An incarcerated inguinal hernia is a textbook example of a basic and straightforward diagnosis. In rare cases, an incarcerated hernia may be a symptom of more complex underlying pathology. In this case report a patient with a ruptured abdominal aortic aneurysm presented with an incarcerated left inguinal hernia. Only two other cases have been reported with a stable patient at initial presentation. Femoral hernias are not common in the adult population however they are often associated with higher rates of incarceration compared to other hernia subtypes. Subsequently they have an increased need for emergency surgical intervention. It has been well documented in the literature that femoral hernia sacs can contain an array of anatomical structures and rarely this includes fallopian tube. Within the limits of the authors' literature review, this case represents the only documented pre-operative diagnosis of femoral hernia containing fallopian tube.

Femoral hernias account for only about 0.7% of all operated hernias and are rather infrequent among male patients. By contrast, acute appendicitis is one of the most frequent diagnoses in surgery requiring urgent operation. The authors present one of the rare cases in which both these diagnoses occur together - De Garengeots hernia. Incarcerated femoral hernia with acute appendicitis is diagnosed preoperatively only sporadically. Imaging methods can help in the diagnosis, but often the final diagnosis, and especially the final solution, is brought about only by surgical intervention.

Children often place nonedible objects into their mouths. Occasionally, these objects are inadvertently swallowed, and fortunately, the majority passes spontaneously without intervention. We present the case of a 10-month-old girl who presented with an incarcerated Richter hernia through an indirect inguinal defect containing a wall of sigmoid colon perforated by a swallowed lollipop stick. Although this is a rare case, we conclude that prompt laparoscopic abdominal exploration is an extremely valuable tool in making an accurate diagnosis in difficult-to-reduce or incarcerated hernia cases.

Hernias containing incarcerated Meckel's diverticulum are rare and often asymptomatic. The proper preoperative diagnosis is difficult to establish. The presence of a Meckel's diverticulum incarcerated in a hernia should be consider in a differential diagnosis of abdominal disease that is not sufficiently apparent.
