OBJECTIVES: To describe the visual outcome of keratoconus managed with Intacs implantation (Addition Technology Inc, Fremont, Calif) and to define criteria that predict good outcome.

Generally in keratoconus the corneal apex displacement is inferior in position and bilateral in expression. Two young male keratoconic subjects enrolled in the Dundee University Scottish Keratoconus Study (DUSKS) demonstrated with corneal topography the presence of unilateral superior keratoconus. Successful visual rehabilitation of a superior cone with contact lens fitting represents a particular challenge to the contact lens practitioner. This report discusses the management of two such cases.

PURPOSE: To describe a 30-year-old man with bilateral but asymmetric anterior keratoconus and unilateral cornea guttata in the eye with more advanced keratoconus.
METHOD: Case report.
RESULTS: The patient's keratoconus was diagnosed three or four years earlier. The keratoconus was confirmed by slit-lamp examination, keratometry, and computer-assisted topographic analysis. Cornea guttata were confirmed by clinical examination and specular microscopy.

PURPOSE: To determine the physical dimensions and location of the cone in keratoconic corneas from videokeratoscopic height data.
METHODS: Corneal height date from keratoconus patients are obtained with a commercial videokeratoscope and decomposed into the set of orthogonal Zernike polynomials. The low-order Zernike terms are removed from the height data to expose the cone. The location of the peak of the cone and the cone's height and lateral dimensions are measured from the residual height data. We retrospectively studied 91 penetrating keratoplasties in 65 patients to evaluate the overall incidence of allograft rejection and the incidence in unilateral as compared with bilateral cases following penetrating keratoplasty for keratoconus. All surgeries were performed by the senior author following a standardized technique. The average age of the patients was 35 years; the average follow up, 6.5 years. The occurrence of lattice and granular corneal dystrophies in the same eye has been reported in individuals who trace their families to the province of Avellino in Italy. We present the first histopathologic report of keratoconus and bilateral lattice-granular corneal dystrophies occurring in the same patient. This individual's family also originated in Avellino, Italy.
