Diagnosing inflammatory and non-inflammatory periapical disease.

Most central bony lesions appearing in a periapical location are inflammatory in nature secondary to pulpal degeneration. However, cysts, tumors, and other diseases may be easily confused with such endodontic lesions. Four clinical cases are presented, illustrating the importance of thorough evaluation and the role of biopsy in appropriate cases.

Condylar metastasis from prostatic carcinoma mimicking temporomandibular disorder: a case report.

BACKGROUND: Although metastatic carcinoma is the most common malignant tumor of the bone, less than 1% of all metastatic bone lesions are presented in the maxillofacial area. As the mandibular body is the most frequent localization, metastasis to the mandibular condyle is extremely rare. CASE REPORT: This report describes a rare case of prostate carcinoma metastatic to the mandibular condyle in a 75-year old man, who was referred because of persistent pain in the temporomandibular joint (TMJ) region and a limitation of opening, initially misdiagnosed and treated as temporomandibular disorder

Ulnar forearm osteocutaneous flap harvesting using Kapandji procedure for pre-existing complicated fibular flap on mandible reconstruction--cadaveric and clinical study.

UNLABELLED: It is not uncommon that after using a fibular flap for lower gum cancer reconstruction, nonunion, chronic osteomyelitis, or fibular bone exposure occurs, which requires a composite bone and soft tissue reconstruction. Radial forearm osteocutaneous flap possesses the risk of stress fracture. Ulnar forearm osteocutaneous flap can be another option for small bone defect reconstruction. PATIENTS AND METHOD: Six patients who had undergone fibular flap for mandible reconstructions and sustained either bone exposure (3 patients), chronic osteomyelitis (1 patient), malocclusion (1 patient

Improved mandible function after hemimandibulectomy, condylar head preservation, and vascularized fibular reconstruction.

Temporomandibular joint dysfunction after tumor extirpation of the hemimandible is a frequent sequela after condylar head reconstruction. Condylar head resection is often performed because of oncological and vascular considerations. Recent studies have demonstrated that malignancies of the mandibular ramus and body rarely involve the condylar head, and that the vascularity and supportive structures of the condylar head are sufficient to maintain viability and function. This study demonstrates that temporomandibular joint function is preserved after hemimandibulectomy without resection of the c

Predictors and Implications of Bony Union of Osseous Free Flaps for Oromandibular Reconstruction.

BACKGROUND: Optimal contouring of osseous free flaps for mandibulectomy often requires osteotomies. The impact of malunion or nonunion on hardware extrusion is not well-documented. This study assesses postoperative bony union, its correlation with plate extrusion, and related demographic and surgical predictors. METHODS: A retrospective study of 127 patients who underwent segmental mandibulectomy and osseous free tissue transfer from 2004 to 2019. Bony union was assessed via computed tomography (CT) scans at 6-9, 9-15, and 15-24 months using a callus scale. Logistic regression analyzed the re

Pathological fractures of the mandible: a diagnostic and treatment dilemma.

Pathological fractures of the facial bones are relatively rare. Four pathological mandibular fractures are presented and the difficulty in diagnosing the underlying cause and the dilemma of treatment choice are discussed with a review of the literature. The authors found the incidence of pathological fractures of the facial bones in their series was 0.47%. Biopsy followed by rigid internal fixation, preferably with an AO reconstruction plate bridging the area of pathology after its removal, permits normal function and allows good and undisturbed healing which will inevitably be prolonged in su

Numb chin syndrome: a rare and often overlooked symptom.

Numb chin syndrome (NCS) describes a sensory neuropathy characterized by numbness in the distribution of the terminal branch of the mandibular division of the trigeminal nerve. Benign as well as malignant diseases are known to cause NCS. This is often misdiagnosed, and in some cases the symptom may lead to a late detection of malignancy. Reports of 10 cases in which NCS was the presenting and isolated symptom, caused by extracranial malignancies, drugs, or dental/surgical interventions, are presented. This article outlines the symptoms and the diagnostic approaches taken, provides a short revi

Awake Fiberoptic Intubation for Segmental Mandibulectomy in a Patient with Follicular Ameloblastoma of the Anterior Mandible: A Case Report.

Awake tracheal intubation (ATI) is recommended for patients with predicted difficult airways. This approach preserves spontaneous ventilation until the airway is secured. In this case, a 52-year-old male (ASA II) with a large anterior mandibular tumor underwent segmental mandibulectomy. Awake fiberoptic intubation (AFOI) was performed nasally via a flexible bronchoscope in a semi-sitting, face-to-face position. Topical anesthesia was achieved by combining nebulized 2% lidocaine (4 mL, 80 mg) with the spray-as-you-go technique (2-4 mL of 2% lidocaine). The total dose used was approximately 120-

The role of surgical navigation in computer-assisted mandibular reconstruction: is it necessary?

BACKGROUND: Accurate mandibular reconstruction following tumor ablation is important yet challenging. While computer-assisted surgery and surgical navigation have been applied widely in maxillofacial reconstruction, the accuracy and the efficacy remain debatable due to the native mobile nature. This study aimed to evaluate the surgical outcomes and accuracy of mandibular reconstruction aided by different types of adjunctive computer-assisted techniques with or without intraoperative navigation. MATERIAL AND METHODS: Patients with anterior and/or lateral mandibular defects who underwent microv

Quantification of the Bimodal Plate-Specific Complication Profile Associated With Reconstruction of Segmental Mandibular Defects With Reconstruction Plate and Soft Tissue Flap: Evidence From Systematic Review.

Reconstruction of segmental mandibular defects with a mandibular reconstruction plate and separate soft tissue flap can be indicated in certain patients. Whilst this technique can provide acceptable results, its exact longevity and timing of associated complications is poorly understood. This study was designed to quantify and plot in time the complication profile associated with this technique of mandibular reconstruction.A search of the Medline and PubMed Central databases was conducted using the terms "Mandibulectomy" and "Reconstruction Plate." Articles reporting total number of plate extr

Plate-related complication and health-related quality of life after mandibular reconstruction by fibula flap with reconstruction plate or miniplate versus anterolateral thigh flap with reconstruction plate.

BACKGROUND: Plate-related complications are major long-term complications in mandible reconstruction. There are controversies regarding the use of a reconstruction plate versus miniplates and a bone flap versus a soft tissue flap with a bridging plate. Direct comparisons of a fibula flap and an anterolateral thigh flap, the applicability between a reconstruction plate and miniplate, and the correlation between plate-related complications and quality of life remain unclarified. Therefore, this study aimed to the explore complications of different flaps and plates and how they impact the patient
