Economic benefits of T cell-depleting induction in deceased donor kidney transplant recipients.

Thymoglobulin (TMG) induction improves long-term outcomes in high-immunologic-risk kidney transplant (KTx) by reducing acute cellular rejection. However, routine use of TMG in lower-immunologic-risk KTx is controversial, given concerns about posttransplant infectious complications and added health care expenditures. United States Renal Data System data were examined to compare real-world clinical and economic outcomes-including 3-year median Medicare expenditures, incidence of key adverse events, and allograft survival-in a cohort of Medicare-insured deceased donor KTx recipients (2006-2020) t

Characteristics of Adults With Hepatitis C Virus: Evidence From the National Health and Nutrition Examination Survey 2011-2012.

Hepatitis C virus (HCV) infects more than 3 million people in the United States (U.S.). Long-term complications of hepatitis C infection result in increased liver disease and financial burden for the nation. The purpose of this study was to identify characteristics of adults with HCV in the U.S. This secondary, descriptive study analyzed data from the 2011-2012 National Health and Nutrition Examination Survey. The weighted sample included 2,075,749 adults diagnosed with HCV. Descriptive statistics were calculated. The findings revealed that most adults in the U.S. with HCV were insured non-His

Adherence to Pediatric Ophthalmology Follow-Up After Completion of Retinopathy of Prematurity: A Retrospective Review.

PURPOSE: Retinopathy of prematurity is a leading cause of preventable childhood vision loss, and infants remain at risk of long-term ocular complications even after ROP screening concludes. However, adherence to recommended pediatric ophthalmology follow-up after ROP screening completion is not well characterized. This study aimed to evaluate adherence to pediatric ophthalmology follow-up visits after completion of ROP screening and identify factors associated with loss to follow-up in a tertiary care setting. PATIENTS AND METHODS: We performed a retrospective chart review of premature infant

The impact of sociodemographic factors on the utilisation of support services for family caregivers of elderly dependents - results from the German sample of the EUROFAMCARE study.

OBJECTIVES: As in nearly all European countries, demographic developments in Germany have led to both a relative and an absolute increase in the country's elderly population. The care and support needed by these people is primarily provided by relatives or friends and close acquaintances within the home environment. The major challenges for society are to sustain, promote and support these informal resources. In order to achieve this, it is crucial that family caregivers are provided with situation-specific services that support them and relieve their burden of care. The major challenges for s

How to improve survival in geriatric peritoneal dialysis patients.

BACKGROUND: Recently, more elderly patients who are independent or able to live at home with the support of family are opting for continuous ambulatory peritoneal dialysis (CAPD). At the end of 2005, the annual statistical survey conducted by the Japanese Society for Dialysis Therapy indicated that the mean age of patients at initiation of dialysis treatment is 66.2 years. Only 3.6% of the overall end-stage renal disease population were treated with CAPD, and this small number of elderly patients was treated with CAPD despite the many merits of peritoneal dialysis (PD) for the elderly. In the

Group-based trajectory modeling to assess adherence to chronic urate-lowering therapies among commercially insured US adults with gout.

BACKGROUND: The benefits of urate-lowering therapies (ULTs) for the long-term management of gout are well established. However, suboptimal adherence remains a significant challenge, resulting in increased gout flares and higher health care utilization. The proportion of days covered (PDC) is commonly used to assess adherence but provides only a single value that fails to distinguish among individuals with differing and dynamic adherence patterns over time. Understanding fluctuations in adherence and their associated characteristics can inform interventions aimed at improving adherence. OBJECT

Long-Term Observation of Patients With Cancer - an Entity-Independent Registry for Healthcare and Translational Research at the University Medicine Dresden (Cancer-Reg-VT).

INTRODUCTION: Translational research is important, especially in medicine where decisions affect people's lives. Clinical registries and the studies embedded in them allow the depiction of actual care practice under routine conditions. Translating the findings of health services research back into clinical research through prospective cohort studies has the potential to drive medical innovations faster, more effectively and, above all, in a more targeted manner. These must therefore be a central component of cutting-edge oncological research. OBJECTIVE: The aim of the registry is the establis

Universal health coverage and equal access in Sweden: a century-long perspective on macro-level policy.

BACKGROUND: When today's efforts to achieve universal health coverage are mainly directed towards low-income settings, it is perhaps easy to forget that countries considered to have universal, comprehensive and high-performing health systems have also undergone this journey. In this article, the aim is to provide a century-long perspective to illustrate Sweden's long and ongoing journey towards universal health coverage and equal access to healthcare. METHODS: The focus is on macro-level policy. A document analysis is divided into three broad eras (1919-1955; 1955-1989; 1989-) and synthesises

End-of-life healthcare utilisation and costs of older patients on kidney replacement therapy - analysis of Dutch health insurance claims data.

BACKGROUND: Patients on kidney replacement therapy (KRT) face high symptom and treatment burden, especially at end of life. Yet insights into end-of-life healthcare utilisation and costs remain limited. We aimed to describe healthcare utilisation and costs during the last year of life across KRT modalities. METHODS: We used Dutch health insurance claims data to identify incident and prevalent patients aged ≥ 65 years, treated with KRT during the year preceding death, and who deceased between June 2016 and December 2021. Healthcare utilisation and costs in the last year of life were analysed f

Utilization patterns of insulin for patients with type 2 diabetes from national health insurance claims data in South Korea.

Type 2 diabetes mellitus (T2DM) is a chronic disease that requires long-term therapy and regular check-ups to prevent complications. In this study, insurance claim data from the National Health Insurance Service (NHIS) of Korea were used to investigate insulin use in T2DM patients according to the economic status of patients and their access to primary physicians, operationally defined as the frequently used medical care providers at the time of T2DM diagnosis. A total of 91,810 participants were included from the NHIS claims database for the period between 2002 and 2013. The utilization patte

Impact of Longitudinal Changes in Dynamic Lower-Limb Function on Care-Needs Certification: A 4-Year Prospective Study of Older Women Attending Community Gathering Places (Kayoi-no-ba).

Background: Japan's rapidly aging society faces a critical challenge in sustaining the public long-term care insurance (LTCI) system. Community gathering places (Kayoi-no-ba) have been promoted for preventive care, but the predictive value of longitudinal changes in physical performance measures (PPMs) within these settings is not fully understood. Objectives: Our objective was to clarify the association between baseline PPMs and their longitudinal change over four years with subsequent care-needs certification among community-dwelling older women. Methods: We conducted a 4-year prospective st

The family burden of paediatric heart disease during the chronic phase of illness.

BACKGROUND: CHD is a lifelong condition with a significant burden of disease to patients and families. With increased survival, attention has shifted to longer-term outcomes, with a focus on social determinants of health. Among children with CHD, socioeconomic status is associated with disparities in outcomes. Household material hardship is a concrete measure of poverty and may serve as an intervenable measure of socioeconomic status. METHODS: A longitudinal survey study was conducted at multiple time points (at acute hospitalisation, then 12-24 months later in the chronic phase) to determine

Disparities in low dose aspirin use for preeclampsia risk reduction and opportunities for improvement.

Low-dose aspirin (LDA) is a safe, inexpensive, and evidence-based intervention that reduces the risk of preeclampsia among pregnant individuals at elevated risk. Despite strong evidence and national guideline recommendations, implementation of LDA prophylaxis remains inconsistent, and emerging evidence suggests that disparities in LDA use may contribute to inequities in maternal health outcomes. This review summarizes the current literature on disparities in LDA use for preeclampsia prevention and explores opportunities for equitable implementation. Existing studies demonstrate disparities acr

Economic burden of vertigo: a systematic review.

BACKGROUND: Vertigo, a highly prevalent disease, imposes a rising burden on the health care system, exacerbated by the ageing of the population; and further, contributes to a wide span of indirect burden due to reduced capacity to work or need of assistance in activities of daily living. The aim of this review was to summarise the evidence on the economic burden of vertigo. METHODS: All original studies published between 2008 and 2018 about the economic evaluation of peripheral or central vestibular vertigo in developed countries were considered eligible, unrestricted to setting, health care
