Surgical strategies in complex gastroschisis.

Currently, the most important determinant of gastroschisis outcomes in high resource settings is whether the condition is associated with intestinal complications, such as atresia, necrosis, perforation, or volvulus. This form of the anomaly, known as complex gastroschisis, accounts for most of the mortality and a disproportionate burden of the morbidity from gastroschisis. There is some disagreement about what constitutes complex gastroschisis, and little consensus on the type and timing of surgical interventions. This article establishes a clear definition of complex gastroschisis. Surgical

Partial midgut agenesis in a premature neonate: A case report and literature review of small bowel agenesis.

INTRODUCTION AND IMPORTANCE: Small bowel agenesis (SBA) is a rare congenital abnormality characterized by the absence of part or all of the small intestine, leading to significant disruptions in nutrient absorption. While hereditary factors are suspected to contribute, the precise etiology of SBA remains largely unknown. Prenatal ultrasonography is typically used for diagnosis, but postnatal diagnosis can be based on clinical signs such as vomiting, abdominal distension, and poor growth. Surgical intervention is often necessary for management, yet the long-term prognosis varies depending on th

Three faces of midgut duplication.

This is an analysis of three unusual cases of gastrointestinal duplications seen over a two year period. They were all males aged eight and 48 hours, and 4 1/2 years. Two had cystic duplications; in one of them it was associated with a partially fixed cecum resulting in a volvulus, while in the second the duplication occurred in association with duodenal atresia and jejunal deletion. The third patient had a tubular malformation involving distal jejunum, whole of the ileum, and right side of the colon as far as its transverse level. It was also associated with duodenal stenosis and rectal atres

Necrotizing enterocolitis (ischemic enteropathy) with the sequel of colonic atresia.

Two cases of acquired colonic atresia subsequent to necrotizing enterocolitis (ischemic enteropathy) in the neonatal period are reported. These infants were noted to have complete atresia of the colon in the region of the splenic flexure 8 and 11 weeks after small bowel resection and ileostomy formation. Recognition of this and other late sequelae of ischemic enterocolitis becomes critical to the successful outcome of these patients as survival rates increase.

Does duodenal atresia and stenosis prevent midgut volvulus in malrotation?

Thirty-six of the 64 (56%) neonates with duodenal atresia and stenosis who were retrospectively reviewed had associated malrotation. A volvulus neonatorum (age: <28 days) was not encountered among these patients. Can the duodenal atresia and stenosis act as a flood gate mechanism, in the prevention of volvulus in these children?

[Green amniotic fluid as initial symptom of high intestinal obstruction in infants].

At the birth of two children the amniotic fluid was green colored. The Apgar scores were good. Because of bilious vomiting and food retention, respectively, an open stomach tube was inserted, out of which bilious stomach contains were drained. The cause of green amniotic fluid was not meconium production or infection with Listeria monocytogenes, but mixing with green bile. At further investigation the children both proved to have a high intestinal obstruction distal of the papilla duodeni major.

Jejunal atresia secondary to intrauterine intussusception, presenting as acute perforation.

Intussusception in neonates is rare. The authors report the second case in the Western literature of neonatal intussusception presenting as pneumoperitoneum. A male infant was referred 30 hours after birth because of possible midgut volvulus after emesis and failure to pass meconium. An abdominal x-ray film revealed a large amount of free air in the peritoneal cavity. At operation a type 3 jejunal atresia was found, with a 2-mm perforation in the dilated proximal end and a viable intussusceptum just inside the distal end. The authors conclude that this case represents an atresia secondary to i

Ultrasound diagnosis and management of fetal intestinal obstruction and volvulus in utero.

A case has been described in which the diagnosis of acute intestinal obstruction of the fetal small bowel was made antenatally with the aid of ultrasonography. Cesarean section was done in the 34th week of pregnancy. One hour after birth ileostomy and colostomy were performed after resection of a volvulus in the terminal ileum. Two months later the ileostomy and colostomy were closed, and the baby is doing well. The importance of precise diagnosis of the anomaly in utero is emphasized to avoid unjustified termination of pregnancy.

The Surgical Correction of Congenital Deformities: The Treatment of Diaphragmatic Hernia, Esophageal Atresia and Small Bowel Atresia.

BACKGROUND: More than half of all congenital deformities can be detected in utero. The initial surgical correction is of paramount importance for the achievement of good long-term results with low surgical morbidity and mortality. METHODS: Selective literature review and expert opinion. RESULTS: Congenital deformities are rare, and no controlled trials have been performed to determine their optimal treatment. In this article, we present the prenatal assessment, treatment, and long-term results of selected types of congenital deformity. Congenital diaphragmatic hernia (CDH) affects one in 350

An Unusual Case of Hypoproteinemia in Childhood: Keep in Mind Trichobezoar.

Protein-losing enteropathy (PLE) is a rare condition characterized by protein loss through the gastrointestinal tract, leading to hypo-proteinemia. Patients may be asymptomatic or present with variety of complications of hypoproteinemia (e.g., oedema, ascites, pleural, and cardial effusions). We describe a case report of a young girl suffering from behavioral disorder since childhood who presented with generalized oedema, hypoproteinaemia, and microcytic hypochromic anemia. In addition, the girl had an intervention for jejunal atresia and intestinal malrotation in her past medical history. Upp

Accuracy of radiographic estimation of small bowel dimensions in pediatric patients with short bowel syndrome.

BACKGROUND: The decision to perform autologous intestinal lengthening in patients with short bowel syndrome (SBS) depends on total bowel length and the diameter and length of dilated segments. This study evaluated the accuracy of radiologic measurements of intestinal length and diameter. METHODS: Patients who underwent an intestinal lengthening procedure with preoperative upper gastrointestinal study (UGI) were identified from 10/2012 through 1/2015. Measurements of total length and diameters and lengths of dilated segments on UGI were compared to intraoperative measurements using Spearman's

Neonatal bowel obstruction.

Newborn intestinal obstructions are a common reason for admission to neonatal ICUs. The incidence is estimated to be approximately 1 in 2000 live births. There are 4 cardinal signs of intestinal obstruction in newborns: (1) maternal polyhydramnios, (2) bilious emesis, (3) failure to pass meconium in the first day of life, and (4) abdominal distention. The presentation may vary from subtle and easily overlooked findings on physical examination to massive abdominal distention with respiratory distress and cardiovascular collapse. A careful history and physical examination often identify the diag

Neonatal ultrasonography to distinguish between meconium ileus and ileal atresia.

Meconium ileus can be difficult to distinguish from ileal atresia on plain radiographs and on contrast enema. Both show a microcolon in the face of a small bowel obstruction. The treatment of the two is very different. Meconium ileus obstruction may be relieved medically by contrast enema; ileal atresia requires prompt surgical intervention. This study was made to determine if abdominal ultrasonography might be helpful in distinguishing between these two entities. Abdominal ultrasonograms from the past 10 years of all patients with these two diseases who were studied with preoperative ultrason

[Abnormalities in positioning and shape of the intestines in children].

Volvulus, ileus, and internal herniation may be consequences of intestinal malposition in childhood. Bacterial overgrowth, obstipation, diarrhoea, and malabsorption may additionally develop over time in the absence of adequate treatment. One in three positional abnormalities is diagnosed only intraoperatively. The surgeon has to expect such surprise findings as a condition for adequate therapeutic action.

Calcified meconium in the newborn.

Six neonates are described in whom plain films of the abdomen demonstrated scattered flecks of calcification. Three of the patients had rectal atresia with recto-urethral fistula, one rectal atresia without fistula, one jejunal atresia and one a severe stenosis of the distal small bowel associated with log segment aganglionosis. At operation the calcification was found to be in meconium within the bowel lumen and not as expected to represent serosal plaques following meconium peritonitis due to an intrauterine bowel perforation. Previous case reports are reviewed and possible mechanism discuss
