Newborn intensive care and neonatal mortality in low-birth-weight infants: a population study.

We examined the neonatal mortality rates of low-birth-weight infants (501 to 2250 g) born between 1976 and 1978 in three kinds of hospitals in New York City: those with newborn-intensive-care units (Level 3), those with capabilities for the care of most premature infants (Level 2), and those without any special facilities for premature newborns (Level 1). Among 13,560 singleton low-birth-weight infants, the adjusted neonatal mortality rate for Level 3 hospitals was 128.5 per thousand live births - significantly lower (P less than 0.001) than the rates for both level 2 (168.1) and Level 1 units

Initial chest radiography in the neonatal intensive care unit: value of the lateral view.

This study was designed to assess the diagnostic efficacy of the routine lateral chest film in symptomatic newborn infants. The initial anteroposterior chest film was supplemented by alateral view in 53 consecutive patients on admission to the newborn intensive care unit. Subsequently three radiologists and two neonatologists performed independent analysis of the anterposterior films with and without the lateral view. A total of 530 observations were tabulated. Assessment of degree of lung inflation and discovery of abnormalities of the heart, mediastinum, and trachea were unaffected by the us

Necrotizing enterocolitis. Controlled study of 3 years' experience in a neonatal intensive care unit.

During the 3 years 1972-74, 17 infants were treated for necrotizing enterocolitis (NEC) in the Neonatal Unit at University College Hospital. The incidence of the illness was 0.2% of live births in the hospital and 2.7% of those referred from elsewhere. The mean birthweight of the affected infants was 1832 g (range 878-3850 g) and mean gestational age 33 weeks (range 28-40 weeks). The illness was diagnosed at a mean age of 16 days (range 3-33 days). 14 infants (82%) survived. One infant developed NEC because of a volvulus, and another because of an apparently abnormal arterial supply to a segme

Evaluation of point-of-care hemoglobin testing vs. central laboratory measurements in neonates: a prospective study in neonatal intensive care unit.

INTRODUCTION: Point-of-care (POC) testing for blood gas analysis is commonly used for neonates in the Neonatal Intensive Care Unit (NICU) to determine blood gases using small blood volumes. This helps reduce blood loss and speeds up treatment. However, many clinicians remain skeptical about the accuracy of POC hemoglobin results. Our study evaluated the agreement between POC testing and laboratory measurements of hemoglobin concentration in infants born at >28 weeks gestational age. METHODS: This prospective cohort study analyzed 187 paired blood samples from infants born after 28 weeks of ge

Molecular epidemiology of gram-negative bacilli from infected neonates and health care workers' hands in neonatal intensive care units.

We sought to characterize the molecular epidemiology of gram-negative bacilli (GNB) causing infections in infants and associated with carriage on nurses' hands after hand hygiene was performed. From March 2001 to January 2003, GNB caused 192 (34%) of 562 hospital-acquired infections in the 2 participating neonatal intensive care units (NICUs) and were isolated from the hands of 45 (38%) of 119 nurses. Five species--Klebsiella pneumoniae, Escherichia coli, Pseudomonas aeruginosa, Serratia marcescens and Enterobacter cloacae, all of which were typed by pulsed-field gel electrophoresis--caused 16

Parental viewpoints and experiences of therapeutic hypothermia in a neonatal intensive care unit implemented with Family-Centred Care.

AIMS AND OBJECTIVES: To explore parental experiences of therapeutic hypothermia (TH) in their newborn infant suffering from hypoxic ischaemic encephalopathy following perinatal asphyxia. BACKGROUND: Since more than a decade, newborn infants are treated with TH following perinatal asphyxia to reduce mortality and disabilities and to improve neurological outcome. The infants' body temperature is lowered to 33.5°C for 72 hr, and the infant is usually cared for in an open incubator. The parents are not able to hold their infant skin to skin, which risks causing emotional reactions in parents and

Effect of tactile-kinesthetic stimulation on breastfeeding performance among preterm neonates in a neonatal intensive care unit.

INTRODUCTION: Preterm birth, which means the baby is born before the 37th week of pregnancy, is very often linked to a later start in breastfeeding and to feeding problems because the baby’s neuromotor system is not yet fully coordinated. Early sensory interventions like tactile-kinesthetic stimulation (TKS) can potentially improve the feeding skills and physiological stability of preterm babies. METHODS: This research, a quasi-experimental pre-test and post-test control group design, was carried out in the Neonatal Intensive Care Unit (NICU) of a tertiary care hospital. Forty eligible neonate
