Muscle fiber conduction velocity in the diagnosis of familial hypokalemic periodic paralysis--invasive versus surface determination.

Muscle fiber conduction velocity (MFCV) in the brachial biceps muscle was determined in a large family of patients with hypokalemic periodic paralysis (HOPP) by both a surface and an invasive method. Other surface EMG parameters and the muscle force were also determined. Both the surface and the invasive method showed a significantly lower mean MFCV in the proven gene carriers but only the invasive method showed a lower MFCV in all proven carriers. It can be concluded that MFCV determination is a reliable method to detect the membrane defect in HOPP carriers and that the invasive method is not

[A case of acute polyradiculoneuropathy with autonomic disturbances following rubella infection].

A 32-year-old man developed a rash on his body and extremities following acute fever of a few days duration, and also noticed pain and spontaneous tingling sensations in his lower extremities. Because severe pneumonia with dyspnea and low arterial blood oxygen concentration were found on examination, he was admitted and treated. After recovering from pneumonia in two months, he complained of abdominal symptoms, such as constipation, nausea and vomiting, spontaneous tingling sensations in the lower extremities, and orthostatic dizziness and fainting. On neurological examination, a mild to moder

Strain, stress and stretch of peripheral nerve. Rabbit experiments in vitro and in vivo.

Mechanical stretching is known to cause morphologic and functional changes in peripheral nerve. The points at which these changes occur, however, are not clearly defined and reported data are conflicting. The studies presented in this paper provide a basic understanding of the biomechanical properties, stretch-injury patterns, and changes of conduction properties of peripheral nerves due to stretching. Our studies showed that peripheral nerves exhibited non-linear stress-strain characteristics when placed under tension. Initially, under tension, the nerve had a low modulus that increased gradu

AAEM case report #25: anterior interosseous nerve syndrome.

A case study is reported regarding a 57-year-old woman, chose chief complaint was weakness in her thumb that she had noted while gardening. The patient described difficulty pulling weeds out because of an inability to get a firm grip when using the thumb. Physical examination showed weakness of the flexor pollicis longus and flexor digitorum profundus to the index finger. There was no other weakness and no clinical sensory deficit. Electrodiagnostic studies revealed normal median motor and sensory nerve conduction studies with needle examination abnormalities noted only in the flexor pollicis

Intrafamilial phenotype variation in Friedreich's disease: possible exceptions to diagnostic criteria.

Three families are described which include members with "typical" Friedreich's disease (FD) and others who are ataxic but do not satisfy all the diagnostic criteria for that disease. In family A two patients have an early-onset, rapidly progressive FD, while two others have a late-onset, more benign form. In families B and C one member has "typical" FD, and another has a similar ataxic syndrome, except for preservation of knee jerks. Laboratory evaluation is consistent with the diagnosis of FD in all cases. FD diagnosis appears justified in secondary cases with late onset or preserved tendon r

Nerve conduction studies in upper limbs of patients with cervical spondylosis and motor neurone disease.

Proximal conduction studies by F-wave technique, with conventional distal motor and sensory conduction were performed along the ulnar nerves of 20 patients each with cervical spondylotic radiculopathy and/or myelopathy and with classical motor neurone disease (MND). Such F-wave parameters as shortest F-latency, F-conduction velocity, conduction time and F-ratio were calculated. Twenty-five age- and sex-matched healthy volunteers acted as controls. Proximal slowing associated with sensory conduction abnormalities and normal distal motor conduction favored cervical spondylosis (CS). Distal slowi

[Clinical amyotrophic lateral sclerosis syndromes in gammapathies: clinical, electrophysiologic and histoimmunologic aspects].

Eleven patients with motor neuron disease associated with a monoclonal gammopathy were studied. One patient had a previously known multiple myeloma. In the other patients neurological symptoms preceded diagnosis of the gammopathy by one month to 8 years. They were 5 multiple myeloma, 1 macroglobulinemia, 1 chronic lymphatic leukemia, 1 lymphoma and 3 benign monoclonal gammopathies. EMG and motor conduction velocities results were consistent with anterior horn cell disease but sensory conduction studies were abnormal in seven cases out of eight. Superficial peroneal nerve biopsy performed in al

Tarsal tunnel syndrome. A case report and review of the literature.

An illustrative case report and a review of the literature are presented. The tarsal tunnel syndrome is a relatively rare syndrome consisting of pain, paresthesias, and varying degrees of motor and vasomotor changes in any or all three branches of the posterior tibial nerve at the ankle. It represents a pressure neuropathy secondary to intrinsic or extrinsic stenosis of the tarsal tunnel, with a wide variety of underlying causes. The clinical suspicion may be confirmed by electromyography and nerve conduction studies, comparing motor and sensory action potential latency, amplitude and duration

Muscle hypertrophy and a syndrome of continuous motor unit activity in prednisone-responsive Guillain-Barré polyneuropathy.

The clinical, electrophysiological and morphological findings (light and electron microscopy of the sural nerve and gastrocnemius muscle) are reported in an unusual case of Guillain-Barré polyneuropathy with an association of muscle hypertrophy and a syndrome of continuous motor unit activity. Fasciculation, muscle stiffness, cramps, myokymia, impaired muscle relaxation and percussion myotonia, with their electromyographic accompaniments, were abolished by peripheral nerve blocking, carbamazepine, valproic acid or prednisone therapy. Muscle hypertrophy, which was confirmed by morphometric data

Changes of n-hexane neurotoxicity and its urinary metabolites by long-term co-exposure with MEK or toluene.

It is well known that the neurotoxicity of n-hexane may be modified upon co-exposure with other organic solvents. In order to elucidate this mechanism further, rats were exposed to 500ppm n-hexane, 500ppm n-hexane plus 500ppm methyl ethyl ketone (MEK), 500ppm n-hexane plus 500ppm toluene, or air only for 8h per day for 33 weeks. The body weight, motor nerve conduction velocity (MCV) and distal latency (DL) were determined before exposure and after 4, 8, 12, 16, 20, 24, 29, and 33 weeks of exposure. From each group one rat was histologically examined after 33 weeks of exposure. To establish a r

The tarsal tunnel syndrome.

Tarsal tunnel syndrome is relatively rare and the diagnosis may be difficult even though the etiologies for the syndrome are multiple. The symptoms are often vague but are usually burning pain and paresthesias in the toes and soles of the feet with nocturnal exacerbations. The physical examination may elicit a sensory deficit over the cutaneous distribution of the median and/or lateral plantar nerve. There may also be tenderness over the flexor retinaculum. Electrodiagnosis, especially with the addition of the orthodromic compound nerve action potential latency technique, is essential to confi

[Measurement of the propagation rate of the summation action potential in the vomeronasal and olfactory nerves of Rana temporaria frogs].

It was shown that summation action potential (AP) in various points of the nerve result from summation AP in various bundles of fibres. The rate of propagation of AP in various bundles may differ rather significantly. Propagation of AP from the site of stimulation takes place with acceleration, which is presumably due to the inhibitory effect of the stimulus at the site of its application (together with the excitatory effect as well). Both nerves exhibit also supernormal phenomenon which reveals itself in the increase of the propagation rate of the second AP during stimulation by paired stimul

Electrophysiologic methods as an aid in diagnosis of botulism: a review.

Electrophysiologic studies can be of help to the clinician in establishing the diagnosis of botulism. This is particularly true when serologic and toxologic confirmation cannot be obtained. Among the situations in which laboratory confirmation is lacking are cases diagnosed late and some cases of wound botulism. The expected electrophysiologic findings in botulism are as follows: (1) There is almost always a small evoked muscle action potential in response to single supramaximal nerve stimulation. (2) Post-tetanic facilitation is similar to, but less notable, than that seen in the Lambert-Eato

Use-dependent conduction block produced by volatile general anesthetic agents.

The ability of tertiary amine local anesthetic, barbiturates, and several volatile general anesthetics to block conduction of the compound action potential was measured in the frog sciatic nerve. Action potentials from large myelinated fibers were monitored using a desheathed nerve preparation in a sucrose-gap chamber. Supramaximal stimuli were applied over a range of stimulus frequencies from 0.1 Hz to 500 Hz. All three anesthetic agents produced a phasic, frequency-dependent anesthetic block, which developed with repetitive stimulation and returned to a tonic level when the nerve remained a

Isolated mechanical lesions of the sural nerve.

One case of compression neuropathy and one case of iatrogenic stretch injury to the sural nerve were demonstrated with electrophysiologic (EP) testing. Isolated mechanical injuries to the sural nerve may occur more frequently than is generally realized, and EP studies can greatly aid in the diagnosis of these injuries, thus avoiding the necessity for neuroradiologic investigations.
