Mycobacterium tuberculosis resistance to antituberculosis drugs in Mozambique.

OBJECTIVE: To determine the drug resistance profile of Mycobacterium tuberculosis in Mozambique. METHODS: We analyzed secondary data from the National Tuberculosis Referral Laboratory, in the city of Maputo, Mozambique, and from the Beira Regional Tuberculosis Referral Laboratory, in the city of Beira, Mozambique. The data were based on culture-positive samples submitted to first-line drug susceptibility testing (DST) between January and December of 2011. We attempted to determine whether the frequency of DST positivity was associated with patient type or provenance. RESULTS: During the stud…

Nosocomial spread of Mycobacterium bovis in domestic cats.

Five domestic cats were euthanased owing to confirmed or suspected Mycobacterium bovis infection. The initial source of infection remains unclear. Cat A was presented to a veterinary clinic in County Kildare, Ireland, with a discharging submandibular lesion. The infection appears to have been transmitted to four other cats through direct (cats B and C living in the same household as cat A) and non-direct (nosocomial spread during routine operations; cats D and E) contact over a 13.5-week period. Of the five cases, two (B and D) had post-mortem examinations in which gross changes consistent wit…

Lack of antimicrobial bactericidal activity in Mycobacterium abscessus.

Antibiotic therapy of infections caused by the emerging pathogen Mycobacterium abscessus is challenging due to the organism's natural resistance toward most clinically available antimicrobials. We investigated the bactericidal activity of antibiotics commonly administered in M. abscessus infections in order to better understand the poor therapeutic outcome. Time-kill curves were generated for clinical M. abscessus isolates, Mycobacterium smegmatis, and Escherichia coli by using antibiotics commonly categorized as bactericidal (amikacin and moxifloxacin) or bacteriostatic (tigecycline and linez…

Structure of Mycobacterium smegmatis Eis in complex with paromomycin.

The Rv2416c gene of Mycobacterium tuberculosis (Mtb) encodes the enhanced intracellular survival (Eis) protein that enhances intracellular survival of the pathogen in host macrophages during infection. The Mtb Eis protein is released into the cytoplasm of the phagocyte during intracellular infection and modulates the host immune response. It also contributes to drug resistance by acetylating multiple amine groups of aminoglycosides. Interestingly, the nonpathogenic M. smegmatis (Msm) contains a homologous eis gene (MSMEG_3513). The overall structures of Mtb Eis and Msm Eis are highly similar t…

The human antibody response to the surface of Mycobacterium tuberculosis.

BACKGROUND: Vaccine-induced human antibodies to surface components of Haemophilus influenzae and Streptococcus pneumonia are correlated with protection. Monoclonal antibodies to surface components of Mycobacterium tuberculosis are also protective in animal models. We have characterized human antibodies that bind to the surface of live M. tuberculosis. METHODS: Plasma from humans with latent tuberculosis (TB) infection (n = 23), active TB disease (n = 40), and uninfected controls (n = 9) were assayed by ELISA for reactivity to the live M. tuberculosis surface and to inactivated M. tuberculosis…

Prednisolone and Mycobacterium indicus pranii in tuberculous pericarditis.

BACKGROUND: Tuberculous pericarditis is associated with high morbidity and mortality even if antituberculosis therapy is administered. We evaluated the effects of adjunctive glucocorticoid therapy and Mycobacterium indicus pranii immunotherapy in patients with tuberculous pericarditis. METHODS: Using a 2-by-2 factorial design, we randomly assigned 1400 adults with definite or probable tuberculous pericarditis to either prednisolone or placebo for 6 weeks and to either M. indicus pranii or placebo, administered in five injections over the course of 3 months. Two thirds of the participants had …

Endobronchial Mycobacterium avium Infection in an Immunocompetent Patient.

Although Mycobacterium avium complex (MAC) is the most common pathogen in nontuberculous mycobacterial (NTM) pulmonary diseases, endobronchial lesions caused by MAC infections are very rare even in an immunocompromised host. Herein, we describe the case of a 59-year-old, HIV-negative and non-immunocompromised woman who developed multifocal pulmonary infiltrations with endobronchial lesion caused by M. avium. Bronchoscopic examination revealed white- and yellow-colored irregular mucosal lesions in the bronchus of the left lingular division. M. avium was identified using sputum culture and bronc…

Mycobacterium simiae: a possible emerging pathogen in Iran.

Mycobacterium simiae has been reported worldwide, particularly from the Middle East. This organism has been recognized as a causative agent of pulmonary and disseminated infections. In this study, we used molecular methods to detect this organism from patients who were suspected of having tuberculosis. A total of 117 isolates of mycobacteria were evaluated from different regions of Iran. Isolates were identified using phenotypic methods and gene sequencing of 16S rRNA, rpoB, hsp65, and ITS. Of the 117 isolates, 12 M. simiae isolates (10.2%) were identified from different clinical samples, incl…

Identification of the leprosy agent Mycobacterium lepromatosis in Singapore.

BACKGROUND: A new leprosy-causing species, namely Mycobacterium lepromatosis, was discovered recently to be the cause of diffuse lepromatous leprosy (DLL) in Mexico. It is unknown whether this organism exists beyond Mexico. METHODS: We sought to determine the identity of the mycobacteria in the skin tissue of two patients from Singapore who died of DLL. DNA was extracted from archived biopsy tissue, and conserved polymerase chain reaction primers were used to amplify and sequence two to three mycobacterial genes in each skin sample. RESULTS: Both M. lepromatosis and the well-known leprosy ag…

Fluoroquinolone resistance in renal isolates of Mycobacterium tuberculosis.

SETTING: Alberta, Canada, 1990-2003. OBJECTIVE: Monotherapy of active tuberculosis (TB) promotes drug resistance. Given the common practice of empiric fluoroquinolone (FQ) therapy for urinary tract infections (UTI) and frequent delayed diagnosis of renal TB, we assessed urine Mycobacterium tuberculosis isolates for FQ resistance. DESIGN: Retrospective study. Urine M. tuberculosis isolates underwent FQ susceptibility testing. Records were reviewed for evidence of FQ exposure and diagnostic delay. RESULTS: Among 78 culture-positive renal TB patients between 1990 and 2003, initial isolates of …

MPB70 and MPB83--major antigens of Mycobacterium bovis.

MPB70 and MPB83 are among the most studied mycobacterial antigens. They are highly homologous proteins within Mycobacterium tuberculosis complex members, and the orthologs in different members of the complex are virtually identical. They are major antigens highly expressed by Mycobacterium bovis and considerably less abundantly expressed by M. tuberculosis. There are two genes encoding these proteins within an operon of six genes. MPB70 and MPB83 are encoded as precursor proteins with typical signal peptides for export through the general secretory pathway. MPB70 is a soluble secreted protein …

Mycobacterium fortuitum infection masquerading as an orbital mass.

Orbital infection with nontuberculous mycobacteria is rare and usually presents as an inflammatory process. We report a case of a 34-year-old male hospital worker with a positive purified protein derivative skin test who had Mycobacterium fortuitum infection that presented as an orbital mass causing diplopia. This presentation is unlike previously published reports and demonstrates the importance of orbital biopsy and tissue culture.

Seroprevalence of Mycobacterium pneumoniae in healthy adolescents in Taiwan.

This study was designed to understand the seroprevalence of Mycoplasma pneumoniae infection in healthy Taiwanese adolescents. The study included 2,233 college freshmen (female:male = 1.29:1; mean age, 19.7 years). The percentages of subjects residing in northern, central, southern, and eastern Taiwan were 66.91, 15.89, 9.0, and 8.2%, respectively. All enrolled subjects underwent a serologic agglutination test to detect serum concentrations of antibodies to M. pneumoniae. The test results showed that 19.84% of the subjects were infected and, of those, 6.0% were estimated to have subsequently ac

Novel agents in the management of Mycobacterium tuberculosis disease.

The goals of tuberculosis control are to cure active disease, prevent relapse, reduce transmission and avert the emergence of drug resistance. However, since the 1960s, there have been few developments in available therapies. Currently available agents are complicated by numerous side-effects, drug interactions and the need for a long duration of therapy. Rifampicin-containing regimes lead to hepatic enzyme induction which can complicate or preclude the use of protease inhibitors and non-nucleoside reverse transcriptase inhibitors in patients infected with the human immunodeficiency virus. Fur

[Can optic perineuritis be triggered by Mycobacterium tuberculosis?].

A 38-year-old woman presented with a swollen left optic disc while being treated for miliary tuberculosis. The diagnosis of optic perineuritis was made, antibiotics were adapted, and the patient slowly recovered. We discuss the elements required to confirm optic perineuritis and the differential diagnosis in cases of papilledema with no visual impairment, as well as the hypotheses on the possible etiologies: dysimmune neuropathy as opposed to infection or toxicity.
