a clear cause for hypothyroidism, assessment of calcium and phosphate metabolism should be considered. Ini …

Vitamin D-Mediated Hypercalcemia in a Dog With Putative Cutaneous Pythiosis.

A 12-month-old dog was diagnosed with cutaneous pythiosis after presenting with a nonhealing metatarsal wound, an inguinal mass, and regional lymphadenopathy. Both cytology of the abnormal lymph nodes and histopathology of the inguinal mass showed granulomatous inflammation. ELISA serology was positive for Pythium insidiosum. The dog developed hypercalcemia secondary to excessive vitamin D production. This conclusion was based on the combination of hyperphosphatemia, suppressed parathyroid hormone, suppressed 25-hydroxyvitamin D level, and a calcitriol level at the high end of the reference in…

Prognosis and Phenotypes of Advanced Head and Neck Carcinoma Associated With Hypercalcemia.

PURPOSE: Hypercalcemia is the most common metabolic disorder in cancer, affecting 10%-20% of patients with advanced malignancies, including squamous cell carcinoma of the head and neck (HNSCC), though its prognostic significance remains poorly studied. This study aimed to evaluate the prognostic impact of hypercalcemia at diagnosis in patients with locally advanced or metastatic HNSCC and to explore underlying mechanisms and treatment options. METHODS: We conducted a bicentric, retrospective analysis of patients diagnosed between 2015 and 2021, including those with locally advanced or metasta…

Endocrine Shades of Silicone Fillers: A Case of Calcitriol-mediated Hypercalcemia.

BACKGROUND: The inflammatory reaction of foreign body granulomas (FBG) may be so vast that it leads to severe systemic effects. CASE REPORT: A 42-year-old woman was referred to the ED with severe recurrent symptomatic hypercalcemia associated with worsening kidney function. She had presented multiple times with similar complaints. Severe hypercalcemia (13.8 mg/dL) was noted, with appropriately low PTH, elevated PTHrP, low 25-hydroxyvitamin D, and normal 1,25-dihydroxyvitamin D levels. She admitted having significant subcutaneous silicone filler injections in the hips six years prior. Admissio…

A Case of Hypercalcemia and Acute Kidney Injury Caused by Adrenal Insufficiency.

Hypercalcemia is a common electrolyte disturbance, most frequently caused by hyperparathyroidism or malignancy, though it can also arise from adrenal insufficiency, creating diagnostic challenges. We present a case of a 78-year-old male patient with stage 3b chronic kidney disease due to immunoglobulin A nephropathy who exhibited altered mental status following dehydration caused by a five-day episode of diarrhea. The patient presented with hypercalcemia and acute kidney injury. His history of adrenal insufficiency had been managed with glucocorticoid replacement. Dehydration and inability to …

Denosumab treatment for refractory hypercalcemia in a hemodialysis patient with tertiary hyperparathyroidism.

The most appropriate surgical procedure for tertiary hyperparathyroidism is still controversial. Medical management may be considered in those patients with failed previous surgical intervention. There are limited medical options for tertiary hyperparathyroidism with renal dysfunction. The monoclonal antibody denosumab has been used in patients with osteoporosis and hypercalcemia of malignancy. We report a case of medically refractory hypercalcemia caused by tertiary hyperparathyroidism treated with denosumab. A 46-year-old female was on hemodialysis for 10 years. She was diagnosed with tertia…

Lithium-Induced Hypercalcemia Presenting as Catatonia in a Patient With Schizoaffective Disorder.

Catatonia, a relatively rare and less explored consequence of lithium-induced hypercalcemia, represents a notable yet understudied side effect. Lithium is utilized in the management of acute mania and as a maintenance therapy for bipolar disorder. However, the specific catatonic presentation resulting from hypercalcemia remains poorly understood. Here, we present a case study involving a 55-year-old male with a history of schizoaffective disorder, bipolar type, who had been receiving lithium therapy. The patient presented with catatonia and altered mental status. Manifesting as mutism, rigidit…

[Milk-alkali syndrome: a careful history is crucial in diagnosing hypercalcemia].

BACKGROUND: Milk-alkali syndrome is a rare cause of hypercalcemia in the Netherlands, due to ingestion of large amounts of calcium and absorbable alkali. CASE DESCRIPTION: A 38-year-old female patient was admitted with severe stomach pain, vomiting and weight loss. Laboratory results showed hypercalcemia and acute kidney injury. We initially suspected that the hypercalcemia was related to primary hyperparathyroidism, a malignancy with bone metastasis or a granulomatous disease. Gastroduodenoscopy, however, revealed a duodenal ulcer, which turned out to be Helicobacter pylori-related. A thorou…
