Postoperative Rehabilitation with a CAD/CAM- Fabricated Occlusal Splint Following Plate Reconstruction After Hemimandibulectomy and Condylectomy: A Case Report.

Mandibular deviation (MD) is a common reconstruction sequela after segmental mandibulectomy. Although proper postoperative rehabilitation is critical for MD management and minimization, the available information is limited. This report describes postoperative. rehabilitation with an occlusal splint fabricated using CAD/CAM (CAD/CAM-OS) and the results of a 3D occlusal analysis using an intraoral scanner (IOS) after hemimandibulectomy and plate reconstruction. Despite the short follow-up, adherence to postoperative rehabilitation with CAD/CAM-OS for MD correction, even during radiotherapy, was

An Innovative Methodology: The Submental Island Flap Offers Enhanced Soft Tissue Coverage to the Vascularized Iliac Crest Flap.

The vascularized iliac crest flap has garnered widespread acclaim within the field of mandibular reconstruction attributable to its sufficient bone mass and congruent curved morphology. However, when the precise orientation of the iliac crest is imperative during mandibular reconstruction and there exists an accompanying defect within the oral soft tissue, the indispensability of an additional flap to facilitate concurrent defect repair becomes evident. In such instances, the submental island flap emerges as an exemplary candidate.

Clear cell variant of intraosseous calcifying epithelial odontogenic tumor: a case report and review of the literature.

CEOT is a rare benign, but locally aggressive odontogenic tumor, and some authors have claimed that the biologic behavior of the clear cell variant is even more aggressive, with greater propensity to recur. We report a rare case of clear cell calcifying epithelial odontogenic tumor (CEOT) and discuss its possibly aggressive behavior. A 40-year-old woman experienced an asymptomatic expansion of the left posterior mandible. After radiographic examination and biopsy, a block resection was performed. Histologic examination included analysis of Ki-67 reactivity as a marker of tumor growth activity.

Unique Application of the Jaw-in-a-Day Technique in a Case of Trauma.

Mandibular reconstruction with a fibula free flap is often used for advanced oral malignancies, osteonecrosis, or benign tumors to restore function and appearance after segmental mandibulectomy. The "Jaw in a Day" (JIAD) technique enables immediate restoration of form and function with a fibula free flap, implants, and a temporary prosthesis. This case reports the unique use of the JIAD technique in a young female patient with a traumatic mandibular nonunion fracture and chronic osteomyelitis, demonstrating its efficacy in significantly enhancing her quality of life. Laryngoscope, 135:2331-233…

Digitally-assisted Design for Precise Mandibular Defect Repair Using Autogenous Bone.

This study introduces a novel surgical technique that leverages digital design for the precise repair of mandibular defects resulting from benign jaw tumors. The restoration of the mandibular defect is accomplished through autologous bone grafting from the mandible itself. This method significantly diminishes surgical complexity and risk, meeting the patient's preference to avoid additional surgical sites. Notably, 15 months postsurgery, the patient's mandible dimensions were suitable for dental implantation. Therefore, this technique has proven effective in repairing mandibular defects caused

Axial split step osteotomy of free iliac crest flaps for mandible reconstruction: Preliminary results.

BACKGROUND: Vascularized bone grafting (VBG) is preferred for mandibular reconstruction post-tumor ablation. Although various bone-free flaps are used, their application is compromised by limitations including insufficient bone volume and poor shape. Here, we report mandibular reconstruction using axial split-step osteotomy with an iliac crest-free flap. METHODS: Over December 2018-November 2020, 12 patients underwent mandibular reconstruction via axial split osteotomy using a free iliac-crest flap. RESULTS: The preoperative iliac-crest length was 5.7-9.5 mm (median, 7.5 cm); the mean post-a…

Reconstruction of acquired oromandibular defects.

Acquired defects of the mandible resulting from trauma, infection, osteoradionecrosis, and ablative surgery of the oral cavity and lower face are particularly debilitating. Familiarity with mandibular and cervical anatomy is crucial in achieving mandibular reconstruction. The surgeon must evaluate which components of the hard and soft tissue are missing in selecting a method of reconstruction. Complexity of mandibular reconstruction ranges from simple rigid internal fixation to microvascular free tissue transfer, depending on defect- and patient-related factors. Modern techniques for microvasc…

A systematic approach to patient safety and surgical education during a surgical mission in a resource-limited setting: A case report of double free flap reconstruction for a lower facial defect.

Reconstruction of complex mandibular and lower facial defects poses significant challenges, especially in resource-limited settings. Double free flap reconstruction offers tailored tissue replacement when single-flap options are inadequate. We report the case of a 25-year-old male with a large composite mandibular and oral floor defect following a failed pedicled flap reconstruction after gunshot trauma. A structured checklist was used for planning and executing a safe and educational microsurgical procedure in a resource-limited setting. The patient underwent successful reconstruction using a

Virtual surgical planning for extensive fibrous dysplasia in the mandible.

The reconstruction of extensive mandibular defects is a challenge for which virtual surgical planning is extremely helpful. This report describes the case of a 33-year-old woman who experienced the gradual development of a severe mandibular deformity with elongation of the chin and mandibular border because of fibrous dysplasia. Consequently, 19 cm of the mandible extending from the neck of the condyle to the contralateral body was resected together with vestibular and lingual deformities. This bone was replaced with a fibula-free flap. For planning, a virtual resection was performed via a Web

A Novel Method for Secondary Mandible Reconstruction to Re-Achieve a Native Condyle Position Comprising a New Design for Cutting Guides and New Positioning Devices.

Secondary mandibular reconstruction using fibular free flaps (FFF) is a technical challenge for surgeons. Appropriate operation planning is crucial for postoperative quality control and is notably necessary for the (re-) achievement of a physiological condylar position, and the sensible expansion and shaping of the transplant. Computer-assisted planning may help to reconstruct mandibular defects in a patient-specific and precise manner. Herein, we present a newly-developed workflow for secondary mandibular reconstruction using FFF; it comprises digital planning and in-house manufacturing to pe…
