Results of the two incidence screenings in the National Lung Screening Trial.

BACKGROUND: The National Lung Screening Trial was conducted to determine whether three annual screenings (rounds T0, T1, and T2) with low-dose helical computed tomography (CT), as compared with chest radiography, could reduce mortality from lung cancer. We present detailed findings from the first two incidence screenings (rounds T1 and T2). METHODS: We evaluated the rate of adherence of the participants to the screening protocol, the results of screening and downstream diagnostic tests, features of the lung-cancer cases, and first-line treatments, and we estimated the performance characterist

[A Case of Small Thymic Carcinoma with Pleural Dissemination Preoperatively Suspected as Pulmonary Metastasis from Rectal Cancer].

A case was 73-year-old man, who had history of laparoscopic high anterior resection surgery for rectal cancer, followed by adjuvant chemotherapy 2 years ago. Preoperative diagnosis was anterior mediastinal tumor, with multiple intrapulmonary nodules noted, though no increasing tendency. During adjuvant chemotherapy for colorectal cancer, the anterior mediastinal tumor showed some shrinkage, while that and 3 intrapulmonary nodules slowly increased in size after completion, thus rectal cancer pulmonary and mediastinal metastasis were suspected. Complete resection of the intrapulmonary nodules an

[Pulmonary Epithelial Hemangioendothelioma Needed to Differentiate from Metastatic Lung Tumor;Report of a Case].

Pulmonary epithelioid hemangioendthelioma(PEH) is rare malignant disease known to originate from hemangioendthelial cells. We report a case of PEH that is difficult to differential diagnosis on image. A 55-year-old woman was diagnosed with malignant lymphoma 15 years ago and had been followed-up. She referred to our hospital due to abnormal shadow on chest X-ray. Multiple nodules of 0.2~1.0 cm in size with clear but irregular margin were found in bilateral lungs on chest computed tomography(CT) scan. Nodules were found to be increased both in size and in number compared to those of 10 years ag

Primary pulmonary peripheral T-cell lymphoma: A case report and review of the literature.

Primary pulmonary T-cell lymphoma is a rare condition characterized by fever, cough, dyspnea, and bilateral pulmonary nodules, usually diagnosed by transbronchial biopsy or computed tomography (CT) guided needle biopsy and pathology. In view of its poor prognosis, it is critical to distinguish and diagnose this disease as early as possible. We report here a case of a 39-year-old man who presented with fever, cough, expectoration with multiple nodules, and patch shadow in both lungs by chest CT. Tissue samples were taken by CT guided needle biopsy. The histological sample and immunohistochemist

An old woman with weight loss and chest pain.

Pulmonary sarcomas constitute only 0.1-0.5% of all primary lung malignancies. These tumors may derive from the lung parenchyma, bronchial tree or pulmonary arteries. The most important entity in the differential diagnosis is metastatic synovial sarcoma. A 76-years-old woman was admitted for investigation of a fever, productive cough, dyspnea, weight loss and left-sided chest pain which had been present for one month. A chest computerised tomography showed enlarged mediastinal lymph nodes were observed, as well as a left-sided pleural effusion. Thoracentesis revealed hemorrhagic pleural effusio

Disseminated histoplasmosis presenting as primary adrenal insufficiency in an immunocompetent host in the United States: a case report.

BACKGROUND: Histoplasma capsulatum is a dimorphic fungus that is mostly seen in immunocompromised individuals. Its presentation ranges from acute pulmonary infections to disseminated disease that can affect multiple organ systems, including the adrenal glands. Dissemination is uncommon in immunocompetent hosts, but involvement of the adrenal glands is largely reported in those with a competent immune system. Subsequent primary adrenal insufficiency is a rare finding, especially in the United States with only two such cases reported before. CASE PRESENTATION: We report the case of 62-year-old i

Atrial tachyarrhythmia as a presenting symptom leading to the diagnosis of pulmonary sarcoidosis treated with catheter-based ablation.

A 55-year-old woman was referred for recurrent palpitations. A 48-hour ambulatory cardiac monitor revealed an atrial tachycardia rate up to 170 beats per minute. A subsequent electrophysiology study revealed atrial fibrillation and both typical and atypical atrial flutter. Computed tomography revealed multiple pulmonary nodules, and an endobronchial ultrasound-guided fine-needle aspiration confirmed the diagnosis of pulmonary sarcoidosis. The patient underwent radiofrequency ablation of the cavotricuspid isthmus and left common, right superior, and right inferior pulmonary vein isolation via c…

FDG PET/CT findings in 3 cases of pulmonary peripheral T-cell lymphoma.

Pulmonary involvement of peripheral T-cell lymphoma is rare. It is difficult to make a clinical and radiologic diagnosis of pulmonary lymphomatous involvement, yet the diagnosis is often critical in determining treatment. We present 3 cases of pulmonary peripheral T-cell lymphoma with FDG PET/CT findings. In these 3 cases, FDG PET/CT images revealed multiple bilateral FDG-avid nodules, masses, or mass-like consolidations with or without air bronchogram. Familiarity with these imaging features may be helpful for the diagnosis and differential diagnosis.

Pulmonary hyalinizing granuloma.

Twenty patients with pulmonary nodules consisting of concentric hyaline lamellae, usually accompanied by perivascular collections of plasma cells and lymphocytes, were studied. In most instances, the lesions were multiple, bilateral, and mildly symptomatic. Many of these nodules showed all of the staining characteristics of amyloid, but others had an atypical birefringence pattern. No infectious agents were identified, and no consistent pattern of dysproteinemia was observed. Two of the patients had prior histories of tuberculosis. In other cases, the nodules were of unknown origin and pathoge

MRI appearance of pulmonary Wegener's granulomatosis with concomitant splenic infarction.

The MRI appearance of pulmonary lesions in a patient with Wegener's granulomatosis is presented. Pulmonary involvement was characterized by multiple solid and cavitated nodules connected to vessels. Transbronchial biopsy demonstrated typical findings of granuloma. The spleen presented diffuse low signal intensity and an enhancing capsule on T1-weighted images, and inhomogeneously high signal intensity on T2-weighted images. This appearance of diffuse infarction could result from the vasculitis caused by Wegener's disease.

Performance of Lung Cancer Prediction Models for Screening-detected, Incidental, and Biopsied Pulmonary Nodules.

Purpose To evaluate the performance of eight lung cancer prediction models on patient cohorts with screening-detected, incidentally detected, and bronchoscopically biopsied pulmonary nodules. Materials and Methods This study retrospectively evaluated promising predictive models for lung cancer prediction in three clinical settings: lung cancer screening with low-dose CT, incidentally detected pulmonary nodules, and nodules deemed suspicious enough to warrant a biopsy. The area under the receiver operating characteristic curve of eight validated models, including logistic regressions on clinica

A 57-Year-Old Man With Persistent Miliary Pattern Pulmonary Nodules and New Lung Masses on Chest CT Scan.

A 57-year-old man presented to the hospital with a 6-year history of slowly progressive persistent cough, shortness of breath, and wheezing. He had been prescribed budesonide and oxygen therapy 2 months prior for presumable asthma, but his symptoms had not subsided. He had been intermittently using 2 L oxygen via nasal canula with exertional activities until the day of hospitalization. He first experienced similar symptoms 5 years prior, and a workup at that time showed multiple tiny pulmonary nodules. His history was notable for a remote 27-pack-year smoking history, quitting 10 years prior,

Robotic-assisted Navigation Bronchoscopy: A Meta-Analysis of Diagnostic Yield and Complications.

BACKGROUND: Robotic-assisted navigation bronchoscopy (RANB) is a novel method to biopsy lung nodules, with initial reports demonstrating excellent accuracy. We aimed to evaluate pooled estimates of diagnostic yields and complication rates with RANB by performing a meta-analysis of the available literature. METHODS: We searched 3 databases, including PubMed, EmBase, and Web of Science. The resulting abstracts were reviewed by 2 investigators. Analyses were performed using random effects models, and diagnostic yield and complication rates were estimated after the Freeman-Tukey transformation.

The role of computer-assisted radiographer reporting in lung cancer screening programmes.

OBJECTIVES: Successful lung cancer screening delivery requires sensitive, timely reporting of low-dose computed tomography (LDCT) scans, placing a demand on radiology resources. Trained non-radiologist readers and computer-assisted detection (CADe) software may offer strategies to optimise the use of radiology resources without loss of sensitivity. This report examines the accuracy of trained reporting radiographers using CADe support to report LDCT scans performed as part of the Lung Screen Uptake Trial (LSUT). METHODS: In this observational cohort study, two radiographers independently read

Human Pulmonary Dirofilariasis: A Review for the Clinicians.

Human pulmonary dirofilariasis (HPD) is a rare zoonotic disease caused by Dirofilaria immitis, the nematode responsible for canine cardiopulmonary dirofilariasis (dog heartworm). The incidence of HPD is on the rise throughout the world due to increased awareness and factors affecting the vector (mosquito). Humans are accidental hosts for D. immitis. Most patients are asymptomatic and present with an incidental pulmonary nodule that mimics primary or metastatic pulmonary malignancy. Some patients suffer from pulmonary and systemic symptoms in the acute phase of pneumonitis caused by pulmonary a

[Techniques to Identify Inter-segmental Planes].

There are several different approaches to identify inter-segmental planes in segmentectomy. Inflation-deflation lines can be made by inflating the lung after clumping or resecting the target bronchus, or inflate the target bronchus by jet ventilation, direct tubing, or applying a slip-knot. Indocyanine green can be injected intravenously after resecting or clumping the target pulmonary artery to identify the target segment under an infra-red thoracoscope. Indocyanine green can also be injected through a target bronchus. Virtual-assisted lung mapping (VAL-MAP) is a bronchoscopic multispot dye m

Magnetic Resonance of Pulmonary Nodules in Oncological Patients: Are We Ready to Replace Chest CT in Detecting Extrathoracic Cancer?

OBJECTIVE: This study aims to assess the accuracy of pulmonary nodule detection via MRI compared to the gold standard, CT, in patients with extrathoracic cancer. MATERIALS AND METHODS: MRI of the chest was performed on oncological patients for staging extrathoracic cancer and subsequently compared to their CT. Only the largest nodule was considered in patients with multiple nodules. Nodule size and malignancy were recorded for each modality, as well as the presence of interstitial lung disease (ILD), adenopathy, cardiomegaly, pleural effusion, pericardial effusion, and vertebral fracture.
