INTRODUCTION: Neonatal jaundice (hyperbilirubinemia) affects over half of term infants. Although most cases are benign, some progress to severe hyperbilirubinemia, causing acute bilirubin encephalopathy or kernicterus. Early detection is essential. Total serum bilirubin (TSB) is the diagnostic standard, but invasive. Transcutaneous bilirubin (TcB) provides a reliable non-invasive screening tool. The 2022 American Academy of Pediatrics (AAP) guidelines define phototherapy thresholds but do not specify safe discharge bilirubin margins. Understanding outcomes for infants discharged near, but belo…

BACKGROUND: Neonatal jaundice is a common condition characterized by a yellowish discoloration of the skin, conjunctiva, and sclera caused by elevated serum or plasma bilirubin levels during the newborn period. The condition is usually not dangerous, but it can progress to severe hyperbilirubinemia, which can lead to acute bilirubin encephalopathy and kernicterus, a bilirubin-induced neurological damage. Therefore, this study aimed to assess the pooled prevalence of neonatal jaundice and its determinants in Ethiopia.
METHODS: Scopus, PubMed, Google Scholar, Embase, and CINAHL databases were s…

BACKGROUND: Jaundice is common among neonates and if untreated can lead to kernicterus. Diagnosing neonatal jaundice (NJ) using Kramer's method (visual assessment) is considered user-friendly in resource-limited areas. However, there are conflicting findings on reliability of the Kramer's method in the diagnosis of NJ, particularly of black descent. Therefore, study aimed to determine the accuracy of Kramer's method in comparison to the total serum bilirubin (TSB) test in the diagnosis of NJ among neonates of black descent in Tanzania.
METHODS: A cross-sectional study was conducted between Ju…

BACKGROUND: Neonatal jaundice is a common finding in newborns in Asia, including Indonesia. In some cases, the serum total bilirubin levels exceeds the 95th percentile for hours of life (neonatal hyperbilirubinemia). Severe neonatal hyperbilirubinemia (NH) could lead to kernicterus and neonatal death. Glucose-6-Phosphage Dehydrogenase (G6PD) genetic variations and deficiency have been reported in several studies to be associated with NH. This study aimed to analyze the G6PD genetic variations and its activity in neonates with and without hyperbilirubinemia in the Deutromalay Indonesian populat…

OBJECTIVE: Neonatal unconjugated hyperbilirubinemia is one of the most common conditions encountered by the practicing pediatricians. Although it is usually self-limited and benign, the condition is of importance because of the rare instances in which severe hyperbilirubinemia can lead to bilirubin encephalopathy or kernicterus. The uridine diphosphate-glucuronosyl transferase 1A1 (UGT 1A1) gene controls bilirubin conjugation by determining the structure of the enzyme glucuronosyltransferase, which is synthesized in the hepatocyte. In the recent years much has been learned about the relationsh…

OBJECTIVE: The advent of endoscopic synostectomy has enabled early surgery for infants with craniosynostosis. Even though diagnosis is often … the procedure: one had an incidental durotomy with skin infection, and the other had postoperative kernicterus. All infants were fitted for cranial remodeling orthoses following surgery. Three of the 25 infants required reoperations, with 2 patients with syndromic craniosynostosis needing repeat surgery for cranial volume expansion and cosmetic appearance. A …

OBJECTIVE: The aim of this study was to investigate the long-term effects of hyperbilirubinemia on neurological and hearing function in othe … on and pathologic ABR results (p:0.007, p:0.007; p<0.05).
CONCLUSION: This study demonstrates that kernicterus may develop in term newborns with severe hyperbilirubinemia (sTB>25 mg/dl) without any finding of significant hemolysis. Not only the bilirubin level but also the duration of exposure to high bilirubin levels may be effective in the development of b …

Neurological impairments resulting from bilirubin encephalopathy represent a hallmark of bilirubin's neurotoxic effects. Earlier research suggests that bilirubin may contribute to Alzheimer's disease (AD) pathology by inducing neuronal necrosis and abnormal tau phosphorylation. Nevertheless, the precise mechanisms linking bilirubin to neurodegeneration in both neonates and adults remain unclear. To address this, we established two complementary models: a pathological jaundice model using UGT1A1⁻/⁻ neonatal mice and an adult bilirubin exposure model via lateral ventricle injection, followed by

OBJECTIVE: To characterize discrepancies between transcutaneous bilirubin (TcB) measurements and total serum bilirubin (TSB) levels among newborns receiving care at multiple nursery sites across the United States.
METHODS: Medical records were reviewed to obtain data on all TcB measurements collected during two 2-week periods on neonates admitted to participating newborn nurseries. Data on TSB levels obtained within 2 hours of a TcB measurement were also abstracted. TcB--TSB differences and correlations between the values were determined. Data on demographic information for individual newborn

OBJECTIVE: To determine the current trends in the incidence and outcome of bilirubin encephalopathy among Nigerian babies.
METHODS: A review of the hospital records of babies managed for bilirubin encephalopathy at the Wesley Guild Hospital (WGH), Ilesa and Olabisi Onabanjo University Teaching Hospital (OOUTH), Sagamu, both in southwest Nigeria between 2001 and 2005 was carried out. The age, sex, weight, body temperature on admission, place of delivery and outcome of hospitalization were studied. The fatal cases and the survivors were compared for risk factors for mortality.
RESULTS: Fifty e

BACKGROUND: Severe neonatal jaundice and its progression to kernicterus is a leading cause of death and disability among newborns in poorly-resourced countries, particularly in sub-Saharan Africa. The standard treatment for jaundice using conventional phototherapy (CPT) with electric artificial blue light sources is often hampered by the lack of (functional) CPT devices due either to financial constraints or erratic electrical power. In an attempt to make phototherapy (PT) more readily available for the treatment of pathologic jaundice in underserved tropical regions, we set out to test the hy…
