Recurrent posttransplant lymphoproliferative disorder involving the larynx and trachea: case report and review of the literature.

OBJECTIVES: Posttransplant lymphoproliferative disorder (PTLD) is a well-recognized complication of solid organ transplantation and commonly affects upper airway lymphoid tissue. Tracheal and laryngeal involvement in patients with PTLD, however, is rare. We present one such case. METHODS: We report the case of a patient with recurrent PTLD involving the larynx and trachea and describe the presentation, evaluation, management, and outcome. RESULTS: An 11-year-old boy who underwent bilateral nephrectomy and renal transplantation as an infant was admitted to the hospital with chronic cough, fev

Posttransplant lymphoproliferative disorder presenting as a nonhealing extraction socket: a case report and review of the literature.

Posttransplant lymphoproliferative disorder (PTLD) is a well-recognized complication of long-term immunosuppression following hematopoietic or solid organ transplantation and is associated with significant morbidity and mortality. We present a unique case of PTLD that manifested with a nonhealing dental extraction socket 17 years after renal transplantation. We summarize the existing literature and present a review of an additional 25 cases. These highlight the variable clinical presentations of PTLD within the oral cavity and clear potential for delayed presentation at this anatomical subsite

IgG4-related disease: a novel lymphoproliferative disorder discovered and established in Japan in the 21st century.

IgG4-related disease is a novel lymphoproliferative disorder that shows hyper-IgG4-γ-globulinemia and IgG4-producing plasma cell expansion in affected organs with fibrotic or sclerotic changes. Patients show systemic inflammatory conditions and various symptoms depending on the affected organ. Since the first report of patients with elevated serum IgG4 in sclerosing pancreatitis in 2001, various systemic disorders described by many names have been reported. Despite similarities in the organs involved in IgG4-related Mikulicz's disease and Sjögren's syndrome, there are marked clinical and patho

Post-transplant lymphoproliferative disorder localized in the colon after liver transplantation: report of a case.

Post-transplant lymphoproliferative disorder (PTLD) is a life-threatening complication of any organ transplantation. It is more common in children than in adults, and the risk factors include Epstein-Barr virus (EBV) infection and immunosuppression. We report a case of colonic marginal zone B-cell lymphoma occurring 4 years after liver transplantation in a 52-year-old man who had been taking immunosuppressive agents, namely, cyclosporin, mycophenolate mofetil (MMF), and prednisolone. A routine follow-up colonoscopy showed a 2.0-cm diffuse nodular transverse colon polyp covered with erosive muc

Chemokine receptor repertoire reflects mature T-cell lymphoproliferative disorder clinical presentation.

The World Health Organization classification of mature T-cell lymphoproliferative disorders, combines clinical, morphological and immunophenotypic data. The latter is a major contributor to the classification, as well as to the understanding of the malignant T-cell behavior. The fact that T-cell migration is regulated by chemokines should, in theory, enable us to identify tissue tropism and organ involvement by neoplastic T-cells by monitoring chemokine receptor surface expression. To address this issue we compared the expression of several early and late inflammatory, homeostatic, and organ s…

Intrathecal rituximab treatment for pediatric post-transplant lymphoproliferative disorder of the central nervous system.

Post-transplant lymphoproliferative disorder (PTLD) in the central nervous system (CNS) has a poor prognosis. New therapeutic approaches should be explored. We report our experience with intrathecal administration of rituximab in a 10-year-old kidney allograft recipient with PTLD in the CNS. After standard treatment had failed, we tried to treat the patient by administering rituximab directly into the cerebral ventricle through an Omaya reservoir, in addition to conventional intrathecal and systemic chemotherapy. This strategy resulted in a disappearance of clinical symptoms and a negative pos

Multicentric Castleman disease, Kaposi sarcoma, hemophagocytic syndrome, and a novel HHV8-lymphoproliferative disorder.

The case of a 45-year-old man with HIV disease with bulky lymphadenopathy, fevers, and weight loss is presented. Immune reconstitution inflammatory syndrome was initially believed to be the cause, but the patient was found to suffer from Kaposi sarcoma, multicentric Castleman disease, hemophagocytic syndrome, and a newly described lymphoproliferative disorder. The diagnostic reasoning related to the causes of the fulminant illness is discussed as well as the roles of Epstein-Barr virus, human herpesvirus 8, and hepatitis C virus in the patient's clinical presentation and the therapeutic choice

Primary cutaneous plasmacytoma (posttransplant lymphoproliferative disorder, plasmacytoma-like) in a heart transplant patient.

Extramedullary plasmacytomas in posttransplant patients are rarely encountered. We present the fifth case of a primary cutaneous plasmacytoma (posttransplant lymphoproliferative disorder, plasmacytoma-like) that developed in a heart transplant patient. The tumor presented as a solitary nodule of the skin 10 years after transplantation. It subsequently involved 2 other cutaneous sites and remained confined to the skin for 5 years. The neoplastic cells were Epstein-Barr virus small RNAs (EBER 1,2) positive and EBV-latent membrane protein 1 (LMP 1) negative, corresponding to type I EBV latency. T

Dermatomyositis associated with lymphoproliferative disorder of NK cells and occult small cell lung carcinoma.

We report a case of dermatomyositis that was associated with a marked expansion of circulating NK cells (CD3-CD16+) with homogeneous phenotype (CD56- CD94- CD159a- CD244+) which satisfied the updated diagnostic criteria for the lymphoproliferative disorder of granular lymphocytes/NK cells. Immunosuppressive treatment induced remission of the dermatomyositis and decreased the number of circulating NK cells. However, a small cell lung carcinoma, that was occult at the time of the presentation of dermatomyositis, was diagnosed 10 months thereafter.

Humanized anti-CD20 monoclonal antibody (Rituximab) treatment for post-transplant lymphoproliferative disorder.

INTRODUCTION: Post-transplant lymphoproliferative disorders (PTLD) is a consequence of Epstein-Barr virus (EBV) infection and is a B-cell hyperplasia with CD-20 positive lymphocytes. The treatment of PTLD includes reduction/withdrawal of immunosuppression and chemotherapy. This study reports our center experience with humanized monoclonal antibody against CD-20 (Rituximab) for the treatment of PTLD. MATERIAL AND METHODS: Eight cases of PTLD after solid organ transplantation [six kidney, one kidney/pancreas (KP) and one liver] occurred between September 1998 and October 2001. The mean time bet…

Concomitant angioimmunoblastic T-cell lymphoma and low grade B-cell lymphoproliferative disorder.

The presence of a rearranged immunoglobulin gene, in addition to the expected T-cell receptor gene rearrangement, is a frequent, albeit poorly understood, finding in the setting of angioimmunoblastic lymphadenopathy. A case of an angioimmunoblastic T-cell lymphoma is presented, where this apparently paradoxical dual gene rearrangement could be ascribed to the coexistence of an occult B-cell lymphoproliferative disorder.

Primary posttransplant lymphoproliferative disorder of the gallbladder in a lung transplant patient presenting with acute cholecystitis.

BACKGROUND: Acute cholecystitis in an immunocompromised host is potentially devastating. Posttransplant lymphoproliferative disorder (PTLD) is a well described complication of immunosuppressive therapy used after solid organ transplantation; however, isolated involvement of the gallbladder has not been described. METHODS: Case report format is used. RESULTS: We report a case of PTLD isolated to the gallbladder, as well as histological evidence of acute cholecystitis, in a patient who presented with signs and symptoms of acute cholecystitis 1 year after single lung transplant. CONCLUSIONS: P

Unusual gingival presentation of post-transplantation lymphoproliferative disorder: a case report and review of the literature.

Post-transplantation lymphoproliferative disorder is a well-documented complication of solid organ or bone marrow transplantation. Histologically, it is characterized by an abnormal proliferation of lymphocytes, which can range from benign B-cell hyperplasia to malignant lymphoma. Non-Hodgkin's lymphoma (NHL) is associated with several risk factors, such as congenital or acquired immunodeficiency states, autoimmune disorders, and infectious agents (eg, Epstein-Barr virus). Primary sites of presentation in the head and neck are Waldeyer's ring, paranasal sinuses, salivary glands, the oral cavit

A unique malignant T-cell lymphoproliferative disorder with neutropenia simulating hairy cell leukemia.

The association of neutropenia with an indolent chronic T-suppressor cell lymphoproliferative disorder (LPD) has been well documented. The morphologic features and course suggest that this is a benign disorder. The authors studied a 58-year-old man with a chronic T-cell LPD, splenomegaly, and neutropenia. Chromosomal analysis revealed multiple abnormalities which progressively increased in number as the marrow lymphocytosis became more prominent. Subsequently he developed small bowel infiltration. A splenectomy resulted in only brief improvement in the neutropenia. Immunopathologic examination

Behçet's disease associated with a lymphoproliferative disorder, mixed cryoglobulinemia, and an immune complex mediated vasculitis.

A woman, now 59 has been followed for 13 years with several manifestations of Behçet's disease. These were aphthous stomatitis, genital ulcers, uveitis causing blindness, recurrent erythema nodosum, and synovitis. In 1970 a poorly differentiated diffuse lymphocytic lymphoma appeared and was treated with radiotherapy. In 1976 she developed a mixed cryoglobulinemia and an immune complex mediated vasculitis manifested by purpura and neuropathy which improved on prednisone and chlorambucil therapy. The subsequent course of her lymphoproliferative disorder suggest that it was in fact benign.

Chronic lymphoproliferative disorder with unusual clinical, morphologic, ultrastructural and membrane surface marker characteristics.

Four of 105 patients with chronic lymphocytic leukemia (CLL) manifested clinical, morphologic, ultrastructural and membrane surface marker characteristics that differed from those found in patients with typical CLL of demonstrated B-lymphocyte origin. These four patients presented with moderate increases in absolute lymphocyte counts, absolute neutropenia, polyclonal hypergammaglobulinemia and hepatosplenomegaly without lymphadenopathy. Two of them were unusually young, 19 and 25 years old, at the time of diagnosis. The proliferating lymphocytes carried receptors for sheep erythrocytes, a T-ly

Machine Learning Models for Posttransplant Lymphoproliferative Disorder (PTLD) Risk Prediction in Thoracic Transplantation.

Posttransplant lymphoproliferative disorder (PTLD) is the second most common malignancy in thoracic transplant recipients and is associated with poor survival. Accurate risk prediction is essential for prevention and management. Adult (≥18 years) heart, lung, and heart-lung transplant recipients were extracted from the Scientific Registry of Transplant (SRTR) and the United Network of Organ Sharing (UNOS) databases. A total of 160 donor and recipient pretransplant variables, including demographics, laboratory tests, induction therapy, and human leukocyte antigens (HLA), were analyzed. Risk sco

Exosomal miRNA expression in transplant recipients with EBV-associated post-transplant lymphoproliferative disorder.

BACKGROUND: Epstein-Barr Virus (EBV) drives the development of EBV-associated malignancies such as post-transplant lymphoproliferative disorder (PTLD), evading immune detection. However, how EBV is able to evade immune surveillance is not well understood. One theory suggests that modulation of host microRNAs (miRNAs) expression can help escape immune recognition. EBV-encoded miRNAs support tumor cells survival and manipulate expression of host miRNAs. EBV-infected cells package their content into exosomes, which are transferred throughout the body. Tumor and infected cells by packaging their o

Management of a Complex Orbital Post-transplantation Lymphoproliferative Disorder: A Case Report and Literature Review.

BACKGROUND/AIM: Post-transplantation lymphoproliferative disorder (PTLD) is an uncommon but serious complication of organ transplantation. Management often requires multiple treatment modalities and reduction in immunosuppressant treatment (RIT). Orbital involvement is rare, and optimal diagnostic and therapeutic approaches remain uncertain. CASE REPORT: Here, we report a case of localized orbital PTLD that was successfully treated with RIT alone. A 62-year-old man, who had undergone kidney transplantation 18 years prior, presented with left eye pain and proptosis. Imaging revealed an enhanci
