Myasthenia gravis can be induced in mice by injecting the extracellular domain of rat muscle-specific kinase (MuSK), a transmembrane receptor tyrosine kinase involved in agrin signaling at the neuromuscular junction. About 5-10% of human myasthenia gravis patients have autoantibodies against MuSK. Here we have examined mouse neuromuscular junctions following MuSK immunization in two groups of muscles that can be distinguished on the basis of the timing of neuromuscular synaptogenesis and their response to perturbation of agrin signaling. We used confocal microscopy to characterize the distribu…

Myasthenia gravis is the most frequent abnormality of the neuromuscular junction in adults and children. In its milder forms satisfactory management may be obtained with oral neostigmine, but in more severely affected patients steroid therapy and thymus resection may be necessary. Congenital myasthenic syndrome is rather infrequent but timely recognition an identification are very important because treatment and prognosis vary widely in its different forms. Hypermagnesemia, botulism, organophosphate insecticides, are also highlighted among toxic and metabolic causes of neuromuscular junction a…

This article briefly reviews the physiology of the neuromuscular junction and the pharmacologic mechanisms of neuromuscular blocking agents. The clinical use of modern agents is discussed. Monitoring techniques used to assess the level of neuromuscular block and to exclude residual paralysis at the end of an anesthetic procedure are reviewed.

Myasthenia gravis (MG) is an autoimmune disease in which autoantibodies target structures within the neuromuscular junction, affecting neuromuscular transmission. Muscle-specific tyrosine kinase receptor-associated MG (MuSK-MG) is a rare, often more severe, subtype of the disease with different pathogenesis and specific clinical features. It is characterized by a more severe clinical course, more frequent complications, and often inadequate response to treatment. Here, we review the current state of knowledge about potential pathomechanisms of the MuSK-MG and their therapeutic implications as …

Interferon-alpha (IFN-alpha) therapy is frequently associated with significant fatigue, which is often the dominant dose-limiting side effect. The fatigue associated with IFN-alpha therapy is usually dose related, worsens with continued therapy, and is associated with significant depression. Although the direct cause of IFN-alpha-induced fatigue is unknown, it is possible that neuromuscular fatigue, similar to that observed in patients with postpolio syndrome, is one component of this syndrome. The induction of proinflammatory cytokines observed in patients treated with IFN-alpha is consistent

The fundamental pathology in rigid clubfoot appears to be a neuromuscular defect. In order to test this hypothesis, 90 muscle biopsies were obtained from the posteromedial, peroneal, and some intrinsic muscles at surgical correction of 13 clubfoot patients. All were examined ultrastructurally, and 48 biopsies in nine of these patients were studied using enzyme specific histochemical stains at the light microscopic level. Neuromuscular junctions were isolated in the latter group. Consistent ultrastructural abnormalities were observed in all specimens. Histochemistry revealed a dominant Type I f…

Many clinical features of autosomal centronuclear myopathies (CNM) and X-linked myotubular myopathy (XLMTM) are common to congenital myasthenic syndromes (CMS). We describe three children whose clinical and electrophysiological findings originally suggested CMS, in whom CNM was diagnosed pathologically, though not yet genetically characterised. A fourth case, with XLMTM, also showed electrophysiological features of a neuromuscular transmission defect. Three (including the XLMTM case) showed improved strength with acetylcholinesterase inhibitor treatment. We also studied neuromuscular junction …

A hereditary and congenital disease of Border Leicester lambs clinically indistinguishable from the "draft lambs" of Innes et al. (1949) but with no cerebellar lesion, is defined in terms of the ultrastructural pathology of the muscle, intramuscular nerves and neuromuscular junctions. The pathognomonic feature of these congenitally affected lambs was the occurrence of large intrafascicular muscle cells. On the basis of the mitochondrial content and Z disc thickness these were considered to be one of two subtypes of type I cell. There was evidence of muscle cell growth in all cell types between…

Aminodeoxykanamycin (Kaneudomycin) as well as other members of the aminoglycoside antibiotics, has an action at the neuromuscular junction. Kanendomycin was found to be five times less potent than gentamycin, as far as their neuromuscular blocking action on the rat diaphragm is concerned. The neuromuscular blocking action on the rat diaphragm is concerned. The neuromuscular blockade produced by Kanendomycin is not reversed by neostigmine but only by calcium chloride.

Introduction: Lambert-Eaton myasthenic syndrome is an autoimmune disease of the neuromuscular junction characterized by a presynaptic defect of neuromuscular transmission resulting in muscle weakness and fatigability. Diagnostic features are specific neurophysiological alterations and autoantibody detection. The present review is focused on the use of Amifampridine Phosphate to treat LEMS patients.Areas covered: Medline search from 1990 to 2019 was examined using the free subject terms: Lambert-Eaton myasthenic syndrome, LEMS, Amifampridine, 3,4-diaminopyridine, which were then combined with T…

Neuromuscular junction is a prototypical synapse, and most synaptic molecules expressed in the central nervous system have been first identified at the neuromuscular junction. Congenital myasthenic syndromes are caused by genetic defects of molecules expressed at the neuromuscular junction. We have identified and characterized five defective molecules. (1) Defects in choline acetyltransferase reduce acetylcholine contents at the nerve terminal. (2) Collagen Q (ColQ) anchors acetylcholinesterase at the synaptic basal lamina, and its defects cause endplate acetylcholinesterase deficiency. ColQ h…

Congenital myasthenic syndromes (CMS) result from impaired neuromuscular transmission and are due to genetic mutations in one of several genes involved in the development, function, or maintenance of the neuromuscular junction (NMJ). The clinical presentation, age of onset, and prognosis can vary significantly depending on the underlying genetic defect. Since therapeutic management should be tailored to the specific causative mutation, achieving an accurate diagnosis is essential for optimal patient care. This review summarizes the common diagnostic tests used for CMS and highlights critical f…

Myasthenia gravis is a disorder of neuromuscular junction transmission, the result of antibodies against the post-synaptic aspect of the neuromuscular junction. Its clinical hallmark is fatigable weakness of skeletal muscles, which tends to vary in location and severity among patients. It is treated with pyridostigmine, immunotherapy, and thymectomy. Treatment is often individualized according to disease severity, antibody status, comorbidities, and other factors. This review uses a question-and-answer format to provide up-to-date, high-yield, clinically relevant information on myasthenia grav…

Extracellular vesicles are endocytic origin nano-sized particles secreted by diverse cell types. These vesicles exchange information that alters the biological response of recipient cells across distant compartments. Neuromuscular diseases are a broad range of acquired or inherited progressive condition that affects the neuron cells and voluntary muscle functioning. Due to mutations and environmental factors, the muscles and neuron cells degenerate and are unable to regenerate. This review focuses on the biological significance and theranostic roles of EVs in the neuromuscular junction and cur…

PURPOSE OF REVIEW: Congenital myasthenia syndromes (CMS) are treatable, inherited disorders affecting neuromuscular transmission. We highlight that the involvement of an increasing number of proteins is making the understanding of the disease mechanisms and potential treatments progressively more complex.
RECENT FINDINGS: Although early studies identified mutations of proteins directly involved in synaptic transmission at the neuromuscular junction, recently, next-generation sequencing has facilitated the identification of many novel mutations in genes that encode proteins that have a far wid…
