Recurrent Mastoiditis Mimics IgG4 Related Disease: A Potential Diagnostic Pitfall.

IgG4-related disease (IgG4-RD) is a recently recognized entity that causes progressive fibrosis and formation of mass lesions. IgG4-RD can be diagnosed histologically by its hallmarks of storiform fibrosis, prominent lymphoplasmacytic infiltrate, and obliterative phlebitis, accompanied by the infiltration of excessive numbers of IgG4-positive plasma cells as well as elevations in serum IgG4 concentrations. A recent publication reported a case of IgG4-RD in the mastoid sinus, representing a new anatomic location for this disease. We report two additional cases of IgG4-RD occurring in the mastoi…

[Peculiar features of mastoiditis in a brest-fed infant with the "exposed" facial nerve].

This paper reports the clinical case of mastoiditis in a 5-month old child in whom an unusual localization of the totally "naked" facial nerve outside of the bone canal in the mastoid part was discovered intraoperatively. This finding was quite unexpected because nerves are not visible on CT scanograms. The author emphasizes that the clinical course of otitis media in the breast- fed infants and young children is characterized by a number of peculiarities due to specific anatomical, physiological, and immunological features of the child's organism. She also notes that the number of antromastoi…

Acute mastoiditis in children: a 10-year retrospective and validated multicenter study.

BACKGROUND: Conservative treatment of acute otitis media may lead to more complications. This study evaluates changes in incidence, the clinical and microbiological findings, the complications and the outcome of acute mastoiditis in children in a country employing conservative guidelines in treating acute otitis media. METHODS: All admitted children (0-15 years) diagnosed with acute mastoiditis during the period from 1998 to 2007 in eastern Denmark (population 2.2 million) were identified. Patient files were retrieved and reviewed; the data were entered into a database. RESULTS: The average …

Mastoiditis secondary to metastatic lung carcinoma: case report and literature review.

OBJECTIVE: We present a case report and systematic review of acute mastoiditis caused by metastatic lung cancer. CASE REPORT: A 62-year-old woman developed acute mastoiditis as a complication of otitis media. Cortical mastoidectomy revealed deposits of metastatic non-small cell lung carcinoma around the sigmoid sinus. The patient had previously received treatment for lung cancer, but was thought to be in remission. DISCUSSION: A literature review confirmed that this is the first reported case of mastoiditis caused by metastatic lung cancer. Only four similar case reports were identified: two…

Do patients with sclerotic mastoids require aeration to improve success of tympanoplasty?

CONCLUSION: We could not find any significant difference in the results for graft success rate and functional hearing results between the myringoplasty and tympano-mastoidectomy groups. So mastoidectomy may not be necessary for successful tympanic membrane reconstruction and hearing improvement. OBJECTIVE: To investigate the effect of aerating mastoidectomy on the surgical success rate of myringoplasty. METHODS: This was a retrospective study. Data were analyzed from 92 patients who underwent surgical repair of tympanic membrane perforations due to chronic suppurative otitis media (CSOM) wit…

Otitic hydrocephalus associated with lateral sinus thrombosis and acute mastoiditis in children.

The incidence of intracranial complications of acute otitis media (AOM) has decreased and the need for operative and medical treatment is declined during the antibiotic era. To describe pathognomonic signs, evaluation management, operative findings, clinical course and outcome of otitic hydrocephalus and lateral sinus thrombosis as complications of AOM and mastoiditis in pediatric patients. Two children, 9 and 13 years old, with the diagnosis of OH and TK and MRI findings are presented. Intracranial complications in children resulted from unsuccessful treatment of AOM, which led to acute masto…

Penicillin-susceptible and erythromycin-resistant Streptococcus pneumoniae in children with acute mastoiditis.

Resistance to erythromycin is usually associated with resistance to penicillin in Streptococcus pneumoniae isolates. Over the last few years, however, an increase has been detected in the number of Streptococcus pneumoniae isolates obtained from children with acute otitis media complicated by acute mastoiditis. These isolates are characterized by a penicillin-susceptible and erythromycin-resistant antibiotic profile. This observation prompted a review of the microbiology database and analysis of all Streptococcus pneumoniae isolates with this peculiar antibiotic profile at a university hospita…

[Acute mastoiditis in infants: penicillin-resistant Streptococcus pneumoniae and its therapy].

Seven patients with acute mastoiditis are reported. They were treated at Chiba Children's Hospital from 1991 to 1996. Patients included one 4-year-old child and 6 one-year-old infants. Except the 4-year-old girl whose body weight was 17 kg, all infants body weights were under 11 kgs. Chief complaints were ear discharge, retroauricular swelling and high fever (> 38 degrees C). Streptococcus pneumoniae was isolated in the 6 infants. One patient was diagnosed with penicillin-sensitive Streptococcus pneumoniae (PSSP), three patients were diagnosed with penicillin-insensitive Streptococcus pneumoni…

Granuloma inguinale (donovanosis): an unusual cause of otitis media and mastoiditis in children.

Granuloma inguinale (donovanosis) is seen predominantly in adults (it rarely occurs in children) and mainly affects genital skin and mucosa. Infection occurs at other skin and mucosal sites, and hematogenous dissemination to bone also has been described. The infection responds dramatically to appropriate antibiotic treatment. We present two cases of granuloma inguinale occurring in children (8 months and 5 months of age) causing mastoiditis and external ear discharges. A temporal lobe abscess also developed in the 8-month-old child. Subsequent computed tomography scans showed marked improvemen…

[Tuberculous otitis media, mastoiditis associated with meningitis: report of one case].

Tuberculosis of the middle ear is rare; associated meningitis is even more unusual. This report contains our experience with a one and a half year old psychomotor retarded boy. He had a poor healing left postauricular abscess four months prior to admission. Also noted was postprandial vomiting and left sided involuntary movement. On admission spinal tapping was done. Results showed leukocytosis, with lymphocytes being predominant, as well as high protein and low glucose levels. A cranial CT revealed left mastoiditis, hydrocephalus, basal cistern abnormal enhancement and a prominent posterior f…

Acute mastoiditis complicated by sigmoid sinus thrombosis in congenital aural atresia.

Congenital aural atresia occurs approximately once in every 20,000 live births, although the incidence of otitis media in aural atretic children should not differ from the rest of the population, detection is difficult. Unless severe, the infection usually goes unnoticed, especially with the use of antibiotics. A two-year-old male with congenital aural atresia presented with fever and facial nerve paresis. As his illness progressed, he developed mastoiditis with subperiosteal abscess and sigmoid sinus thrombosis. Literature review showed this to be the first reported case of sigmoid sinus thro…
