This study aimed to evaluate the effectiveness and safety of retrograde intrarenal surgery (RIRS) compared with mini-percutaneous nephrolithotomy (mini-PCNL) in children with renal pelvic stones < 2 cm and density > 1000 HU. A prospective randomized clinical trial was conducted at Beni-Suef University Hospitals including 70 children aged 3–14 years. Eligible participants had a single renal pelvic stone < 2 cm with density > 1000 HU on low-dose non-contrast CT and were randomized equally to undergo RIRS or mini-PCNL (n = 35 each). The stone-free rates at one month were 91.4% for RIRS and 94.3%

BACKGROUND AND OBJECTIVE: When performing retrograde intrarenal surgery (RIRS), compared with conventional ureteral access sheaths (UAS), flexible and navigable suction ureteral access sheaths (FANs) can reduce the number of lithotripsy sessions and increase the stone-free rate (SFR). They have been widely applied in flexible ureteroscopic lithotripsy (FURL). Currently, ureteroscopic flexible ureteroscopes equipped with FANs are also showing remarkable outcomes in the treatment of large kidney stones. Therefore, the objective of this study was to explore the efficacy of single-use flexible ure

BACKGROUND: Mini-percutaneous nephrolithotomy (mini-PCNL) is widely used for the management of renal calculi, and postoperative infectious complications remain a concern. Although the addition of suction mitigates the risk of infectious complications, certain patients still develop sepsis, which can be life-threatening. Accurate early prediction of sepsis could improve patient outcomes and guide perioperative management. This study aimed to develop and validate an artificial intelligence (AI)-based model to predict postoperative sepsis in patients undergoing suction mini-PCNL. METHODS: Data we

Percutaneous nephrolithotomy has proved to be a cost-effective, safe and expeditious means of renal calculus removal since its introduction in the late 1970s. The inherent risks and complications of this procedure have been well documented but less common complications continue to surface as case selection becomes increasingly more difficult. We report a case of chyluria after percutaneous nephrolithotomy. The patient recovered completely after treatment with total parenteral nutrition and required no further intervention. The etiology as well as the treatment of this previously unreported com

INTRODUCTION: Renal stones are common, with a lifetime prevalence of 10% in adults. Global incidence is increasing due to increases in obesity and diabetes, with these patient populations being more likely to suffer renal stone disease. Flank pain from stones (renal colic) is the most common cause of emergency admission to UK urology departments. Stones most commonly develop in the lower pole of the kidney (in ~35% of cases) and here are least likely to pass without intervention. Currently there are three technologies available within the UK National Health Service to remove lower pole kidney

BACKGROUND: Percutaneous nephrolithotomy (PCNL) is widely regarded as the first-line treatment for upper urinary tract stones larger than 2 cm, as recommended by major urological associations. However, a growing body of research highlights the safety and effectiveness of retrograde intrarenal surgery (RIRS) for managing stones of this size in the upper urinary tract, especially with the advent of suction-assisted techniques in RIRS. This study aims to systematically evaluate the safety and efficacy of negative pressure aspiration-assisted RIRS (NPAA-RIRS) in comparison to traditional PCNL.

Percutaneous nephrolithotomy is a widely accepted treatment for urinary calculi, but it is not without complications. We present the case of a 76-year-old male with a retained council tip catheter after percutaneous nephrolithotomy. Fluoroscopic guidance was used to perform percutaneous puncture of the catheter balloon, and the catheter was removed without complication. Advantages of various nephrostomy tube designs and additional measures to prevent this type of complication are discussed.

BACKGROUND: The optimal management of 2-3 cm renal stones remains challenging because effective stone clearance must be balanced against perioperative morbidity. Mini-percutaneous nephrolithotomy (mPCNL) provides high stone-free rates (SFR) but is associated with greater procedural burden, whereas flexible ureteroscopic lithotripsy (fURL) is less invasive but may be limited by fragment clearance efficiency. A flexible vacuum-assisted ureteral access sheath (FV-UAS) has recently been introduced to facilitate fragment evacuation and improve intrarenal drainage during fURL.

Renal pelvis epidermoid cysts are extremely rare and are often misdiagnosed as stones or tumors. Only one previous case has been treated using the percutaneous approach. A 67-year-old female with diabetes who was receiving empagliflozin presented with imaging findings suggestive of a staghorn calculus. During percutaneous nephrolithotomy, whitish keratinous material was found in the renal pelvis, along with a small obstructing stone. All material was evacuated through the Amplatz sheath. Urine culture was negative. Histopathological examination confirmed an epidermoid cyst. A postoperative com

OBJECTIVES: To determine the differential effects on renal pressures and irrigation flow associated with the application of different ureteral catheters during percutaneous nephrolithotomy.
METHODS: Using ex vivo fresh cadaveric tissue, we established a percutaneous nephrolithotomy model. After obtaining lower pole percutaneous access, we recorded the pressure and irrigant flow measurements. Measurements were made with an empty ureter, 6F ureteral catheter, occlusion balloon catheter, and ureteral access sheaths (10/12F and 12/14F). Three 1-minute trials for each condition were recorded in ea

Pyeloduodenal fistula is a rare event, which can occur spontaneously or be caused by trauma. We present two such cases, of which, one was complicated by percutaneous nephrolithotomy for a right renal stone and the other occurred spontaneously from right renal and perirenal abscesses. Its etiology, pathogenesis, clinical manifestations, diagnosis and treatment are discussed. We emphasize that with the increasing use of interventional uroradiologic procedures, particularly percutaneous nephrolithotomy, we may encounter more traumatic pyeloduodenal fistulae.

The introduction of percutaneous nephrolithotomy and ureterolithotomy has greatly widened the therapeutical spectrum of urolithiasis. In this paper indications and contraindications of percutaneous stone removal will be discussed as well as techniques of this procedure. 210 percutaneous stone operations were performed after an initial training period primarily using local anesthesia (neuroleptanalgesia). This operative procedure allowed stone removal in patients otherwise inoperable because of increased operation risk or of advanced age. With increasing experience the success rate has been imp

Ultrasonically-guided percutaneous nephrolithotomy (PNL) has largely replaced conventional open surgery for treatment of stones of the upper urinary tract. PNL has proved to be a safe and effective therapy of upper urinary tract calculi. The different therapeutic strategies using percutaneous nephrolithotomy, comprise PNL monotherapy (large renal pelvis stones, partial staghorn stones, stones in caliceal diverticula, stones and UPJ stenosis) and combination therapy with ESWL (complete staghorn stones). Results reveal high success rates with an acceptable risk of complications and morbidity.
