Pulmonary Function Testing and Upper Airway Disorders: Analysis From the STAMPEDE III Database.

INTRODUCTION: Upper airway disorders such as vocal cord dysfunction (VCD) may present in a similar manner to asthma. Identifying and differentiating between underlying asthma and VCD may significantly affect treatment options. This study reanalyzed the comprehensive pulmonary function data obtained from the STAMPEDE III study to compare findings in patients diagnosed with either asthma, airway hyperreactivity, and VCD. MATERIALS AND METHODS: The STudy of Active duty Military for Pulmonary Disease Related to Environmental Deployment Exposures (STAMPEDE) III was a prospective, observational stu

Novel Musculoskeletal Hypotheses in the Armed Services Trauma and Rehabilitation Outcome (ADVANCE) Cohort: Development and Application of Sparse Group Factor Analysis Methodology.

BACKGROUND: Musculoskeletal conditions are a leading global cause of disability, yet the factors influencing long-term musculoskeletal health, particularly following trauma, remain incompletely understood. Machine learning could be applied to identify previously unknown patterns in large-scale, multimodal datasets. OBJECTIVE: This study aims to test the ability of a new sparse group factor analysis method to uncover hidden patterns in large-scale multimodal datasets and generate testable, clinically relevant hypotheses. METHODS: This study applies sparse group factor analysis, a hierarchical

Exploring the Therapeutic Effects of Psychedelics Administered to Military Veterans in Naturalistic Retreat Settings.

BACKGROUND: Military veterans are at risk of various mental health conditions, with profound implications for post-deployment quality of life. Current treatment options encounter high dropout rates and non-responsiveness, and overlook the importance of community building in veterans' holistic recovery. Preliminary research suggests psychedelics offer therapeutic benefits for depression and post-traumatic stress disorder (PTSD) in veterans. Integrating psychedelic therapies with a communal retreat setting could provide a more holistic framework for improving veterans' well-being. OBJECTIVES: T

Deployment after limb salvage for high-energy lower-extremity trauma.

BACKGROUND: Many wounded warriors experienced high-energy lower-extremity trauma (HELET) that may be limb threatening. Volumetric muscle loss, posttraumatic osteoarthritis, nerve injuries, and pain may severely limit physical function. Several wounded warriors express a strong desire to return to their units and be deployed in their original military occupational specialty. We began the return-to-run (RTR) clinical pathway at our institution 2 years ago to facilitate high-performance goals such as these. It involves an energy storing ankle foot orthosis, the intrepid dynamic exoskeletal orthos

Risk factors for post-traumatic stress disorder among UK Armed Forces personnel.

BACKGROUND: There is considerable interest in understanding further the factors that increase the risk of post-traumatic stress disorder (PTSD) for military personnel. This study aimed to investigate the relative contribution of demographic variables; childhood adversity; the nature of exposure to traumatic events during deployment; appraisal of these experiences; and home-coming experiences in relation to the prevalence of PTSD 'caseness' as measured by a score of 50 on the PTSD Checklist (PCL) in UK Armed Forces personnel who have been deployed in Iraq since 2003. METHOD: Data were drawn fr

Renal Replacement Therapy Capability for the Treatment of Combat-Associated Acute Kidney Injury: A Historical Perspective to Plan for Future Conflicts.

Trauma-induced acute kidney injury (AKI) has affected many U.S. warfighters throughout history. We seek to provide a historical review of the epidemiology of combat-acquired AKI and to highlight the importance of adapting current renal replacement therapy (RRT) capabilities to prepare for the next armed conflict. While severe AKI was rare in the recent conflicts in Iraq and Afghanistan, an analysis of prior wars suggests that it will be more common in future combat operations characterized by prolonged evacuation times, limited resuscitation capabilities, and delayed aeromedical evacuation. Upper extremity deep vein thrombosis in a military patient.

We describe the case of a 23-year-old serviceman on overseas deployment who presented with a painful, swollen arm. Investigations showed an upper extremity deep vein thrombosis (UEDVT) of the right arm with an associated asymptomatic pulmonary embolism, which was treated with warfarin anticoagulation. Further investigation identified positional obstruction at the thoracic outlet, and the patient was diagnosed with Paget-Schroetter syndrome. He underwent elective resection of the first rib, and has now returned to normal duties. After review of the literature on UEDVT, it is suggested that in t

Emergency canine surgery in a deployed forward surgical team: a case report.

INTRODUCTION: Forward surgical teams (FSTs) perform a variety of non-doctrinal functions. During their deployment to Afghanistan, the 541st FST (Airborne) performed emergency surgery on a German shepherd military working dog (MWD). METHODS: Retrospective examination of a case of veterinary surgery in a deployed FST. RESULTS: A 5 1/2-year-old German shepherd MWD presented with extreme lethargy, tachycardia, excessive drooling, and a firm, distended abdomen. These conditions resulted from gastric dilatation with volvulus. Since evacuation to a veterinarian was untenable, emergency laparotomy w

[Surgical teams in extraordinary stress environments : Education, mental resilience and simulation-based training].

Extraordinary stress situations create a dual challenge for surgical teams: they must make technical decisions under time pressure while maintaining communication, leadership and situational awareness. In trauma rooms, intraoperative crises, mass casualty incidents, military deployments and resource-limited environments, risks arise due to stress, cognitive narrowing, unclear roles and impaired team coordination. The literature describes surgical skills, simulation-based education, structured debriefing and resilience training as relevant components to increase reliable team performance. This …

Leishmaniasis: new insights from an old and neglected disease.

Leishmaniases are a clinically heterogeneous group of diseases caused by protozoa of the genus Leishmania. There is growing evidence that the true incidence of the disease is underestimated, especially in hyperendemic regions. Moreover, climate changes together with the increasing movement of humans and animals raise concerns about the possible introduction of Leishmania infection in previously spared areas. The disease is emerging in immunocompromised patients undergoing bone marrow or solid organ transplantation or treatment with biologic drugs. Furthermore, the deployment of military troops

PCR for rapid diagnosis of acute Q fever at a combat support hospital in Iraq.

Acute Q fever is occasionally seen in U.S. military service members deployed to Iraq. Diagnosis relies on serology, which is not available in the combat zone. Improved diagnostic modalities are needed. We performed a pilot study investigating whether Joint Biological Agent Identification and Diagnostic System (JBAIDS), a ruggedized, deployable polymerase chain reaction (PCR) platform, might be useful in the diagnosis of acute Q fever. Patients presenting to a Combat Support Hospital in Iraq with undifferentiated fever had blood drawn for Q fever PCR and these results were compared with serolog

Validation and operational application of a rapid method for rabies antigen detection.

The Veterinary Laboratory Europe (VLE) deployed a direct, rapid immunohistochemical test (DRIT), developed by the Centers for Disease Control and Prevention (CDC), to the US military veterinary units in Iraq and Afghanistan. The test detects rabies virus antigen in fresh and frozen brainstem samples from various animal species. The goal was to conduct surveillance in 2 countries without current rabies diagnostic capabilities and little historical data on rabies prevalence. During the deployment the DRIT was evaluated, and Veterinary Corps officers and technicians were trained to operate the te

Emergency response to an anthrax attack.

We developed a mathematical model to compare various emergency responses in the event of an airborne anthrax attack. The system consists of an atmospheric dispersion model, an age-dependent dose-response model, a disease progression model, and a set of spatially distributed two-stage queueing systems consisting of antibiotic distribution and hospital care. Our results underscore the need for the extremely aggressive and timely use of oral antibiotics by all asymptomatics in the exposure region, distributed either preattack or by nonprofessionals postattack, and the creation of surge capacity f

Efficacy of Conservative Interventions for Musculoskeletal Conditions on Pain and Disability in Active Serving Military Personnel-A Systematic Review.

INTRODUCTION: Musculoskeletal (MSK) injuries and associated pain disorders are one of the leading causes for soldiers not being medically fit for deployment, impacting force capability and readiness. Musculoskeletal pain continues to be a leading cause of disability within military services and is associated with a substantial financial burden. A better understanding of the effectiveness of MSK pain management strategies is required. This review was designed to determine the efficacy of nonsurgical interventions, such as physiotherapy, exercise, pharmacology, and multidisciplinary programs, to

Burns and military clothing.

Burn injury is a ubiquitous threat in the military environment. The risks during combat are well recognised, but the handling of fuel, oil, munitions and other hot or flammable materials during peacetime deployment and training also imposes an inherent risk of accidental burn injury. Over the last hundred years, the burn threat in combat has ranged from nuclear weapons to small shoulder-launched missiles. Materials such as napalm and white phosphorus plainly present a risk of burn, but the threat extends to encompass personnel in vehicles attacked by anti-armour weapons, large missiles, fuel-a

Chapter 8 Military Personnel With Traumatic Brain Injuries and Insomnia Have Reductions in PTSD and Improved Perceived Health Following Sleep Restoration: A Relationship Moderated by Inflammation.

BACKGROUND: Up to one-third of deployed military personnel sustain a traumatic brain injury (TBI). TBIs and the stress of deployment contribute to the vulnerability for chronic sleep disturbance, resulting in high rates of insomnia diagnoses as well as symptoms of posttraumatic stress disorder (PTSD), depression, and declines in health-related quality of life (HRQOL). Inflammation is associated with insomnia; however, the impact of sleep changes on comorbid symptoms and inflammation in this population is unknown. METHODS: In this study, we examined the relationship between reported sleep chan

System Evaluation of Optical Screening and Conditioning for Injury Resilience.

INTRODUCTION: Non-combat musculoskeletal injuries (MSKIs) are the leading cause of medical disability across the military, with poor exercise form-including misalignment and mistiming-being key contributors. Rehabilitation and conditioning can build resilience and reduce risk. Our recent work resulted in a prototype Optical Screening and Conditioning for Injury Resilience (OSCIR) system offering real-time, non-contact, biofeedback through field-deployable motion capture equipped with three-dimensional (3D) camera-based technology and an augmented reality (AR) viewer for tracking exercise form

The mental health needs of military service members and veterans.

The prevalence in active duty military service members of 30-day DSM-IV psychiatric disorders, including posttraumatic stress disorders and major depressive disorder, is greater than among sociodemographically-matched civilians. Only 23-40% of returning military who met strict criteria for any mental health problem in 2004 had received professional help in the past year. One-fourth of Regular Army soldiers meet criteria for a 30-day DSM-IV mental disorder, two-thirds of whom report a pre-enlistment age of onset. Both pre- and post-enlistment age of onset are predictors of severe role impairmen
