BACKGROUND: Left atrial appendage closure (LAAC) prevents stroke in nonvalvular atrial fibrillation, while device-related left atrial appendage (LAA) perforation may lead to cardiac tamponade.
CASE SUMMARY: A man in his early 80s underwent percutaneous LAAC using the Amulet device. During deployment, sustained counterclockwise torque in a ball-shaped configuration resulted in perforation of the LAA, with device extrusion and progressive pericardial effusion. To avoid bleeding upon extraction, we executed a sequential second-device rescue: The first device was left in situ to tamponade the def…

We present the case of a 2-year-old girl with congenital stenosis of the left inferior pulmonary vein associated with a large perimembranous ventricular septal defect. The child underwent repair of the left inferior pulmonary vein with autologous left atrial appendage as a pedicled tube, followed by closure of the ventricular septal defect. Important technical steps to minimize the restenosis rate are highlighted.

Iatrogenic perforation of the left atrium and aorta during transseptal puncture (TSP) is a rare but potentially life-threatening complication. This report presents the case of an elderly male patient with a 20-year history of atrial fibrillation who experienced this complication during a scheduled left atrial appendage closure procedure. During the procedure, the sheath inadvertently penetrated the left atrial wall and mediastinal tissues, resulting in perforation of the descending aorta. To prevent hemorrhage and secondary injury, we successfully repaired the defects by simultaneously deployi…

Percutaneous non-valvular structural interventions, encompassing patent foramen ovale, atrial or ventricular septal defect closure and left atrial appendage closure, are usually performed in young and healthy patients and represent a valid alternative to pharmacological or surgical interventions. In order to minimize procedural and device related complications, a careful pre-procedural planning together with an accurate intra-procedural imaging are crucial to improve patient's outcome. In this article, we review currently available evidence on patient selection, procedural planning and interve…

BACKGROUND: Left atrial appendage occlusion (LAAO) device dislodgment or embolization is a rare but potentially life-threatening complication. Although there is a designated apparatus available for delivery and implantation of these devices, there are no predefined instructions or required equipment available for extraction of a dislodged device.
OBJECTIVES: This multicenter experience aims to provide a detailed description of the Watchman extraction procedure and to evaluate both its success rates and the potential complications.
METHODS: We analyzed consecutive patients undergoing percutan

AIMS: Randomized trials comparing percutaneous left atrial appendage occlusion (LAAO) with oral anticoagulation (OAC) for stroke prevention in patients with atrial fibrillation have yielded conflicting results across device generations and anticoagulant eras. This Bayesian meta-analysis of randomized trials aimed to compare LAAO vs. OAC for stroke or systemic embolism (SSE) and non-procedural clinically relevant bleeding (CRB).
METHODS AND RESULTS: We conducted a systematic search across five databases (PubMed, Embase, Scopus, Web of Science, and Cochrane CENTRAL) to identify randomized trial

BACKGROUND: Atrial fibrillation (AF) is the most prevalent sustained cardiac arrhythmia and is associated with a markedly increased risk of thromboembolic stroke, primarily originating from the left atrial appendage (LAA). Percutaneous left atrial appendage occlusion (LAAO) is an established alternative for stroke prevention in patients with contraindications to long-term oral anticoagulation. Transesophageal echocardiography (TEE) has traditionally guided LAAO, but intracardiac echocardiography (ICE) is increasingly adopted to facilitate conscious sedation and operator-controlled imaging. This video tutorial demonstrates the minimally invasive closure of a left atrial appendage (LAA) using the WATCHMAN device in a patient with a prior history of heart surgery. Given the failure of anticoagulation therapy, the decision was made to perform surgical closure of the LAA. Due to the patient's previous heart surgery, our standard LAA closure device, the AtriClip, could not be used. As an alternative, the WATCHMAN device was chosen. This device is FDA- and EMA-approved and is designed for implantation via a transcatheter-based approach.

Atrial fibrillation is the most prevalent sustained cardiac arrhythmia, and the therapeutic landscape for stroke prevention has expanded to include catheter ablation for rhythm control and left atrial appendage closure in patients unsuitable for long-term oral anticoagulation. Although minimally invasive, these interventions present complex thrombotic challenges. The left atrial and appendage environment is inherently prothrombotic due to structural remodeling, endothelial injury, blood stasis, and abnormal hemostasis-processes further amplified by procedural instrumentation such as transsepta…

INTRODUCTION: Primary cardiac tumors are exceedingly rare in neonates, with rhabdomyomas predominating in this age group. While myxomas are the most common primary cardiac tumor in adults, they remain a clinical rarity in newborns and typically arise in the right atrium. We report an exceptional case of a left atrial myxoma originating from the left atrial appendage, associated with an unexplained cardiorespiratory arrest and imaging features consistent with a "ball-valve" mechanism, a presentation that, to our knowledge, has not been previously reported in the neonatal population.

BACKGROUND: Atrial fibrillation (AF) is a major risk factor for ischemic stroke, with >90% of thrombi in nonvalvular AF originating in the left atrial appendage (LAA). Thoracoscopic LAA occlusion (LAAO), with or without ablation, is a minimally invasive alternative to endocardial devices or anticoagulation, but multicenter data are limited.
METHODS AND RESULTS: The Totally Thoracoscopic Left Atrial Appendage Occlusion Study (TT-LAAOS) is a multicenter Japanese registry of thoracoscopic LAAO in nonvalvular AF. From March 2018 to December 2024, 567 patients underwent the procedure at 6 institut

AtriClip device has demonstrated an excellent efficacy, long-term durability and safety of left atrium appendage (LAA) closure. We report, the unexpected postoperative transesophageal echocardiography (TEE) finding of thrombi in left atrium (LA) after deployment of an AtriClip in a 73-year-old man with chronic atrial fibrillation and 3-vessel-coronary artery disease undergoing coronary artery bypass grafting (CABG) surgery and LA appendage ligation. The heart was re-arrested and thrombi were retrieve successfully through a left atriotomy. This case emphasizes the unanticipated role of AtriClip

Left atrium appendage closure is a safe and effective therapy for patients with atrial fibrillation and high thromboembolic and hemorrhagic risks. Prosthesis embolization is a potential major complication with an incidence of 0.07%. We report a case of migration of an Amplatzer Amulet (Abbott) device that was successfully retrieved in an innovative way (4).

BACKGROUND: Atrial fibrillation significantly increases the risk of ischaemic stroke, primarily due to left atrial appendage (LAA) thrombosis. Percutaneous left atrial appendage occlusion (LAAO) offers an alternative approach to oral anticoagulation. However, the presence of LAA thrombosis has been considered a contraindication due to the risk of cerebral embolization.
CASE SUMMARY: An 82-year-old woman presented to our department with dyspnoea and history of non-valvular persistent atrial fibrillation as well as heart failure. Serial transoesophageal echocardiography revealed persistent LAA
