Comparison of costs of surgical site infection and endometritis after cesarean delivery using claims and medical record data.

We used administrative and clinical data from a case-control study to calculate the costs of surgical site infection and endometritis after cesarean delivery. Attributable costs determined by multivariate generalized least-squares regression models with the 2 data sets were similar, suggesting that administrative data can be used to calculate infection costs.

An update on advance directives in the medical record: findings from 1186 consecutive patients with unresectable exocrine pancreas cancer.

BACKGROUND: The Terri Schiavo case and other recent events underscore the importance of advance directives. Yet, in the past, only a small subgroup has utilized them. This study from a large tertiary medical center was undertaken to assess current rates of advance directives among patients with incurable pancreas cancer. METHODS/RESULTS: The medical records of 1,186 consecutive patients with unresectable pancreas cancer were reviewed over a 4-year span. Only 174 patients (15%) had an advance directive in the medical record. Older age and having cancer therapy at our institution were associate…

Can data from an electronic medical record identify which patients with pneumonia have Pneumocystis carinii Infection?

BACKGROUND: Pneumocystis carinii is the leading opportunistic pulmonary infection in HIV-infected patients. Invasive diagnostic procedures m … s (37%). Five of the seven significant independent predictors of PCP came from patients' electronic medical records: infiltrate on chest radiograph, male gender, lower red cell distribution width, lower serum creatinine, and a prior positive HIV test. The other two (duration of illness and presence of dyspnea) came from the emergency department record. A simple i …

Antenatal risk factors for childhood diabetes mellitus; a case-control study of medical record data in Yorkshire, UK.

Environmental risk factors for childhood insulin-dependent diabetes mellitus (IDDM) have been investigated using data abstracted from the obstetric records of mothers participating in a population-based case-control study of children (0-15 years) diagnosed with IDDM during 1993-1994. A univariate analysis of 196 age and sex matched sets (129 triplets, 67 pairs) gave significantly raised odds ratios (OR) for mothers over 35 years (OR 2.13, 95%CI 1.04-4.36) and the following exposures in pregnancy: amniocentesis (3.85, 1.34-11.04), oedema, proteinuria and/or hypertensive disorders (1.62, 1.03-2.

How to Evaluate Hospital Care in the Dying Phase-Development of a Data Extraction Tool for Retrospective Medical Record Analysis.

BACKGROUND: Hospitals are the most common place of death in European countries, including Germany, where nearly half of the population dies in hospitals, mostly outside specialised palliative care wards. At the same time, quality of hospital care in the dying phase is reported as poor. Although existing (inter-)national guidelines provide outcome variables, their evaluation of implementation is lacking. This study aims to develop and test a structured tool for data extraction from medical records (MRs) to evaluate hospital care in the dying phase. The provision of such a tool can help to ident…

A cluster randomized controlled trial of an electronic medical record-based pathway for pediatric asthma in primary care in Alberta.

BACKGROUND: Primary care physicians, who manage the care of most children with asthma, often do not optimally assess disease control, prescribe asthma controller medications, or provide family asthma education. We developed a pediatric asthma clinical pathway embedded in an electronic medical record (EMR) for use by primary care practices, and we sought to evaluate its effects on prescription and use of asthma controller medication for children with asthma. METHODS: We conducted a cluster randomized controlled trial, enrolling primary care practices in Alberta that used Wolf or Med Access EMR

Evaluating the accuracy and efficacy of an electronic medical record alert to identify paediatric patients with low-risk febrile neutropenia.

BACKGROUND: Point-of-care decision support, embedded into electronic medical record (EMR) workflows, has the potential to improve efficiency, reduce unwarranted variation and improve patient outcomes. A clinical-facing best practice advisory (BPA) in the Epic EMR system was developed to identify children admitted with low-risk febrile neutropenia (FN) who should be considered for treatment at home after a brief inpatient stay. We evaluated the accuracy and impact of this BPA and identify areas for improvement. METHODS: The low-risk FN BPA was co-designed with key-stakeholders and implemented

The electronic medical record management systems may improve monitoring and control of disease activity in patients with ankylosing spondylitis.

To investigate the impact of an electronic medical record management system (EMRMS) on disease activity and the frequency of outpatient visits among patients with ankylosing spondylitis (AS). We identified 652 patients with AS who were followed up for at least 1 year before and after the first Ankylosing Spondylitis Disease Activity Score (ASDAS) assessment and compared the number of outpatient visits and average visit time within 1 year before and after the initial ASDAS assessment. Finally, we analyzed 201 patients with AS who had complete data and received ≥ 3 continuous ASDAS assessments a

Prevalence and association of early onset severe hyperbilirubinemia in newborn in the East China region: Retrospective medical record analyses.

Research on the prevalence and association of hyperbilirubinemia is controversial because of different cultures, demographics, and clinical conditions. The etiology of hyperbilirubinemia is affected by the environment and other factors in the newborn. The World Health Organization recommended a 1-day hospital stay after uncomplicated delivery, jaundice assessment before discharge, and screening on 3rd and 7th days after birth for hyperbilirubinemia. However, the implementation of these recommendations is difficult in China. The objective of this study was to evaluate the prevalence and associa

Improving Hepatitis B Screening at Family Health Centers Using Quality Improvement and Electronic Medical Record Tools.

Chronic hepatitis B viral (HBV) infection is associated with significant morbidity and mortality with endemic areas carrying most of the global burden of HBV disease. Current HBV screening rates in the United States are suboptimal. We aimed to improve HBV screening rates at regional family health centers serving high-risk refugee populations by 20% over 2 years. We used quality improvement (QI) methodology and implemented interventions providing electronic medical record (EMR)-enabled HBV screening tools within known clinical workflows. EMR tools captured country-of-origin data to identify per
