Refractory pyoderma gangrenosum associated with ulcerative colitis successfully treated with infliximab.

Pyoderma gangrenosum (PG) is a rare, immune-mediated ulcerating skin disease. In up to 50 percent of the cases, PG is associated with underlying systemic disorders, most commonly inflammatory bowel diseases, connective tissue diseases, or hematological disorders. Herein, we present a case of refractory PG associated with ulcerative colitis (UC), successfully treated with infliximab.

Autoimmune hepatitis during infliximab therapy for Crohn's disease: a case report.

We present the case of a 60-year-old Caucasian male with Crohn's disease treated with infliximab. Within 14 weeks of treatment induction, an asymptomatic acute hepatitis was detected. Elevated autoantibodies and liver biopsy findings supported the diagnosis of autoimmune hepatitis. No competing aetiologies were present. The hepatitis completely responded to infliximab cessation and administration of corticosteroids. This case is the most compelling to date of an infliximab-induced autoimmune hepatitis. Although the role of liver enzyme monitoring is unclear, an awareness of this adverse effect…

New-onset polyarthritis during successful treatment of hidradenitis suppurativa with infliximab.

BACKGROUND: Hidradenitis suppurativa (HS) can be associated with several forms of arthritis, usually considered as reactive arthritis. A new observation is that some patients with HS develop arthritis after treatment with infliximab (antitumour necrosis factor-α). OBJECTIVES: A retrospective study was performed to establish the frequency and clinical presentation of new-onset arthritis during infliximab treatment. METHODS: Between 2005 and 2009, 27 individuals with severe HS were treated with infliximab and followed up closely. Laboratory parameters and side-effects were recorded. The freque…

Intralesional infliximab in noninfectious cutaneous granulomas: three cases of necrobiosis lipoidica.

BACKGROUND: Necrobiosis lipoidica is a rare granulomatous noninfectious skin disease. Treatment of this chronic debilitating disease is of importance because ulceration of the plaques may induce important psychological and physical morbidity. OBJECTIVE: Infliximab, an anti-TNF-α chimeric monoclonal antibody used intravenously and intralesionally for other extradermatological granulomatous diseases including Crohn's disease and sarcoidosis, was administered by intradermal injection in necrobiosis lipoidica. The aim of this study was to evaluate the efficacy and safety profile of a locally deli…

Cystoid Macular Edema: Possible Complication of Infliximab Therapy in Behçet's Disease.

AIM: Infliximab, an anti-tumor necrosis factor (TNF)-alpha monoclonal antibody, has been reported to be effective in refractory uveoretinitis in Behçet's disease. Because it has been used clinically for a short time, information on its adverse effects is limited. We report a patient who developed cystoid macular edema (CME) following infliximab use for uveoretinitis associated with Behçet's disease. Case Report: A 27-year-old man had refractory uveoretinitis and neuro-Behçet's disease, and intravenous infliximab was administered. RESULTS: One day after infliximab infusion, the patient complain…

[Crohn's disease complicating psoas abscess treated by drainage and infliximab administration].

A 19-year old man was admitted for treatment of a right psoas abscess. He was first diagnosed as Crohn's disease with ileocolitis and fistula, which caused the abscess. Following the drainage of the abscess and conservative treatment including administration of antibiotics, total parenteral nutrition and medication, his symptoms were temporarily improved. After recurrence, additional therapy with infliximab successfully induced remission. He has remained free from abdominal symptoms and recurrence of the abscess. It seems that conservative treatment including infliximab administration is usefu…

Dramatic efficacy of infliximab in cauda equina syndrome complicating ankylosing spondylitis.

Cauda equina syndrome is an uncommon complication of ankylosing spondylitis (AS) characterized by the slow and insidious development of severe neurologic impairment. Imaging studies usually show a wide lumbar canal with dural ectasia. No medical or surgical treatment has been proven effective. We managed the care of a 66-year-old man who had longstanding AS and clinical features of cauda equina syndrome, including anal incontinence and buttock hypoesthesia. Magnetic resonance imaging demonstrated no cause for these symptoms other than AS. The patient was treated with infliximab, a monoclonal a…

Serious adverse events with infliximab: analysis of spontaneously reported adverse events.

BACKGROUND & AIMS: Serious adverse events such as bowel obstruction, heart failure, infection, lymphoma, and neuropathy have been reported with infliximab. The aims of this study were to explore adverse event signals with infliximab by using a long period of post-marketing experience, stratifying by indication. METHODS: The relative reporting of infliximab adverse events to the U.S. Food and Drug Administration (FDA) was assessed with the public release version of the adverse event reporting system (AERS) database from 1968 to third quarter 2005. On the basis of a systematic review of adverse…

Fistulating anal Crohn's disease: results of combined surgical and infliximab treatment.

INTRODUCTION: Infliximab is a monoclonal antibody against tumor necrosis factor-alpha, which has been shown to be effective in fistulating Crohn's disease. The safety of infliximab in patients with potential perianal sepsis is uncertain. This study was designed to assess the safety and outcome of infliximab therapy combined with surgery for patients with fistulating anal Crohn's disease. METHODS: All patients receiving infliximab for fistulating anal Crohn's disease between 2000 and 2004 were studied. Patients' demographics, clinical findings, magnetic resonance imaging, and examination under…

Chimeric monoclonal antibody to tumor necrosis factor alpha (infliximab) in psoriasis.

BACKGROUND: Insights into the pathogenesis of psoriasis have provided opportunities to target key steps in the disease process. Tumor necrosis factor-alpha (TNF- alpha) being crucial to the pathogenesis of psoriasis, monoclonal antibodies against this cytokine have proved useful in its treatment. AIM: To study the efficacy of chimeric monoclonal antibody to TNF- alpha (infliximab) in Indian patients with recalcitrant psoriasis vulgaris. MATERIALS AND METHODS: Three patients with recalcitrant psoriasis vulgaris were studied. Baseline haemogram, biochemical parameters, chest radiograph and Man…

HLA-B27 associated spondyloarthropathy, vasculitis, and amyloid enteropathy: response to infliximab.

In 2000 we described a patient with HLA-B27 associated spondyloarthropathy (SpA) and severe ascending aortitis requiring surgical intervention. Despite continued immunosuppressive therapy she developed narrowing of the distal part of the right subclavian artery and proximal axillary artery secondary to active vasculitis. In addition, biopsy-proven amyloid gastroenteropathy developed causing persistent diarrhea and iron deficiency anemia. Treatment with infliximab resulted in resolution of joint symptoms and rapid improvement in laboratory markers of inflammation. Diarrhea settled more graduall…
