Most knee injuries are the result of rotational, varus, valgus, hyperextension, or translational forces or a combination of these. The pattern of abnormalities seen on magnetic resonance imaging depends on the mechanism of trauma and the position of the joint at the time of injury. Knee injuries can be classified under two broad categories of direct and indirect injuries.

In patients complaining of recurrent patellar dislocations or persistent retropatellar pain due to a dysplastic femoral trochlea, we perform a Henri Dejour trochleoplasty. In this technique the femoral trochlea is deepened by removing the subchondral trochlear bone followed by incision, impaction and fixation of the cartilage flange along the trochlear groove. Between 1976 and 1999, 3714 consecutive primary total knee arthroplasties (TKAs) were done at our institution. Of these, 20 (0.54%) TKAs were done in 18 patients who were > or =90 years old. The average follow-up period was 62.2 months. There was 1 postoperative death among the nonagenarians within 90 days of surgery. All patients had complete pain relief and excellent knee scores using the Knee Society clinical assessment scale. None had an excellent function score at final follow-up examination, however. Chronic anterior knee pain following a direct injury to the front of the knee is common. The persistent symptoms can be troublesome. There is a paucity of documentation regarding its definition, natural history and prognosis. We have reviewed 46 (92%) of a consecutive series of 50 patients with chronic traumatic anterior knee pain, in which other significant acute knee pathology was excluded. Knee dislocations are complex cases requiring careful initial management to prevent devastating consequences caused by limb ischemia from vascular injury. Initial treatment includes gentle reduction and neurovascular assessment including an arteriogram and vascular reconstruction when necessary. Chronic knee pain is common at all ages, particularly in the elderly, among whom it has its greatest impact. Chronic knee pain is often ascribed to osteoarthritis in adults and to chondromalacia patellae in children and adolescents.
