Atrophic maxillary floor augmentation by mineralized human bone allograft in sinuses of different size: an histologic and histomorphometric analysis.

OBJECTIVE: The aims of this work were to histologically examine the healing of mineralized human bone allograft (MHBA) in sinus augmentation for elevating a severe maxillary atrophy ridge (≤2 mm residual ridge height) and to correlate the results to the sinus cavity size. MATERIAL AND METHODS: A two-stage protocol was conducted in 23 patients, all having crestal bone ≤2 mm. A mixture of 80/20 cortical/cancellous of MHBA particles was used to augment sinus using the lateral window approach in narrow (NS; <15 mm bucco-palatal distance) and wide (WS; ≥15 mm bucco-palatal distance) sinuses, based

Three-year analysis of tapered screw vent implants placed into maxillary sinuses grafted with mineralized bone allograft.

With an increase in the number of patients presenting for dental implant treatment, it is becoming more common for clinicians to encounter inadequate bone volume. Several surgical techniques have been advocated for placing dental implants in the posterior maxilla, including the lateral window sinus elevation technique. This article reports the results of implants placed into maxillary sinuses grafted with particulate mineralized cancellous bone allograft alone or in combination with resorbable hydroxyapatite over a 3-year period. A total of 56 sinuses were grafted, and 136 dental implants were

Macrolide immunomodulatory effects and symptom resolution in acute exacerbation of chronic bronchitis and acute maxillary sinusitis: a focus on clarithromycin.

Bacterial respiratory tract infections are common in the primary care setting and patients often seek the assistance of a healthcare professional in order to achieve resolution of their symptoms. Antibiotic agents that offer rapid symptom relief, in addition to excellent bacteriological and clinical cure, are highly desired. Macrolides have proven to be highly effective in treating acute bacterial exacerbations of chronic bronchitis and acute maxillary sinusitis. In addition, immunomodulatory effects that may contribute to symptom resolution have been reported. This article reviews current lit…

Bone grafting to the maxillary sinuses, nasal floor and anterior maxilla in the atrophic edentulous maxilla. A two-stage technique.

This study presents the results from 20 consecutive patients treated with an autogenous bone graft from the iliac crest. In ten patients the graft was placed in the maxillary sinuses and the floor of the nose (inlay group). Ten patients, in addition to the inlay graft, had a corticocancellous bone block secured with mini-screws to the anterior maxillary ridge (inlay/onlay group). Endosteal implants (Brånemark) were placed six months after surgery. A total of 136 implants were placed, of which eight failed to integrate during the six-month healing period. A further 15 implants were lost during

Localization of IL-1 beta mRNA and cell adhesion molecules in the maxillary sinus mucosa of patients with chronic sinusitis.

Interleukin-1 beta (IL-1 beta) is a predominant cytokine in retained paranasal sinus fluid of chronic sinusitis where infiltration by polymorphonuclear neutrophils (PMNs) of nasal and paranasal mucosa is characteristic. The authors investigated the localization of IL-1 beta messenger RNA (mRNA) in the maxillary sinus mucosa of patients with chronic sinusitis, using digoxigenin-labeled oligonucleotide probes. IL-1 beta mRNA was detected in some extravascular PMNs and small numbers of mononuclear leukocytes but was not detected in other tissue cells or intravascular leukocytes. The expression an

The efficacy of oxymetazoline administered with a nasal bellows container and combined with oral phenoxymethyl-penicillin in the treatment of acute maxillary sinusitis.

The efficacy of a new administration form of oxymetazoline, a nasal bellows container, was investigated in two separate studies by means of a combined treatment with phenoxymethyl-penicillin for acute maxillary sinusitis. In the first study (study 1), oxymetazoline administered with a bellows (OXBE) was compared both with a placebo belows (PLBE) as well as with oxymetazoline and placebo administered with a conventional nasal spray (OXSP respective PLSP) in 73 patients. In the second study (study 2), OXBE was compared only with PLBE in 48 patients. Objective evaluation was made by comparing the…

Infective Endocarditis With Origin in Orbital Vascular Malformation and Maxillary Sinusitis: A Case Report and Review of Four Patients in the Literature.

Infective endocarditis is a life-threatening disease and the early diagnosis is crucial for a better outcome. We report an old adult who developed infective endocarditis in association with new-onset maxillary sinusitis as well as proptosis, which was caused by an orbital mass lesion in the background of pre-existing orbital vascular malformation. A 74-year-old woman was found incidentally to have right orbital vascular (venous) malformation by head magnetic resonance imaging when she was hospitalized for left dorsal pontine infarction. No paranasal sinusitis was noted at that time. She was we…

Effects of Platelet-Rich Fibrin on Bone Healing Around Implants Placed in Maxillary Sinuses: A Histomorphometric Assessment in Rabbits.

This study investigated the effect of platelet-rich fibrin (PRF) on bone healing around implants placed in elevated sinus cavities. Forty New Zealand albino rabbits were divided into eight groups, based on the time of sacrifice (14 or 40 days) and the material used: blood clot (control), hydroxyapatite (HA) from bovine bone, HA combined with PRF, and PRF alone. Each group consisted of five animals (n = 5). A histological analysis measured bone-to-implant contact (BIC) and bone area fraction occupancy (BAFO). The results showed significant increases in the BIC and BAFO values at 40 days compare

Maxillary sinuses, multiple aspects to consider prior to bone grafting procedure: a retrospective study in a sample of a Palestinian population.

OBJECTIVES: To analyze various anatomical aspects of the maxillary sinuses on CBCT volumes, mainly, the posterior superior alveolar artery (PSAA), and to compare its visibility on CBCT vs. panoramic radiographs. MATERIAL AND METHODS: A retrospective radiographic analysis was conducted on 120 patients (240 maxillary sinuses) using their CBCT coronal views (CCV) and panoramic radiographs (CP). On CCV, the PSAA's maximum dimension (MD), vertical distance (PVD) to the alveolar crest, and bone plate thickness (T1 and T2) buccally and inferiorly to the PSAA, respectively, were analyzed. Additionall

A Successful Treatment Regimen for the Prevention of Sinusitis after Maxillary Sinus Floor Elevation Surgery in a High-Risk Case.

Maxillary sinus floor elevation (sinus lift) is a widely recognized dental-surgical approach for dental implant placement. However, for an otorhinolaryngological high-risk patient with severe anatomic-structural impairments of the maxillary sinus drainage pathway, surgical intervention is recommended before sinus lift to avoid postsinus lift maxillary sinusitis. Here, we show a case that postsinus lift maxillary sinusitis in such a high-risk patient was noninvasively prevented by the collaboration of otorhinolaryngologist and dentist. A 48-year-old Japanese male intended to undergo a sinus lif…

Factors influencing the prevalence of peri-implantitis in implants inserted in augmented maxillary sinuses: A multicenter cross-sectional study.

BACKGROUND: Peri-implantitis is widely recognized as a major cause of late implant failure, both in pristine and regenerated bone. The present study aims to evaluate the prevalence of peri-implantitis in implants inserted in augmented maxillary sinuses and to analyze possible risk factors. METHODS: A cross-sectional study was conducted in four centers including patients who underwent lateral or transcrestal sinus augmentation and received dental implants. Clinical and anamnestic data were collected using a standardized form. Univariate and multivariate logistic regression analyses have been p

Surgical management of persistent maxillary sinusitis with subtle residual symptoms following lateral sinus floor elevation: a report of three cases.

Postoperative graft infection and subsequent acute maxillary sinusitis are infrequent complications following sinus floor elevation (SFE). However, inadequate management may result in persistent infection with only subtle residual symptoms. This case study presents three patients who developed persistent subclinical graft-related infection following SFE. Patient 1 underwent staged implant placement (two implants) after lateral SFE, whereas the other two underwent simultaneous implant placement and SFE (four implants in Patient 2, two implants in Patient 3). Between 2 and 3 weeks postoperativel…

Characteristics and risk factors in odontogenic maxillary sinusitis from different dental infections: a retrospective study based on sinus CT imaging.

BACKGROUND: Odontogenic maxillary sinusitis is a common type of maxillary sinusitis, which is secondary to adjacent infectious maxillary dental lesions. However, the prevalence and the different odontogenic risk factors that influence the pathogenesis of odontogenic maxillary sinusitis are not clear. Thus, odontogenic maxillary sinusitis is often overlooked and facing great difficulties of multi-disciplinary treatment decisions for odontogenic maxillary sinusitis. The objective of this study was to analyze the different dental potential risk factors of odontogenic maxillary sinusitis and optim…
