Acute bladder and small bowel perforation as a complication of Foley catheterization.

The complications of chronic urethral catheterization are well documented in the medical published data. Chronic urethral catheterization has been demonstrated to cause inflammatory changes within the bladder wall, promoting perforation and small bowel fistualization. In this report, we document the clinical course of a patient who presented with acute perforation of the bladder and small bowel as a result of Foley catheterization. To our knowledge, this is the first documented acute perforation of the bladder and the small bowel during Foley catheterization.

Choledochal-duodenal fistula presenting as an upper GI bleed.

Gallstones are common and largely asymptomatic, but can result in significant morbidity in a small proportion of patients. Choledochal-enteric fistulation is one such complication with an associated mortality of 15-18%. The authors present a case of an 88-year-old man admitted to the general medical ward with an acute upper gastrointestinal bleed. Oesophagogastroduodenoscopy found a large gallstone impacted in the pylorus and CT scan revealed a choledochal-duodenal fistula. At laparotomy it was found that a 6.2 cm gallstone had fistulated into D1 and the pylorus and impacted there, causing out

An unusual enterovesical fistula.

A 52-year-old woman, who had received radiation therapy for cervical cancer, presented with sepsis as the result of an enterovesical fistula. A loop of necrotic small bowel was retrieved through the urethra on removal of the urethral catheter.

Gallstone ileus. Review of 112 patients in the Japanese literature.

A survey was made of 112 cases of gallstone ileus reported in the Japanese literature, including 3 of our cases. The ratio of males to females was 1 to 1.1, the age range was 13 to 87 years, and biliary-enteric fistulas were demonstrated in 81 percent of the patients. Although cholecystoduodenal fistula was most common, eight cases of choledochoduodenal fistula were reported. Sixteen patients passed obstructing gallstones through a patulous sphincter of Oddi. The ileum was occluded in 54 percent of the cases. Duodenal obstruction was seen in 10 percent. There was no operative mortality in seve

Benign gastric ulceration as a cause of gastro-colic fistula.

Gastro-colic fistulas as a complication of benign gastric ulcers are relatively uncommon. The authors report a 59 year old caucasian female with a known hiatus hernia and gastro-oesophageal reflux disease presenting with diarrhoea and weight loss. Barium enema is the investigation of choice although it is important to exclude malignancy. Management is by en-bloc resection of the fistula with primary gastro-intestinal reconstruction.

Case Report: Innovative surgical management of ileal neobladder fistula after radical cystectomy.

An ileal neobladder fistula is a rare but serious complication of radical cystectomy with orthotopic neobladder reconstruction. Owing to its low incidence, the challenges posed by dense adhesions, and the risk to urinary function, there is little consensus on optimal management. A 68-year-old male with low-grade non-muscle-invasive bladder cancer underwent transurethral resection, followed by radical cystectomy and total laparoscopic orthotopic neobladder reconstruction. One month after surgery, the patient developed fecaluria. Imaging revealed a fistula between the neobladder and the ileum. P

Spontaneous gastrojejunal fistula is a complication of gastric ulcer.

Spontaneous gastrojejunal fistula formation is an extremely rare complication of gastric ulcer disease. We report a 77-year old woman who presented with diffuse abdominal pain, weight loss, malaise, nausea, and occasional dark stools. Laboratory tests showed extreme hyposideremic anemia with inflammatory syndrome. In addition, biochemical parameters of malnourishment were presented. Upper endoscopy revealed the patent esophagus along the full length without any pathological changes. Large and deep ulceration with perforation in the small intestine was detected in the posterior gastric wall. Th

Fistula complicating restorative proctocolectomy.

Twenty-seven patients developed a fistula after 168 restorative proctocolectomies. Thirteen fistulas were enterocutaneous (two with communication to the bladder); their origins were from the pouch (three patients), the ileoanal anastomosis (three), the pouch appendage (three), a previous loop ileostomy (two) and iatrogenic small bowel injury (two). Two patients had Crohn's disease. The pouch was removed in four patients, one of whom died from chronic small bowel obstruction; the remaining nine have satisfactory pouch function after fistula excision. Ten pouch-vaginal fistulas occurred, all fro
