[Recurrent otitis media with effusion in childhood : when should an otolaryngologist consider an allergic etiology?].

Up to 80 % of children can develop otitis media with effusion (OME) between birth and school age. Responsible are longstanding impairments of tubal ventilation which are based primarily on mechanical or functional obstructions. A quarter of the subjects affected by OME show either recurrent episodes over 3 months or protracted clinical courses and in these children an extended diagnosis is required. Besides infection-related adenoid hypertrophy, the differential diagnosis should include ciliary dysfunction, chronic rhinosinusitis, craniofacial malformations, gastroesophageal reflux, tumors and

Experimental evidence against middle ear oxygen absorption.

The present theory of eustachian tube (ET) function and middle ear (ME) ventilation posits that oxygen absorbed by the ME mucosa causes negative ME pressure which is relieved by periodic opening of the ET during swallowing and yawning. After developing a method to cannulate the ET of mongrel dogs we connected the cannulas hermetically to manometers. This system excluded ET function and tested the oxygen absorption capacity of the ME. When we controlled respiration and maintained blood gas PO2 and PCO2 at normal levels, we were unable to find any manometric evidence of negative pressure of gas

Impact of palatoplasty techniques on tympanic membrane findings and hearing prognosis in children with cleft palate.

OBJECTIVE: Children with cleft palate (CP) are at high risk of developing otitis media with effusion (OME) due to Eustachian tube (ET) dysfunction. Palatoplasty has been reported to decrease the frequency of middle ear disease and improve ET function, and although various techniques have been developed, there is no consensus on the differences in the impact of different techniques on the middle ear. The purpose of this study was to determine the differential effects of palatoplasty on middle ear function and hearing. METHODS: We performed a retrospective observational survey of pediatric pati

Oxymetazoline vs ofloxacin vs ciprofloxacin/dexamethasone- effects of drops on tympanostomy tube postoperative otorrhea.

OBJECTIVE: To assess for differences in postoperative otorrhea rates after tympanostomy with tube placement surgery comparing use of oxymetazoline, ofloxacin, or ciprofloxacin/dexamethasone drops prescribed in the postoperative period. METHODS: A retrospective review was conducted of 516 pediatric patients who had either bilateral or unilateral myringotomy with tube placement performed during the year 2018. Information collected from each surgery included whether there was effusion at time of surgery, type of effusion, whether an adenoidectomy was performed the same time or prior, prior histo

Exploring the Factor Structure of the Glasgow Children's Benefit Inventory: New Recommendations for Reporting Results.

OBJECTIVE: Patient-reported outcomes can be useful for reporting benefit from non-life-saving interventions, but often they report a single overall score, which means that much information on the specific areas of benefit is lost. Our aim was to perform a new factor analysis on the Glasgow Children's Benefit Inventory (GCBI) to create subscales reflecting domains of benefit. Further aims were to assess the internal consistency of the GCBI, and to develop guidelines for reporting both a total score and sub-scales in future studies. METHODS: We collected 4 existing datasets of GCBI data from ch

The differential diagnosis of hearing loss.

BACKGROUND: According to the World Health Organization, hearing loss is one of the six leading contributors to the global burden of disease. It is becoming an ever more important problem in society at large, not just because the population is aging, but also because young people increasingly spend their leisure time in activities that expose them to excessive noise. On the other hand, the treatment of hearing loss is improving, as the result of technical developments in otological surgery, hearing aids, and cochlear implants. For nearly every type of hearing loss, there is now some type of reh

Managing Chronic otitis media with Effusion in Children with non-Syndromic Cleft Palate: Short-Term Ventilation Tubes Versus Surveillance.

OBJECTIVE: To compare the effectiveness of short-term ventilation tubes compared to surveillance on conductive hearing loss in children with non-syndromic orofacial clefting involving the muscular palate. INTRODUCTION: Chronic otitis media with effusion is a common finding in children with cleft palate. The accepted convention is insertion of short-term ventilation tubes at the time of palate repair, but some centres are choosing conservative management. Each approach has its advantages but there is currently no consensus on the most appropriate management in children with non-syndromic cleft

Pseudomonal Biofilm Topographic Distribution on Tympanostomy Tubes: An In Vitro Model.

In vitro growth of Pseudomonas aeruginosa biofilm on tympanostomy tubes showed that in Armstrong T-tubes, biofilm colonies were mainly located in the perpendicular junction between the body and the flanges, but in Paparella-type tube, they concentrated on the round rims. These "weakness" zones can be the future target areas for geometry changes, and can be specifically coated with antibiofilm materials.

[First experience with the use of tympanostomy for the management of acute otitis media in children].

The present study included 140 children presenting with acute suppurative otitis media (AOM) who received the standard treatment in combination with paracentesis. In 14% of the patients (28 ears), paracentesis had to be repeated, sometimes in triplicate or even oftener. Seven children with AOM were treated with the use of tympanostomy. This method makes it possible to avoid repetition of paracentesis, ensures faster evacuation of the contents of the tympanic cavity, and reduces the duration of hospitalization; moreover, it prevents the development of exudative otitis media and accelerates the
