A 70-year-old man was referred for evaluation of a 2-week history of numbness and progressive weakness in his lower and upper extremities and subsequently diagnosed with Guillain-Barré syndrome. The patient had been taking mycophenolate mofetil 500 mg twice daily and tacrolimus 6 mg daily for immunosuppression following a kidney transplant 2 years earlier. However, 5 weeks prior to presentation he had been diagnosed with pneumonia due to coccidioidomycosis and his tacrolimus dose was reduced to 1 mg daily to prevent a drug interaction with fluconazole, which was prescribed to treat the coccidi

Cryptosporidiosis, the disease caused by protozoa of the genus Cryptosporidium, represents a major public health problem in both developing and developed countries. The infection can be severe and life threatening among immunocompromised individuals, particularly in AIDS patients and in those with primary immunodeficiency diseases. This review examines the essential aspects of the epidemiology of the infection, highlighting the role of animals, water and food. A critical evaluation of the diagnostic tools used in clinical settings is also provided. Lastly, the review examines the chemotherapeu

Salmonella enterica serovar Typhi (S. typhi) causes human typhoid fever, a serious and widespread disease in developing countries. Other Salmonella serovars are associated with food-borne infections. The recent emergence of multi-drug-resistant Salmonella strains highlights the need for better preventive measures, including vaccination. The available vaccines against Salmonella infection do not confer optimal protection. The design of new Salmonella vaccines must be based on the identification of suitable virulence genes and on knowledge of the immunological mechanisms of resistance to the dis

The standard treatment for scabies has long been based on topical scabicides. Ivermectin, a broad-spectrum anti-parasitic agent, was first used in veterinary medicine. This drug, active against nematodes and arthropods, is used in humans for the treatment of filariasis. For the past few years worldwide reports on the use of ivermectin in human scabies have suggested that this drug could be an alternative oral therapy.

The authors report a male patient, a seller with no detected immunosuppression, with an extensive ulcerated skin lesion localized on the left forearm, caused by Cryptococcus neoformans var. gattii serotype B. Oral treatment with fluconazole was successful. A review of the literature showed the rarity of this localization in HIV-negative patients. In contrast, skin lesions frequently occurs in HIV-positive patients, with Cryptococcus neoformans var. neoformans serotype A predominating as the etiological agent. In this paper, the pathogenicity of C. neoformans to skin lesions in patients immunoc

Eradication of polio is in sight. Once this is finally achieved there is general agreement that the world should move from the current policy of universal childhood immunisation, mostly with the oral poliovirus vaccine (OPV), to no immunisation. The challenge ahead is to build consensus on how and when this is achieved. The major area of scientific uncertainty is whether OPV-derived strains excreted by vaccinees will establish chains of transmission in communities, especially in settings of poor hygiene, when population immunity levels begin to fall after immunisation ceases. Evidence from sev

Despite appropriate antimicrobial and antifungal therapy (amphotericin B), a disseminated infection with Fusarium oxysporum in a 75-year-old immunocompromised patient (acute myeloid leukaemia, minimal leucocyte count of 0.5 giga l-1) led rapidly to death. A similarly fatal course of an F. oxysporum infection has been reported in several cases. Fusarium oxysporum could be isolated shortly before death from blood cultures and from a swab taken from skin efflorescences. An autopsy revealed histopathologically typical fungal infiltrates in the mucosa of the pharynx, epiglottis, trachea, and oesoph

OBJECTIVE: To provide a comprehensive, literature-based guide to the diagnosis of and the management and referral of patients with oral diseases associated with human immunodeficiency virus (HIV) infection.
DATA SOURCES: The literature was reviewed to identify common oral manifestations of HIV infection, the physical and laboratory findings associated with them, and the therapies used to treat them. The National Library of Medicine's AIDSLINE database was searched using the term "mouth diseases." Additional references were identified from the bibliographies of these articles and related textb

We reported a case of non-0:1 group Vibrio cholerae septicemia with myelodysplatic syndrome in Taiwan. We also reviewed the other 22 reported cases of non-0:1 Vibrion cholerae septicemia found in the literature regarding its pathogenesis and treatment. The case mortality rate of these 23 cases was 47.8%. Most of them had immunocompromised diseases, particularly liver cirrhosis and hematologic malignancy. Therefore, the most important factor is the host defense. The cholera-like enterotoxin and E1-Tor-like hemolysin also play a major role, but whether the gall bladder plays a role in organ grow

Capnocytophaga, a CO2-requiring gram-negative bacillus, is a recently recognized pathogen in the immunocompromised host. The authors present two cases with granulocytopenia, oral disease and bacteremia with Capnocytophage ochracea. The microbiology of this genus, its clinical association with periodontal disease, granulocytopenia and oral ulceration, and its implications for the compromised host are reviewed.
