Liver and intestinal flukes.

A number of trematodes besides schistosomes parasitize humans and domesticated animals. Although they do not have as great a public health impact as schistosomiasis, they are prevalent in Southeast Asia as well as among the greater than 1 million immigrants from this region to North America. The human biliary flukes include C. sinensis, O. viverrini, and O. felineus. These chronic infections are often asymptomatic but over time may cause biliary thickening, cholangitis, and a predisposition to cholangiocarcinoma. Zoonotic trematode infections include the sheep liver fluke F. hepatica and the i

[Nonobstructive intestinal ischemia].

Acute or chronic intestinal ischaemia can be the consequence of either intrinsic vascular disease, systemic disease, drugs or surgical procedures. In one quarter of the patients with intestinal ischemia, no major vascular obstructions can be detected. Very rarely, the cause of ischaemia is splanchnic vasoconstriction due to cardiac arrhythmias or sepsis. The bowel becomes ischaemic as a result of underperfusion. The clinical picture resembles the one of occlusive intestinal ischaemia. From the pathological standpoint, the ischaemia is more intense on the antimesenteric border of the bowel and

Intestinal and multivisceral transplantation.

Intestinal transplantation for treatment of irreversible intestinal failure is the newest of the transplantation operations to be developed for clinical use. Because the bowel is more vulnerable to rejection than heart, lung, kidney, liver, or pancreas, practical clinical intestinal transplantation has been relatively slow to develop and is still in its infancy. Imaging studies play an important role in the initial evaluation of the residual native gastrointestinal tract for patients who are potential candidates for enteric implant. After transplantation, gastrointestinal contrast studies help

[Intestinal intussusception in children].

Enteric intussusception is one f the most frequent causes of acute abdomen in early childhood, with an incidence of 1.3-2/1000 children born and higher frequency from the third to the twelfth month. Primary intussusception is related to predisponsing factors such as peristalsis disorders or Peyer's patch hypertrophy induced by viral infection. Secondary intussusception is due to organic injury in the intestinal wall. The most involved sites are the terminal ileum and the ileocecum, the most frequent type is ileocolic intussusception. Many clinical forms exist, including acute enteric intussusc

Laparoscopic intestinal stomas.

PURPOSE: We report our early experiences with laparoscopic intestinal stomas, describing the indications, the surgical techniques, and the complications of this new procedure. METHODS: The medical records of the 17 patients who had successfully undergone laparoscopic intestinal diversion at The University of Texas M. D. Anderson Cancer Center were reviewed. RESULTS: The mean follow-up of this group has been 24.3 (range, 3-61) weeks. Seven patients had their stomas created as part of a laparoscopic abdominoperineal resection, six patients underwent palliative laparoscopic colostomy for an obs

Small intestinal bleeding.

Bleeding from the small intestine may be difficult to diagnose, because of the organ's length and free intraperitoneal location. Although there is a variety of causes of intestinal bleeding, angiodysplasia is the most common. Several different tests can be used to identify the bleeding site preoperatively or intraoperatively, including enteroscopy.

[Vesico-intestinal fistulae].

Review of vesicointestinal fistulae cases treated in the Urology Unit at our hospital over the last few years. Most frequently seen cause was colorectal cancer (66%) and most frequent location was vesicosigmoideal (50%). Clinical signs and symptoms were varied, mainly urinary infection in 100% cases, pneumaturia in 66% and fecaluria in 50%. Opaque enema and cystography were great diagnostic aids as complementary methods. Also U.I.V., cystoscopy, rectoscopy and C.A.T. were used. Treatment was surgical in all cases where the patient's general health status allowed it.

Intestinal protozoa infections.

This article reviews important intestinal protozoal infections in cats. Among intestinal protozoa, Giardia and coccidia are the most important infections. Giardiasis is a common problem in catteries, especially where young kittens run free on the floor. Overcrowding and high humidity favour the survival and transmission of Giardia. Diagnosis is made by fecal examination by direct or concentration methods. Metronidazole is the drug of choice for treating giardiasis. Among the coccidial parasites, Isospora felis and Cryptosporidium sp. are the common infections. Virtually all cats become infecte

[Memento intestinal tuberculosis].

We report the history, symptoms and diagnostic procedures of eight patients (four female, four male; mean age 44.5 years) with abdominal tuberculosis. In all cases differential diagnosis was complicated by an inadequate interpretation of chest X-rays. Only two patients were immigrants. Chemotherapy of tuberculosis was successful in seven patients, one 65-year old female with miliary tuberculosis and ileus died.

Malrotation of the intestine in children.

Intestinal malrotation may be complicated by volvulus and intestinal necrosis. One hundred two children (64 male, 38 female) undergoing surgical abdominal exploration from 1977 to 1987 had malrotation. Fifty-two patients were less than 7 days of age, 13 from 8 to 30 days, 26 from 31 to 365 days, and 11 were older than 1 year of age. Of infants, 39 of 65 had 40-week gestations, 18 of 65 had 36- to 39-week gestations, and 8 of 65 had less than 36-week gestations. Chief symptomatology included: bilious emesis (47), intestinal obstruction (19), abdominal pain (11), and bloody stools (7). Seventy p

Intestinal malrotation in Trinidad.

Fifty-one children under the age of 10 years admitted to a general hospital in Trinidad had a confirmed diagnosis of malrotation of the intestines. This was the primary diagnosis in 20 cases. Analysis of the records of these 20 revealed that one-half were less than 1 month of age at first presentation. Vomiting was a universal complaint, and nearly two-thirds were malnourished. Disturbed bowel habit, anorexia and abdominal pain were also reported. In 30% (six of 20) there were signs of dehydration; an equal number had features of intestinal obstruction. Radiological investigation provided the …

Intestinal malrotation in adults.

Intestinal malrotation presenting with abdominal problems are not common beyond the neonatal period. An adolescent or adult harboring this anomaly may suffer from delay in diagnosis and inappropriate treatment including surgery. We report three patients, aged 17, 31 and 50 years, with intestinal malrotation presenting as an appendicular problem (2) and chronic abdominal pain (1). The former mode of presentation has not been reported so far.
