Rhabdomyolysis is characterized by a marked elevation of the creatine kinase (CK) levels and myoglobinuria, thus leading to renal dysfunction. Various viruses or bacteria can be etiologic agents, but mycosis has only rarely been reported to be a cause of rhabdomyolysis. In this report, we describe an adolescent male with acute myeloid leukemia who underwent allogeneic bone marrow transplantation and thereafter developed rhabdomyolysis and Candida parapsilosis fungemia almost at the same time. Rhabdomyolysis is a clinical and biochemical syndrome resulting from skeletal muscle injury, which may ultimately lead to acute renal failure (ARF) and death. Exertional rhabdomyolysis refers to skeletal muscle injury that is usually induced by strenuous eccentric exercises in a hot and humid environment. Extracorporeal shock-wave lithotripsy (ESWL) is the treatment of choice for the majority of patients with renal or proximal ureteral stones. We describe an unusual case of anuric renal failure following ESWL, in absence of obstruction or myoglobinuria, in the presence of unilateral nephrolithiasis and two normally functioning kidneys. A 17-year-old boy had exercise-induced cramps and myoglobinuria. The mother had myalgia and weakness after exercise but the father was asymptomatic. Muscle biopsy was normal in the father but showed glycogen storage and absent or markedly decreased histochemical stain for phosphorylase in mother and son. A 16-year-old male presenting with anticholinergic symptoms was found to have hematuria and oliguria. Evaluation of the patient revealed a serum creatinine of 2.2 mg/dl, myoglobinuria, and a creatine phosphokinase (CPK) level of 78, 750 IU/l with 99 percent fraction 3 isoenzyme. Two cases of lightning injury which occurred while mountain climbing and took different clinical courses are reported. One case with lightning marks on the abdomen was treated as a crush injury because of myoglobinuria and elevation of serum glutamic oxaloacetic transaminase, lactic dehydrogenase and creatine phosphokinase. Three unusual cases are described, in which, in separate incidents, young snake bite victims were found comatose. Tiger snake vemon was detected by radioimmunoassay in the urine and serum, and in one case on the clothing of these children. Gross myoglobinuria was present in all cases. The three cases demonstrate clearly the myolytic potential of tiger snake venom if specific treatment (administration of antivenom) is delayed.
