Access to Primary Care and Nephrology: Implications for Preemptive Listing and Kidney Transplantation, A National Registry Study.

BACKGROUND: Care coordination between primary care providers and nephrologists is crucial for preemptive kidney transplantation (KT), which confers health advantages over KT after dialysis. Residence in areas with limited primary care (Medically Underserved Areas [MUAs]/Health Professional Shortage Areas [HPSAs]) and nephrology access may differentially affect preemptive listing/KT. OBJECTIVE: To quantify access to preemptive KT by residence in limited primary care/nephrology access areas. DESIGN: Retrospective cohort study from the US national registry. PARTICIPANTS: A total of 353,636 adu…

Determinants of uptake of deworming medication during pregnancy in Tanzania: insights from 2022 demographic and health survey.

BACKGROUND: Anemia during pregnancy increases delivery risks, with untreated parasitic infections being a major cause. In Tanzania, evidence on deworming uptake among pregnant women aged 15-49 is limited. This study assessed national uptake and determinants of deworming medication use in this group. METHODS: We analyzed secondary data from the 2022 Tanzania Demographic and Health Survey (TDHS-2022), using the Pregnancy Outcome and Care for Pregnancies (NR) file. The study population consisted of pregnant women aged 15-49 years who had a live birth or stillbirth in the three years preceding th

Implementation of a Model Program at an Independent Community Oncology Practice for the Outpatient Administration of B-Cell Maturation Antigen-Directed Bispecific Antibody Step-Up Doses.

Regulatory approval of B-cell maturation antigen (BCMA)-CD3 bispecific antibodies (BsAbs) has revolutionized the treatment of patients with heavily pretreated relapsed or refractory multiple myeloma. Label-recommended premedications and step-up dose (SUD) regimens are used to mitigate the incidence and severity of serious adverse events such as cytokine release syndrome and immune effector cell-associated neurotoxicity syndrome. Hospitalization of patients during the SUD regimen is also recommended by the manufacturers to facilitate the rapid therapeutic management of adverse events that may a

Severe childhood asthma in low and middle-income countries.

Although severe asthma in low- and middle-income countries (LMICs) is relatively uncommon in children and adolescents (2.1 % and 4.3 %, respectively), it results in significant morbidity, occasional fatality, and great challenges of management, compared to high-income countries. Additionally, nearly 6 % of infants in LMICs experience nighttime symptoms on a weekly basis. Socioeconomic inequities and exposure to environmental risk factors contributed to variability in prevalence, with children from disadvantaged backgrounds being at a higher risk of developing the condition. Numerous preventabl

A Multi-faceted Approach to Improving Breast Cancer Outcomes in a Rural Population, and the Potential Impact of Patient Navigation.

UNLABELLED: The mastectomy rate in rural areas of the Northern Plains of the U.S. was 64 percent from 2000 through 2005. We implemented a breast cancer patient navigation (BPN) program in May 2007 to increase breast conservation (BC) rates. METHODS: We analyzed mastectomy and BC rates among our 1,466 patients with either ductal carcinoma in situ (DCIS) or stage I/II invasive breast cancer treated from 2000 through 2012. We used interrupted time series (ITS) to compare rates in treatment following implementation of BPN. In addition, breast conservation rates were compared to population data fr

Trends and disparities in anencephaly-related early childhood mortality in the United States between 1999 and 2020: A nationwide CDC WONDER analysis.

Anencephaly is a severe neural tube defect that affects approximately 1 in every 5250 live births in the United States. This fatal congenital condition is linked to modifiable risk factors, including folic acid deficiency, maternal diabetes, and exposure to teratogenic medications such as certain anti-epileptics. Despite public health efforts to promote folic acid supplementation, disparities in mortality persist. This study aimed to evaluate national trends in anencephaly-related early childhood mortality from 1999 to 2020 using the CDC Wide-Ranging Online Data for Epidemiologic Research (CDC

Antenatal transfer of rural women to perinatal centres. High Risk Obstetric and Perinatal Advisory Working Group.

The aim of this study was to describe antenatal transfers of rural women to perinatal centres, and among transferred women, to assess the use of selected evidence-based therapies and determine the predictors of preterm and imminent births. The clinical records of rural women antenatally transferred to perinatal centres in NSW and the ACT during 1997-1998 were reviewed. Of 453 rural antenatal transfers, 408 (90%) were emergency transfers. Increasing remoteness was associated with increased rates of antenatal transfer but not with a lower probability of giving birth. Of all transferred women, 64
