Impact of obstructive jaundice severity and preoperative biliary drainage on laparoscopic pancreaticoduodenectomy: a bicentric retrospective cohort analysis of 879 cases.

BACKGROUND: Obstructive jaundice elevates perioperative risks in laparoscopic pancreaticoduodenectomy (LPD), though the clinical utility of preoperative biliary drainage (PBD) remains controversial. This study investigated the prognostic significance of jaundice severity and the clinical value of PBD strategies in LPD outcomes. METHODS: We conducted a retrospective analysis of 879 consecutive LPD patients from two tertiary hospitals in southwest China. Propensity score matching (PSM) was applied to balance baseline characteristics. Multivariable logistic regression was used to identify whethe

Migration of a Nasopharyngeal Airway Into the Gastrointestinal Tract During Endoscopic Retrograde Cholangiopancreatography: A Rare Complication in Obstructive Jaundice Management.

BACKGROUND: Obstructive jaundice often requires endoscopic retrograde cholangiopancreatography (ERCP) for diagnosis and management. Airway management during sedation in ERCP poses unique challenges, though device-related complications are rarely reported. CASE PRESENTATION: A 35-year-old male with obstructive jaundice due to distal common bile duct stenosis underwent ERCP under sedation with propofol and oxycodone. A 16Fr silicone nasopharyngeal airway (NPA) was inserted due to mild upper airway obstruction. During the prolonged procedure (185 min), the NPA migrated unnoticed into the stomach

Single-session endoscopic ultrasound-guided gallbladder drainage and biopsy in pancreatic cancer, obstructive jaundice, and acute cholecystitis: A case report.

BACKGROUND: An 81-year-old patient presented to our center with pancreatic head cancer, obstructive jaundice, and acute cholecystitis. Due to duodenal tumor infiltration, both endoscopic retrograde cholangiopancreatography and endoscopic ultrasound (EUS)-guided choledochoduodenostomy were technically challenging. CASE SUMMARY: An EUS-guided gallbladder drainage along with an EUS-guided fine needle biopsy were performed, resulting in a diagnosis of pancreatic cancer resolution of jaundice and improvement in acute cholecystitis, all in a safe and effective single endoscopic session. CONCLUSION

Primary duodenal T/histiocyte-rich large B-cell lymphoma complicated with obstructive jaundice: A case report and review of literature.

BACKGROUND: T/histiocyte-rich large B-cell lymphoma (T/HRBCL) is a highly aggressive subtype of diffuse large B-cell lymphoma characterized histologically by the presence of a few neoplastic large B cells amidst an abundant background of reactive T lymphocytes and/or histiocytes. T/HRBCL commonly affects the lymph nodes, followed by extranodal sites, such as the spleen, liver, and bone marrow, with rare occurrences in the gastrointestinal tract. Primary gastrointestinal T/HRBCL lacks specific clinical and endoscopic manifestations, and it is difficult to differentiate from inflammatory disease

A prospective study of risk factors for in-hospital mortality in patients with malignant obstructive jaundice undergoing percutaneous biliary drainage.

BACKGROUND: The in-hospital mortality rate in patients undergoing percutaneous transhepatic biliary drainage (PTBD) for malignant obstructive jaundice (MOJ) is high. There are few reports on the risk factors associated with hospital death after MOJ, with most of them being retrospective analyses of single factors. Therefore, this study aimed to assess pre-, intra-, and post-procedure risk factors that were independently associated with increased in-hospital mortality in MOJ patients who underwent PTBD. METHODS: One-hundred fifty-five patients with MOJ who underwent initial PTBD were included

An HIV-positive Case of Obstructive Jaundice Caused by Immune Reconstitution Inflammatory Syndrome of Tuberculous Lymphadenitis Successfully Treated with Corticosteroids.

A 60-year-old man was admitted to our hospital because of a persistent fever with enlargement of multiple lymph nodes in the mediastinum and around the pancreatic head. He was diagnosed with tuberculosis and human immunodeficiency virus infection. We started antiretroviral therapy three weeks after the initiation of anti-tuberculous therapy. Two weeks later, jaundice appeared with dilatation of the biliary tract due to further enlargement of the lymph nodes, which seemed to be immune reconstitution inflammatory syndrome (IRIS). The administration of corticosteroids resolved the obstructive jau

[Endoscopic ultrasonography (EUS)-guided transluminal cholangiodrainage (EUCD) - a novel option of interventional endoscopy in the interdiciplinary management of obstructive jaundice].

BACKGROUND: ERCP and PTCD are considered the gold standard in the interventional treatment of biliary obstruction, in particular, with palliative intention. If ERCP and PTCD are not possible, an alternative drainage procedure such as the EUS-guided cholangiodrainage (EUCD) can be used. AIM / METHOD: By the mean of a compact review, indication, technique, variants of approach, number of treated patients and therapeutic procedures reported by various authors, success rate, spectrum and management of complications as well as recommendations for an appropriate follow-up-investigation protocol for

MicroRNA-29a mitigation of toll-like receptor 2 and 4 signaling and alleviation of obstructive jaundice-induced fibrosis in mice.

Cholestasis and hepatitis can cause continuous liver damage that may ultimately result in liver fibrosis. In a previous study, we demonstrated that microRNA-29a (miR-29a) protects against liver fibrosis. Toll-like receptor 2 (TLR2) and TLR4 are pattern recognition receptors of bacterial lipoprotein and lipopolysaccharide, both of which participate in activating hepatic stellate cells and liver fibrosis. The purpose of this study is to characterize the biological influence of miR-29a on TLR2 and TLR4 signaling in livers injured with bile duct ligation (BDL). We performed BDL on both miR-29a tra

Gallbladder stent placement for prevention of cholecystitis in patients receiving covered metal stent for malignant obstructive jaundice: a feasibility study.

PURPOSE: Covered self-expanding metal stents (CSEMS) have been used for palliation of malignant distal biliary strictures. Occlusion of the cystic duct by CSEMS may be complicated by cholecystitis. This potentially could be prevented by placement of a transpapillary gallbladder stent (GBS). PATIENTS AND METHODS: Between 11/2006 and 10/2007, a total of 73 patients (50 male) aged 65 +/- 14 years underwent CSEMS placement for palliation of malignant obstructive jaundice. In cases where CSEMS placement caused occlusion of the cystic duct, a 7 French transpapillary pigtail gallbladder stent (GBS)
