We report the case of a 64-year old woman with severe attacks of allergic alveolitis occurring frequently independent of a concrete place. These attacks were regularely preceded by diarrhoe. Laboratory data showed marked leukopenia and circulating Candida albicans (C.a.)-antigen. Bronchoalveolar lavage revealed an increase in neutrophils (13%). Transbronchial biopsies showed focal alveolitis and focal septal fibrosis with bronchiolitis obliterans and an epitheloid cell granuloma. We describe five patients with hypersensitivity pneumonitis (HP) that was related to using home ultrasonic humidifiers. All patients had micronodular infiltrates on their chest radiograph, and their lung biopsy specimens revealed alveolitis with or without epithelioid cell granulomas. Severe anaphylactic reaction secondary to latex allergy has lately been recognized and reported especially in individuals with spina bifida. We report a case of severe intraoperative anaphylactic reaction due to latex allergy. Preoperative testing for latex allergy may be helpful in determining latex allergy. We suggest a preoperative management protocol for patients who are thought to have latex allergy. An increased awareness to latex allergy will help avoid this potentially catastrophic event.

Skin testing is useful in nutritional assessment because decreased immunity is associated with malnutrition. Absence of positive reactions indicates anergy, reaction to one test shows some decreased immunity, and response to two or more tests is considered normal. Variation in number and type of test antigens could lead to contradictory results. Also, conventional intradermal injections vary in antigen strengths, injection techniques, and subjective evaluations, and are also uncomfortable. This study compared results from three, four, or seven antigens in relation to other nutritional indices.

After a pilot study had suggested that a drug augmented delayed hypersensitivity, we conducted a control study in which 19 normal volunteers had the same skin test battery applied as in the pilot study, on two occasions one month apart with no intervening treatment. A 10-year-old black girl had an episode of diphenylhydantoin(DPH)-induced exfoliative dermatitis, lymphadenopathy, hepatitis, peripheral eosinophilia, and transient renal failure. The findings of specific lymphocyte sensitization of DPH, a clinically typical delayed hypersensitivity reaction, multinucleated histiocytes in the renal interstitium, and negative renal immunofluorescence studies for immune reactants indicate that the child's renal injury was at least partially cell-mediated.

Delayed hypersensitivity skin testing is based on the reaction that occurs in response to the intradermal injection of an antigen. The technique, interpretation, and pitfalls of interpretation of delayed hypersensitivity skin testing are presented. The histologic findings and immunologic mechanisms characterizing this form of immunologic response are discussed. The diagnostic and clinical importance of reactivity to recall skin test batteries, common microbial antigens, dinitrochlorobenzene, and tumor-associated antigens is also discussed.
