When immunotherapy hurts: neoadjuvant PD-1 inhibitors increase opioid use after mastectomy with reconstruction.

PURPOSE: Programmed cell death protein-1 (PD-1) inhibitors have transformed cancer therapy, but evidence suggests PD-1 signaling modulates µ-opioid receptor activity, potentially attenuating opioid analgesia. This study evaluated the effect of PD-1-based neoadjuvant chemotherapy (NAC) on perioperative opioid use and pain outcomes following mastectomy with immediate reconstruction. METHODS: We retrospectively reviewed consecutive mastectomies with immediate reconstruction following NAC between 2020 and 2024. Patients were grouped by PD-1-based NAC (NAC + PD-1) versus NAC without PD-1 inhibitor…

Nipple-Sparing Skin-Reducing Mastectomy and Immediate Prepectoral Breast Reconstruction: A New Surgical Approach.

BACKGROUND: Nipple-sparing mastectomy in medium-large, ptotic breasts presents challenges because of redundant skin and risks to the nipple-areola complex (NAC). Advances in skin-reducing techniques and immediate prepectoral reconstruction aim to improve both oncological safety and aesthetic outcomes. OBJECTIVES: The authors of this study present a novel surgical approach designed to enhance NAC vascularization by performing a nipple-sparing skin-reducing mastectomy exclusively through the lower lateral triangle of a Wise-pattern incision with immediate prepectoral polyurethane-coated breast …

Nipple-Preserving Skin-Reducing Mastectomy in Ptotic Breasts: A Systematic Review and Meta-analysis.

BACKGROUND: Skin-reducing mastectomy (SRM) represents a surgical option that provides patients with ptotic breasts the advantage of undergoing simultaneous nipple-preserving mastopexy in conjunction with mastectomy. Nonetheless, existing data, primarily derived from single-center studies, are characterized by limited patient cohorts. Therefore, this study examines the complications associated with SRM. METHODS: We conducted a systematic review of MEDLINE and Embase to evaluate outcomes of skin-reducing mastectomies, following PRISMA guidelines. Search terms included "cancer," "mastectomy," "m…

Non-contrast MRI and post-mastectomy silicone breast implant rupture: preventing false positive diagnoses.

BACKGROUND: Breast implants are not lifelong, with implant rupture being the third leading cause of revisional surgery in augmented women. Noncontrast MRI is a reliable tool to assess implant integrity; however, false positive and false negative diagnoses have been reported due to an incorrect interpretation of MRI signs. This study aims to investigate the incidence of these misleading results, comparing MRI findings with intraoperative surgical observations and exploring signs of nonunivocal interpretation. MATERIALS AND METHODS: Between March 2019 and October 2022, our hospital, a referral

Defining a danger zone for iatrogenic long thoracic nerve injury in gender-affirming mastectomy.

The long thoracic nerve's (LTN) superficial location on the chest wall renders it vulnerable to iatrogenic injury. Plastic surgeons' gender-affirming mastectomy volumes are rapidly increasing. This operation involves lateral chest contouring placing the distal LTN at risk of injury along the chest wall. Although proximal LTN injury can cause debilitating shoulder dysfunction, more distal injury can cause chronic postoperative shoulder pain and dysfunction without frank scapular winging, making diagnosis and treatment difficult. Therefore, injury is best avoided. In this study, the course of th…

Fibrin Glue Sealant: An Effective Modality to Mitigate Postoperative Seroma After Modified Radical Mastectomy.

Introduction Modified radical mastectomy is a common modality used in treating breast cancer and often presents a series of post-operative challenges. Seroma formation is the most common complication with no single method shown to be reliably effective against it. Fibrin glue is one such modality. There is a paucity of studies describing the utility of fibrin glue to prevent seroma formation. Aim To evaluate the effect of intraoperative application of fibrin glue sealant on drain output and seroma formation in patients undergoing modified radical mastectomy by comparing outcomes between the tw…

Methods for quantifying breast cancer-related lymphedema in patients undergoing a contralateral prophylactic mastectomy.

Patients treated for breast cancer are at risk of developing breast cancer-related lymphedema (BCRL). A significant proportion of patients treated for breast cancer are opting to undergo a contralateral prophylactic mastectomy (CPM). Currently, it remains unclear as to whether the relative volume change (RVC) equation may be used as an alternative to the weight adjusted change (WAC) equation to quantify BCRL in patients who undergo CPM. In order to simplify BCRL screening, our cohort of patients who underwent a CPM (n=310) was matched by BMI to a subset of patients who underwent unilateral bre…

Understanding Preoperative Breast Satisfaction among Patients Undergoing Mastectomy and Immediate Reconstruction: BREAST-Q Insights.

BACKGROUND: This study aims to present normative values for satisfaction with breasts among preoperative breast reconstruction patients as assessed using the BREAST-Q instrument and to delineate factors associated with preoperative breast satisfaction. METHODS: A retrospective analysis of prospectively collected data was performed examining women undergoing postmastectomy breast reconstruction at a tertiary care center who preoperatively completed the BREAST-Q from 2010 to 2017. Because breast satisfaction scores were nonnormally distributed, scores were categorized into quartiles for analysi…

Preoperative multimodal protocol reduced postoperative nausea and vomiting in patients undergoing mastectomy with reconstruction.

BACKGROUND: Mastectomy with immediate reconstruction is a high-risk cohort for postoperative nausea and vomiting (PONV). Known risk factors for PONV include female gender, prior PONV history, nonsmoker, age < 50, and postoperative opioid exposure. The objective of this observational, cohort analysis was to determine whether a standardized preoperative protocol with nonopioid and anti-nausea multimodal medications would reduce the odds of PONV. METHODS: After IRB approval, retrospective data were collected for patients undergoing mastectomy with or without a nodal resection, and immediate subp…

A Targeted Approach to Post-Mastectomy Pain and Persistent Pain following Breast Cancer Treatment.

Persistent pain following treatment for breast cancer is common and often imprecisely labeled as post-mastectomy pain syndrome (PMPS). PMPS is a disorder with multiple potential underlying causes including intercostobrachial nerve injury, intercostal neuromas, phantom breast pain, and pectoralis minor syndrome. Adding further complexity to the issue are various musculoskeletal pain syndromes including cervical radiculopathy, shoulder impingement syndrome, frozen shoulder, and myofascial pain that may occur concurrently and at times overlap with PMPS. These overlapping pain syndromes may be dif…

Is Sentinel Lymph Node Biopsy Necessary for Ductal Carcinoma In Situ Patients Undergoing Mastectomy?

BACKGROUND: Current treatment guidelines for ductal carcinoma in situ (DCIS) treated with mastectomy recommend sentinel lymph node biopsy (SLNB). In the modern era, there is a trend toward minimizing invasive staging and treatment of the axilla. In this study, we seek to determine the role of SLNB in patients undergoing mastectomy for the treatment of DCIS. METHODS: Patients undergoing mastectomy were identified from our institution's SLNB database from 2012 to 2016. Patients were included if core needle biopsy demonstrated DCIS. Patient demographics, tumor characteristics, and pathologic var…

Invited Discussion on: Skin Reducing Mastectomy and Prepectoral Breast Reconstruction in Large Ptotic Breasts.

The applicability of acellular dermal matrix (ADM) for breast reconstruction is a consolidated reality, as skin and nipple sparing techniques became standard mastectomy approaches. ADM is a soft connective tissue graft generated via a decellularization process that preserves intact the extracellular skin matrix. ADM not only provides tissue reinforcement, but also better pocket control, and shape without the compressive effects of total sub-muscular coverage. Our preference is using one "Strattice®" ADM in pocket's format to cover the implant's inferior pole, protecting the totality of the imp…

Persistent nipple discharge after nipple-sparing mastectomy secondary to unincorporated AlloDerm: a case report.

BACKGROUND: Rates of nipple-sparing mastectomies have increased over the past decade. In 2017, acellular dermal matrix was used in 56% of breast reconstructive procedures, with complication rates similar to operations without AlloDerm. Although persistent nipple discharge after nipple-sparing mastectomy is a rare event, it has been described in the literature. Other authors have described evaluation and treatment on a case-by-case basis. To the best of our knowledge, this is the first case report to describe a persistent unilateral discharge after multiple operative revisions and to provide an…
