A Diagnostic Challenge in Regional Australia: Concurrent Mycobacterium avium Complex Lymphadenitis and Hairy Cell Leukemia.

Introduction: Hairy cell leukemia is a rare, indolent chronic B cell lymphoproliferative disorder characterized by cytopenia, splenomegaly a … matological and vasculitis workups were inconclusive. Lymph node histology demonstrated necrotizing lymphadenitis, which in the context of an otherwise negative investigations was initially suggestive of Kikuchi-Fujimoto disease. Subsequently, cultures from lymph node tissue and blood yielded Mycobacterium avium complex, establishing a diagnosis of disseminated …

Aseptic Meningitis with Cerebellitis Secondary to Histiocytic Necrotizing Lymphadenitis (Kikuchi-Fujimoto Disease): A Case Report.

Kikuchi-Fujimoto disease (KFD) rarely involves the central or peripheral nervous system, including the cerebellum. Herein, we present a case of KFD, describing its notable clinical findings, diagnostic approach, management, and outcome. A 23-year-old woman received a diagnosis of KFD 5 months before being admitted to our hospital. She presented with fever, headache, and painful cervical lymphadenopathy; low-dose prednisolone (15 mg/day) was administered. She complained of explosive occipital headaches that awakened her in the middle of the night for 2 weeks. Vertigo/dizziness with projectile v

Syphilitic Lymphadenitis and Clavicle Pathological Fracture Mimicking Malignant Tumor on 18 F-FDG PET/CT.

A 66-year-old woman presented with 9 days of left clavicle pain, with no history of recent trauma. CT images showed a left clavicle fracture with a surrounding soft tissue mass. An 18 F-FDG PET/CT scan revealed increased FDG uptake in the tonsils, multiple lymph nodes, and the pathologically fractured clavicle, initially suggesting malignancy. However, serologic tests and pathology confirmed syphilitic lymphadenitis and osteomyelitis. Given the inguinal and cervical lymphadenopathy with FDG uptake, potential bone destruction, or pathological fractures mimicking malignancy, syphilis should be c…

Persistently high plasma procalcitonin levels despite successful treatment of tuberculous pleuritis and tuberculous lymphadenitis patients.

In a prospective cohort study, we evaluated plasma PCT levels in 48 TB lymphadenitis (TBLN) and 41 TB pleuritis (TBPE) patients. Measurements of PCT were done in unstimulated plasma of microbiologically and clinically confirmed TBLN and TBPE patients registered for anti-TB treatment at a tertiary care hospital in Lahore, Pakistan. Plasma levels of PCT were found to be raised in 89% of the patients at baseline with a median of 1.5 ng/ml. Levels were higher (p = 0.001) in TBLN as compared to TBPE (2.69, 0.96 ng/ml). PCT levels were not related to the bacterial burden depicted by culture positivi…

Vector-Borne Zoonotic Lymphadenitis-The Causative Agents, Epidemiology, Diagnostic Approach, and Therapeutic Possibilities-An Overview.

In addition to common skin pathogens, acute focal lymphadenitis in humans can, in rare cases, be caused by a zoonotic pathogen. Furthermore, it can develop in the absence of any direct or indirect contact with infected animals, in cases when the microorganism is transmitted by a vector. These clinical entities are rare, and therefore often not easily recognized, yet many zoonotic illnesses are currently considered emerging or re-emerging in many regions. Focal zoonotic vector-borne lymphadenitis and its numerous causative agents, with their variegated clinical manifestations, have been describ…

Microbiological Investigations of Fine Needle Aspirates from Newly Suspected and Previously Treated Tubercular Lymphadenitis Patients.

BACKGROUND: Extrapulmonary tuberculosis (EPTB), particularly tubercular lymphadenitis (TBLN), remains to pose a huge public health problem in Ethiopia. A significant number of TBLN patients who completed a full course anti-TB treatment regimen were reported to have enlarged lymph nodes and other TB-like clinical presentations. This could either be from a paradoxical reaction or microbiological relapse, possibly due to mono/multi-drug resistance. OBJECTIVE: To investigate the rate of mono and multidrug resistance patterns of Mycobacterium tuberculosis as a cause of the observed treatment failu…

Kikuchi-Fujimoto disease (histiocytic necrotizing lymphadenitis) with atypical encephalitis and painful testitis: a case report.

BACKGROUND: Kikuchi-Fujimoto disease is a self-limited clinicopathologic entity that is increasingly recognized worldwide. Kikuchi-Fujimoto disease is characterized by cervical lymphadenopathy occurring in young adults. Neurologic involvement is rare, and testitis directly caused by Kikuchi-Fujimoto disease has not yet been reported. CASE PRESENTATION: A 19-year-old man was brought to our clinic with complaints of fever, headache, fatigue, and left lower quadrant pain that had persisted for 3 weeks. On physical examination, painful cervical lymphadenopathies were observed. Meningitis was susp

Utility of endobronchial ultrasound-guided transbronchial needle aspiration in diagnosing non-specific inflammatory intrathorcacic lymphadenitis.

BACKGROUND AND OBJECTIVE: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) has emerged as a minimally invasive technique for diagnosing intrathoracic malignancies and some benignancies; however, there are no data available on the utility of EBUS-TBNA for the diagnosis of non-specific inflammatory intrathoracic lymphadenitis. METHODS: A prospective analysis was performed from 104 patients with enlarged lymphadenopathy suspected of non-specific lymphadenitis referred for EBUS-TBNA between October 2009 and March 2012. Rapid on-site cytological evaluation was not adopt…

Diagnostic yield of fine needle aspiration biopsy in HIV-infected adults with suspected mycobacterial lymphadenitis.

BACKGROUND: Fine needle aspiration biopsy (FNAB) has been shown to be the diagnostic procedure of choice for superficial lymphadenitis in tuberculosis endemic regions. METHODS: We conducted a retrospective laboratory-based study to determine the bacteriological yield of clinically suspected mycobacterial tuberculous lymphadenitis following FNAB in adults, and specifically HIV-positive patients, to determine the need for the introduction of automated nucleic acid amplification tests (NAATs) such as the Xpert MTB/RIF assay as the initial diagnostic modality. RESULTS. A diagnostic yield of 80% w…

Conservative wait-and-see therapy versus antibiotic treatment for nontuberculous mycobacterial cervicofacial lymphadenitis in children.

BACKGROUND: In this explorative study, 50 children with microbiologically confirmed nontuberculous mycobacterial cervicofacial lymphadenitis were randomized to either receive antibiotic therapy or follow a conservative wait-and-see approach. Our primary objective was to assess the time for all infected lymph nodes to heal in patients after the nonantibiotic, wait-and-see treatment, compared with patients after a 12-week course of clarithromycin and rifabutin. METHODS: Fifty children (19 boys and 31 girls) with a polymerase chain reaction (PCR)- or culture-confirmed diagnosis of cervicofacial …

Development of a bacterin-toxoid vaccine using a Korean isolate for protection against caseous lymphadenitis in goats.

Caseous lymphadenitis (CLA), a chronic infectious disease caused by Corynebacterium pseudotuberculosis, remains a major concern in small ruminant populations owing to its persistent nature and impact on animal productivity. Despite the availability of commercial vaccines, their efficacy in goats has been inconsistent and field-based evidence supporting their use remains limited. In this study, we report the development and evaluation of a novel bacterin-toxoid vaccine formulated with C. pseudotuberculosis strain 51-12A obtained from a Korean native goat. The toxoid, prepared from culture super…

Incidence of Bacille Calmette-Guérin associated lymphadenitis in healthy children: A systematic review and meta-analysis.

Few systematic reviews or meta-analyses have addressed on the incidence of Bacille Calmette-Guérin (BCG) lymphadenitis in healthy children. A systematic literature search was independently conducted by two researchers using four English and Korean databases (PubMed, Scopus, KMbase, RISS). The risk of bias was assessed. A total of 52 peer-reviewed journal articles (about 1.8 million children) were included in meta-analyses. The incidence proportion of BCG lymphadenitis ranged from 0.0% to 25.6%. The pooled incidence proportion was 2.5% (95% CI, 1.6-3.6; I2 = 99%). Studies employing active (vs. …

Mesenteric Lymphadenitis on Point-of-Care Ultrasound in Pediatric Patients With Group A Streptococcal Pharyngitis: A Case Series.

Abdominal pain is a common complaint in pediatric patients with Group A streptococcal (GAS) pharyngitis. Etiology of abdominal pain from GAS infections are unknown. Point-of-care ultrasound (POCUS) is often used by pediatric emergency medicine physicians to evaluate children with abdominal pain. We describe 12 pediatric patients who presented to our Pediatric Emergency Department with symptoms suggestive of acute pharyngitis and abdominal pain, tested positive for GAS pharyngitis, and were found to have mesenteric lymphadenitis (mLAD) on POCUS. Findings on POCUS suggest that abdominal pain in …

Bartonella henselae mediastinal lymphadenitis mimicking malignancy with critical airway compression in a child: a case report.

Cat-scratch disease, caused by Bartonella henselae, is usually a self-limited infection presenting with regional lymphadenopathy in children. Thoracic involvement is uncommon, and mediastinal lymphadenitis with clinically significant airway compression may closely mimic malignancy. We report a previously healthy 6-year-old boy who presented with persistent fever, mild cough, weight loss, and cervical lymphadenopathy. Chest computed tomography revealed necrotic mediastinal lymphadenopathy forming a mass-like lesion with compression of the right middle lobe bronchus and associated atelectasis. B…

Clinical and Epidemiological Profile and Treatment Outcomes of Tuberculous Cervical Lymphadenitis: A Hospital-Based Observational Study.

BACKGROUND: Tuberculosis (TB) continues to be the biggest health problem in developing countries with enormous social and economic implications. The commonest form of extrapulmonary TB (EPTB) is tubercular cervical lymphadenitis, or scrofula. The diagnosis and management of TB lymphadenitis is challenging, given the multiple differentials. Also, relapse and persistence of lymph nodes (LNs) despite treatment result in significant morbidity. METHODOLOGY: A hospital-based descriptive study was conducted among 35 newly diagnosed cervical tubercular lymphadenitis patients who were initiated on ant…

POCUS Diagnosis of Bacterial Lymphadenitis Later Determined to be Cat Scratch Disease: A Unique Presentation and Review.

Acute lymphadenopathy has a wide range of possible etiologies, ranging from self-limiting viral infections to life-threatening malignancy. Point of care ultrasound (POCUS) can play a crucial role in identifying lymphadenopathy and ruling out other potential causes of soft tissue swelling, such as hernias, abscesses, and malignancies. Furthermore, POCUS enables the characterization of lymphadenopathy by evaluating the size, shape, echogenicity, and vascularity of the involved lymph nodes, thereby helping to identify the presence and extent of pathology. When performed at the time of initial pre

Walking a Tight Rope: Occult Lymphoma in a Case of Resistant Dermatomyositis Complicated by Tubercular Lymphadenitis and Gumma.

Dermatomyositis (DM) is an autoimmune inflammatory disease, characterized by symmetrical proximal myopathy and cutaneous manifestations. DM is associated with upto a 6-fold increased risk of cancer. Complications secondary to underlying cancer are a leading cause of mortality in DM. Here, we discuss the two year clinical course of an elderly male with recalcitrant DM. This case was complicated by tubercular lymphadenitis followed by gumma. Subsequently, he was diagnosed with lymphoma and succumbed to death. This case emphasizes the need to do extensive malignancy screening at regular intervals…
