Critical points for sepsis management at the patient bedside.

Following an interpretative philosophy, dyna-mic and faithful to the complexity theory, a clinical pathway is outlined close to the reality, at the patient bedside, that is comprehensive of the diagnostic process in its temporal dynamism, of the therapeutic process in its specificity (antibiotic therapy, surgical souce control), in the use of organs supportive therapy (haemodynamic, respiratory, renal, etc.) and in the use of adjunctive and immunomodulatory therapies (APC, AT, etc.). The importance of the contextual activation of microbiological, immunological and coagulative monitorings is un

Neuromonitoring: brain oxygenation and microdialysis.

Patients with cerebral lesions run a high risk of developing cerebral hypoxic and ischemic damage due to secondary insults. To minimize the risk of secondary cerebral hypoxia and ischemia, new monitoring techniques of cerebral oxygenation and metabolism have been developed and may help to understand the pathophysiology of secondary brain damage for a better treatment and outcome in critical patients. Cerebral microdialysis is a relatively new technique for measuring brain molecules of the extracellular space. The technical aspects, the interpretation of the commonly measured parameters, the us

Multimodal monitoring and assessment of cerebral haemodynamic reserve after severe head injury.

This article contains an overview of selected clinical techniques employed for neurointensive care monitoring and testing of cerebral autoregulation of patients following severe head injury. Multiple modalities are used for monitoring of cerebral haemodynamic reserve, including intracranial pressure, cerebral perfusion pressure (CPP), blood flow velocity (FV) in the middle cerebral artery (MCA), jugular bulb oxygen saturation, laser-Doppler cortical flowmetry, near infrared spectroscopy of cerebral cortex, tissue oxygenation, and microdialysis. Large volumes of information demand specialised c

Transcutaneous bilirubinometry in Chinese, Malay and Indian infants.

The objective of this study was to investigate the correlation of transcutaneous bilirubinometry (TcB) and plasma bilirubin concentrations in full-term Chinese, Malay and Indian infants. TcB was performed with the Minolta Airshields bilirubinometer on Chinese, Malay and Indian fullterm infants. The readings were taken on the chest (sternum) and forehead (glabella) when capillary blood was sampled for bilirubin determination. Five hundred and forty TcB indices in 253 Chinese infants, 282 in 169 Malay infants, and 182 in 120 Indian infants were obtained over the sternum and forehead. A good corr

Adult respiratory distress syndrome following non-thoracic skeletal trauma.

Thirty adult patients with severe non-thoracic skeletal trauma were monitored over the first five days for evidence of adult respiratory distress syndrome (ARDS). Three of the patients developed progressive respiratory distress, refractory hypoxaemia and pulmonary infiltrates suggestive of ARDS. Two patients died while one recovered completely. Close monitoring is suggested for early recognition of the disease and institution of therapy.

Lidocaine in refractory status epilepticus: confirmation of efficacy with continuous EEG monitoring.

Lidocaine was efficacious in 2 patients with refractory status epilepticus (RSE) unresponsive to several antiepileptic drugs (AEDs), including high-dose barbiturates. We confirmed the efficacy of lidocaine with, for the first time in adults, continuous EEG monitoring. Lidocaine, when properly used, may be a treatment option in RSE.

The intensive care management of severe pregnancy induced hypertension.

Severe pregnancy induced hypertension (PIH, pre eclampsia) is a disease which is now treated in the intensive care unit rather than with sedation in a dark room. The pathophysiology is now well understood and allows for better and more effective management. This paper looks at the strict haemodynamic monitoring and management required to prevent complications such as eclampsia, DIC, HELLP syndrome, maternal and foetal death. The nurse's role in the management of severe PIH is discussed.

External monitoring of vascularized jejunum transfers using laser Doppler flowmetry.

Any buried free tissue transfer presents a dilemma for postoperative monitoring, and the vascularized jejunum as used for head and neck reconstruction is no exception. Exteriorization of a small common segment of the jejunum in a sense permits direct visualization. Objective assessment may then simultaneously be possible with a harmless external probe sewn onto the serosa. The laser Doppler blood perfusion monitor as one such device provides a simple means for continuous analysis of the adequacy of the jejunal circulation. Intraperitoneal in situ monitoring of isolated island jejunal segments

Burn shock.

The approach to fluid resuscitation in burn shock continues to be refined in step with improved knowledge of the complex fluid, electrolyte, and protein shifts that characterize this form of shock. Local burn tissue and generalized nonburn tissue edema occur initially after injury because of the release of histamine, which causes increased microvascular permeability. Subsequent edema formation in burned and nonburned tissue occurs according to distinctly different mechanisms. Burn tissue edema forms because of direct thermal injury to endothelial cells and increased burn tissue osmolarity. Non

Reduced venous admixture in hemorrhagic hypovolemia: maintenance of arterial oxygenation by selective pulmonary vascular collapse.

In nine anesthetized and ventilated swine, a microcomputer calculated cardiac output, venous admixture (Qsp/Qt) and physiologic deadspace (VD/VT) every 20 sec, utilizing dual oximetry and a gas exchange analyzer. After lung injury with ethchlorvynol (ECV), animals were bled 40% blood volume over 40 min. Mean cardiac output decreased 7.0 to 2.2 L/min (p less than .05) accompanied by a decrease in mean Qsp/Qt from 0.28 to 0.14 (p less than .05) and an increase in mean VD/VT from 0.39 to 0.54 (p less than .05). Arterial Hgb saturation (Sao2) increased from 88 +/- 7% to 90 +/- 6%. On regression of

[Evaluation of noninvasive monitoring of preoxygenation].

Administration of 100% oxygen before a "rapid-sequence" induction of anesthesia is recommended to prevent hypoxemia during induction. In the present study, we used a laser scattering analyzer to study the effectiveness of nitrogen washout from the lungs with oxygen wash into the lungs under two different preoxygenation regimens; 4 times of maximal breathing of 100% oxygen in one minute and normal tidal breathing of 100% oxygen for 3 minutes. The volunteers were healthy, ASA physical status 1, 22 to 33 years of age (26 +/- 3), 167 +/- 5 cm tall, and weighing 60 +/- 5 kg. Arterial blood saturati

Infections of CSF shunt and intracranial pressure monitoring devices.

Management of shunt infections varies, but externalization of shunt apparatus and the need for intraventricular antibiotics seem to have been rather uniformly adopted. No recent studies on EVD systems have been reported, largely because of the widespread use of alternatives such as the fiberoptic systems and the hollow subarachnoid bolt. Studies from the 1970s and personal experience with long-term monitoring in children with Reye's syndrome and head trauma indicate that it can be done safely with a low risk of infection. What the defining variable(s) for that low infection rate might be is no

[Diagnostic and therapeutic significance of automatic circadian monitoring in hypertension].

A noninvasive, fully automatic blood pressure monitoring system has been developed to make the measurement of blood pressure more reliable and to help individualizing anti-hypertensive therapy. The authors draw attention to the importance of the circadian blood pressure variability and the "white coat hypertension" phenomenon. The error due to overlooking the changes in circadian blood pressure and to the "white coat" effect could be avoided with automatic blood pressure monitoring. Their system consists of two subunits: the Nippon Colin (BP 203 MII) blood pressure monitor, which has been fitt

[Non-invasive ambulatory recording of blood pressure. Current data].

Recent advances in medical technology have made it possible to non invasively record ambulatory blood pressure over 24 hours. This new method of measurement has a lot of advantages over the classic way: it enables a satisfactory approach of blood pressure circadian rhythm; it seems to be more reliable than clinic readings of blood pressure, which may be subjects to some confounding variables (e.g. observer bias, digit preference, defence reaction...); it appears to be better at predicting cardiovascular prognosis of hypertensive patients and is more closely correlated with target organ damage;

Clinical use of ambulatory blood pressure monitoring.

This paper describes the alerting reaction and blood pressure rise induced in the patient when blood pressure measurement is taken by the doctor, emphasizing that this may be responsible for important errors in estimating the real untreated or treated blood pressure in hypertension. It then examines the advantages offered by ambulatory blood pressure monitoring. It is shown that when performed intra-arterially this approach may be useful in the diagnosis of pheochromocytoma. However, its wider use as a means to better discriminate between normotensive and borderline or mild hypertensive subjec

[Uses and risks of hemodynamic monitoring by inflow catheter in acute myocardial infarct].

Using hemodynamic monitoring by flow-directed heart catheterization in acute myocardial infarction, left ventricular dysfunction can be analyzed and quantified. Differential therapy concerning substitution of volume and administration of drugs with influence on pre- and after-load is facilitated. Since the introduction of the Swan-Ganz flow-directed catheter the monitoring has been technically improved with the additional advantage of lower incidence of catheter-induced arrhythmias and the possibility to measure the pulmonary capillary wedge pressure. The Swan-Ganz thermodilution catheter furt
