Alcoholic liver disease.

Alcoholic liver disease consists of well defined entities. Fatty liver is the commonest hepatic abnormality in alcoholics and in most cases it is reversible. Possibly a sustained fatty liver occasionally may induce fibrosis. Alcoholic hepatitis or subclinical necrosis may also proceed to hepatic fibrosis. The latter can regress if no further injury occurs or it may develop into frank cirrhosis in case of continuing liver injury. A general model of alcoholic liver disease is presented.

Equine Liver Histopathology.

This review summarizes best practices for equine liver biopsy, including when to perform a biopsy, techniques for sample collection and handling, and strategies to optimize diagnostic yield. It discusses the interpretive framework used by pathologists, common diagnostic challenges, and prognostic scoring systems. Limitations such as sampling error and variability among observers are considered alongside ancillary testing, immunohistochemistry, and in situ hybridization. Emphasis is placed on the importance of communication between clinicians and pathologists to maximize diagnostic and prognost…

Intragastric Ectopic Liver.

BACKGROUND: Ectopic liver (EL) is a rare developmental anomaly defined as hepatic tissue located outside the native liver without vascular or biliary continuity. It is most commonly reported in the gallbladder but has also been described in other intra-abdominal and extra-abdominal sites. The gastric location is exceptionally rare, with only a handful of cases reported in the literature. CASE PRESENTATION: We present the case of a 49-year-old previously healthy male with a 2-month history of dyspepsia and heartburn who underwent diagnostic gastroscopy. This latter showed a 15 mm antral submuc…

Acute liver failure.

Acute liver failure, commonly caused by acetaminophen overdose, is associated with numerous systemic complications including cerebral edema, hypotension, acute kidney injury, and infection. Management is primarily supportive, with an emphasis on excellent neurocritical care. Although some antidotes and targeted treatments exist, the only definitive treatment remains orthotopic liver transplant.

Granulomatous liver diseases.

A granuloma is a discrete collection of activated macrophages and other inflammatory cells. Hepatic granulomas can be a manifestation of localized liver disease or be a part of a systemic process, usually infectious or autoimmune. A liver biopsy is required for the detection and evaluation of granulomatous liver diseases. The prevalence of granulomas on liver biopsy varies from 1% to 15%. They may be an incidental finding in an asymptomatic individual, or they may represent granulomatous hepatitis with potential to progress to liver failure, or in chronic disease, to cirrhosis. This review foc…

Acute liver failure.

Acute liver failure (ALF) is a life-threatening disorder characterised by rapid deterioration of liver function, coagulopathy, and hepatic encephalopathy in the absence of pre-existing liver disease. The cause of ALF varies across the world. Common causes of ALF in adults include drug toxicity, hepatotropic and non-hepatotropic viruses, herbal and dietary supplements, antituberculosis drugs, and autoimmune hepatitis. The cause of liver failure affects the management and prognosis, and therefore extensive investigation for cause is strongly suggested. Sepsis with multiorgan failure and cerebral…

Liver fibrosis quantification.

Liver fibrosis (LF) is the wound healing response to chronic liver injury. LF is the endpoint of chronic liver disease (CLD) regardless of etiology and the single most important determinant of long-term liver-related clinical outcomes. Quantification of LF is important for staging, to evaluate response to treatment and to predict outcomes. LF is traditionally staged by liver biopsy. However, liver biopsy is invasive and suffers from sampling errors when biopsy size is inadequate; therefore, non-invasive tests (NITs) have found important roles in clinical care. NITs include simple laboratory-ba…

Liver Gene Therapy.

Gene therapy is an exciting therapeutic concept that offers the promise of a cure for an array of inherited and acquired disorders. The liver has always been a key target for gene therapy as it controls essential biological processes including digestion, metabolism, detoxification, immunity, and blood coagulation. Metabolic disorders of hepatic origin number several hundreds, and for many, liver transplantation remains the only cure. Liver-targeted gene therapy is an attractive treatment modality for many of these conditions. After years of failure, substantial progress in this field in the pa…

[Polycystic liver disease].

Polycystic liver disease (PLD) includes three entities in adults : biliary hamartomas which develop as a result of ductal plate malformation, autosomal dominant polycystic liver disease (ADPLD) and autosomal dominant polycystic kidney disease (ADPKD) which occur in the setting of genetic disorders. Hamartomas are asymptomatic and benign. PLD are marked by a steady growth of cysts over time, clinically silent in the majority of cases. Symptomatic forms mainly affect women due to the influence of estrogens on the growth of cysts therefore estrogen treatments are contraindicated in this setting. …

Liver Disease: Cirrhosis.

Cirrhosis is pathologic scarring of liver tissue that leads to impaired liver function. It can result from any etiology of chronic liver inflammation and causes significant disease burden. Cirrhosis potentially is reversible through management of the cause, such as nonalcoholic fatty liver disease, viral hepatitis, or alcohol use. As liver disease progresses, compensated (ie, asymptomatic) cirrhosis may decompensate, causing ascites, hepatic encephalopathy, or variceal bleeding. Cirrhosis typically is diagnosed with a history, physical examination, and noninvasive testing, which includes labor…
