Modelling faecal pathogen flows and health risks in urban Bangladesh: Implications for sanitation decision making.

Faecal-oral infections are a major component of the disease burden in low-income contexts, with inadequate sanitation seen as a contributing factor. However, demonstrating health effects of sanitation interventions - particularly in urban areas - has proved challenging and there is limited empirical evidence to support sanitation decisions that maximise health gains. This study aimed to develop, apply and validate a systems modelling approach to inform sanitation infrastructure and service decision-making in urban environments by examining enteric pathogen inputs, transport and reduction by va

Informing life-course immunization and pandemic preparedness: a COVID-19 vaccine evaluation and HPV introduction experience in Sierra Leone.

BACKGROUND: Sierra Leone launched its COVID-19 vaccination in March 2021 and achieved 73.2% full coverage among individuals aged 12 and older by December 2022. In October 2022, the country introduced the human papillomavirus (HPV) vaccine for 10-year-old girls, leveraging infrastructure and lessons from the COVID-19 vaccine deployment. We conducted a COVID-19 vaccine post-introduction evaluation (cPIE) to document best practices, assess impact on routine immunization, and identify key levers that supported HPV vaccine rollout, and recommendations for future pandemic preparedness. METHODS: We

Self-management of lower limb lymphedema at home following gynecologic cancer surgery: a qualitative study of women's experiences and challenges.

INTRODUCTION: As a common chronic complication after gynecological cancer surgery, lower limb lymphedema significantly compromises patients' quality of life, both physically and psychologically. However, patients' adherence to home-based CDT, the internationally recommended first-line treatment, was frequently inadequate. This study aimed to explore the lived experiences and core challenges faced by women managing lower limb lymphedema at home after gynecologic cancer surgery. The insights gained can guide the formulation of improved nursing care strategies. METHODS: A descriptive phenomenolo

Caregiving Appraisal and Social Support: Key Determinants of Quality of Life Among Caregivers of Individuals with Alcohol Use Disorder.

BACKGROUND: Caregivers of individuals with alcohol use disorder (AUD) face unique challenges, including stigma and social isolation, that affect their emotional, physical and psychological well-being. According to Szmukler et al. (1996) stress-coping model, the Experience of Caregiving (EoC) and social support (SS) are crucial resources that help caregivers manage stress. This study examines how caregiving appraisals (positive: ECIP; negative: ECIN) and SS influence the quality of life (QoL) of caregivers. PURPOSE: To explore the relationships between QoL, ECIP, ECIN and SS among caregivers o

Quality of life in older immigrant adults on hemodialysis.

BACKGROUND: In the United States (U.S.), over 34% of individuals with Chronic kidney disease (CKD) are aged 65 or older. Understanding the quality of life (QoL) in this population is essential. While there has been research on the experiences of U.S.-born older adults on hemodialysis, there is limited data on the experiences of older immigrant adults with CKD. PURPOSE: This study aims to explore the perceived QoL of older immigrant adults living with stage 5 CKD, with a focus on how the disease impacts their cultural beliefs, practices, and overall well-being. METHODS: A qualitative, basic i

Nutritional status and the risk of malnutrition in older adults with chronic kidney disease - implications for low protein intake and nutritional care: A critical review endorsed by ERN-ERA and ESPEN.

Increased life expectancy is posing unprecedented challenges to healthcare systems worldwide. These include a sharp increase in the prevalence of chronic kidney disease (CKD) and of impaired nutritional status with malnutrition-protein-energy wasting (PEW) that portends worse clinical outcomes, including reduced survival. In older adults with CKD, a nutritional dilemma occurs when indications from geriatric nutritional guidelines to maintain the protein intake above 1.0 g/kg/day to prevent malnutrition need to be adapted to the indications from nephrology guidelines, to reduce protein intake i

Nutritional and Metabolic Derangements in Pancreatic Cancer and Pancreatic Resection.

Pancreatic cancer is an aggressive malignancy with a poor prognosis. The disease and its treatment can cause significant nutritional impairments that often adversely impact patient quality of life (QOL). The pancreas has both exocrine and endocrine functions and, in the setting of cancer, both systems may be affected. Pancreatic exocrine insufficiency (PEI) manifests as weight loss and steatorrhea, while endocrine insufficiency may result in diabetes mellitus. Surgical resection, a central component of pancreatic cancer treatment, may induce or exacerbate these dysfunctions. Nutritional and me

Diabetes-related distress and quality of life in mothers during the adolescent to young adult transition in type 1 diabetes.

OBJECTIVE: The transition to adulthood presents challenges for families managing type 1 diabetes (T1D), yet maternal well-being during this period remains understudied. Guided by family systems theory and the transactional stress and coping model, this study examined relationships among diabetes-related distress, perceived social support, and quality of life (QoL) in mothers of adolescents and young adults (AYAs; ages 15-25) with T1D. METHODS: Using a cross-sectional, descriptive correlational design, 72 mothers (M age = 48.6 years, 91.7% White, 87.5% married) of AYAs with T1D (M age = 19.4 y

A Newborn Infant with Congenital Central Hypoventilation Syndrome and Pupillary Abnormalities: A Literature Review.

We present a neonate with early onset apnea and bradycardia in the absence of primary cardiorespiratory and central nervous system disorders that eventually required chronic ventilator support starting at 6 hours of life. Molecular testing of paired-like homeobox 2b (PHOX2B) gene mutation confirmed the diagnosis of congenital central hypoventilation syndrome (CCHS). CCHS is a rare genetic disorder characterized by impaired central respiratory control with or without broad spectrum of autonomic nervous system (ANS) dysregulations. Ocular ANS dysregulation is a rare finding in CCHS individuals,

Volatile anesthetic agents for life-threatening pediatric asthma: A multicenter retrospective cohort study and narrative review.

BACKGROUND: Volatile anesthetic agents are described as rescue therapy for children invasively ventilated for critical asthma. Yet, data are currently limited to case series. AIMS: Using the Virtual Pediatric Systems database, we assessed children admitted to a pediatric intensive care unit invasively ventilated for life-threatening asthma and hypothesized ventilation duration and mortality rates would be lower for subjects exposed to volatile anesthetics compared with those without exposure. METHODS: We performed a multicenter retrospective cohort study among nine institutions including chi

Co-designing a framework to communicate patient-centred outcomes in palliative care: involving patients and the public to reframe understanding.

BACKGROUND: Palliative care provides person-centred support to alleviate suffering in people with serious illnesses. However, common misconceptions that it is only for end-of-life care often delays timely access. This study aimed to co-design a framework and use it to co-produce resources that enhance public communication and engagement around patient-centred outcomes in palliative care-a field frequently overlooked despite its broader role in improving quality of life throughout the course of illness. METHODS: Two online co-design workshops, guided by the Knowledge-to-Action framework, engag
